[2 or 3 level blocks in the Tawara node during atrial tachycardia].

In atrial flutter (or paroxysmal atrial tachycardia), the ventricular response is dependant on the passage through 3 superposed zones of conduction in the Tawara node, the zone of decremential conduction being the central zone N. When the ventricular response is between half and a quarter of the atrial rate there are two possible explanations: type B alternate Wenckebach period (mobitz I block in the central zone N, 2/1 block at the nodo-ventricular junction) or type A alternate Wenckebach period (Mobitz I block in the central zone N and 2/1 block at the atrio-nodal junction). These two respon

Chemical cardioversion of atrial fibrillation or flutter with ibutilide in patients receiving amiodarone therapy.

BACKGROUND: Ibutilide is a class III drug that is used for the cardioversion of atrial arrhythmias, but it can cause torsade de pointes. Amiodarone also prolongs the QT interval but rarely causes torsade de pointes. There are no studies in which the concomitant use of the 2 agents was examined. The purpose of the present study was to assess the efficacy and safety of cardioversion with combination therapy in patients with atrial fibrillation or flutter. METHODS AND RESULTS: The study included 70 patients who were treated with long-term oral amiodarone and were referred for elective cardiovers

Effects of intravenous verapamil on cardiac arrhythmias and on the electrocardiogram.

The effects of intravenous verapamil on the electrocardiogram in 15 patients with heart disease in sinus rhythm and in 44 patients with supraventricular and ventricular tachyarrhythmias were evaluated. Verapamil prolonged the P-R interval without effect on the QRS duration or the Q-Tc interval. In patients with atrial flutter and fibrillation, A-V block was increased, with slowing of the ventricular rate, in almost all cases but sinus rhythm was restored in only 1 of 12 patients in atrial fibrillation and in 2 of the 11 patients with flutter. Verapamil had no effect in 3 patients with atrial f

[Systemic embolism after reversion to sinusal rhythm of persistent atrial flutter].

BACKGROUND: The incidence of embolism in atrial flutter has been underestimated in the routine clinical practice. PATIENTS AND METHODS: In this study the incidence of thromboembolic events after restoration of sinus rhythm (by catheter ablation or cardioversion) was compared in two groups of consecutive patients, with a different anticoagulation protocol. A total of 169 patients were evaluated. A first retrospective analysis of 79 non anticoagulated patients (group I). A second prospective group of 90 patients who were treated with an anticoagulation protocol (group II) similar to that for pa

Atrial flutter in the neonate and early infancy.

Atrial flutter is a rare arrhythmia in the neonate and early infancy. We retrospectively reviewed the clinical presentations, treatment and outcome of seven patients who presented clinically with atrial flutter. The age of onset ranged from 1 day to 3 months. Atrial flutter was diagnosed in the first 3 days of life in 4. Three cases presented as atrial flutter with 2:1 atrioventricular conduction and the remaining 4 with variable AV block. Heart failure was present in 3 patients and 6 patients showed normal intracardiac structure on echocardiography. Electrical cardioversion was attempted as t

Arrhythmias in the intensive care patient.

PURPOSE OF REVIEW: Atrial fibrillation, atrial flutter, AV-nodal reentry tachycardia with rapid ventricular response, atrial ectopic tachycardia, and preexcitation syndromes combined with atrial fibrillation or ventricular tachyarrhythmias are typical arrhythmias in intensive care patients. Most frequently, the diagnosis of the underlying arrhythmia is possible from the physical examination, the response to maneuvers or drugs, and the 12-lead surface electrocardiogram. In all patients with unstable hemodynamics, immediate DC-cardioversion is indicated. Conversion of atrial fibrillation to sinu

Safety and efficacy of ibutilide in cardioversion of atrial flutter and fibrillation.

This article reviews the safety and efficacy of ibutilide for use in patients with atrial fibrillation and flutter. Ibutilide, a class III antiarrhythmic agent, is primarily used for conversion of atrial flutter and fibrillation and is a good alternative to electrical cardioversion. Ibutilide has a conversion rate of up to 75% to 80% in recent-onset atrial fibrillation and flutter; the conversion rate is higher for atrial flutter than for atrial fibrillation. It is also safe in the conversion of chronic atrial fibrillation/flutter among patients receiving oral amiodarone therapy. Ibutilide pre

Idiopathic Atrial Flutter With Tachycardia-Induced Cardiomyopathy Detected at a Routine Infant Health Checkup: A Case Report.

Idiopathic atrial flutter (AFL) is an uncommon supraventricular tachyarrhythmia in early infancy. When sustained, AFL may lead to tachycardia-induced cardiomyopathy (TIC), a potentially reversible cause of ventricular dysfunction after rhythm control. We report a 3-month-old boy in whom severe TIC was detected incidentally at a routine infant health checkup despite minimal symptoms. The examining pediatrician noted persistent age-inappropriate tachycardia and referred the infant urgently. On admission, the ventricular rate fluctuated between 160 and 240 beats/min, likely reflecting variable at

Cardiac rhythm and conduction in Duchenne's muscular dystrophy: a prospective study of 20 patients.

This study was designed to determine the types and prevalence of rhythm and conduction disturbances in Duchenne's muscular dystrophy, and examine the results in light of known morphologic abnormalities of the myocardium and the small coronary arteries, and in the context of left ventricular function, regional wall motion, mitral valve prolapse and left atrial size. The observed disorders of impulse and conduction extended from the sinus node to the His-Purkinje system. Rhythm disturbances included persistent and labile sinus tachycardia (gradual or abrupt), sinus arrhythmia, sinus pauses, atri

Newly Detected Atrial Flutter After Enclomiphene Dose Escalation in a Patient With Prior Mitral Valve Repair and Chronic Immune Thrombocytopenia: A Case Report.

Enclomiphene citrate is increasingly prescribed off-label for secondary male hypogonadism, and its cardiovascular safety profile remains poorly characterised. We report a 46-year-old man with a mitral valve repair performed 20 years earlier and chronic immune thrombocytopenia (ITP) in whom atrial flutter was first documented shortly after he inadvertently increased his enclomiphene dose, taking 50 mg daily in place of 25 mg daily. Laboratory evaluation demonstrated chronic thrombocytopenia with a platelet count of 36 × 10⁹/L, while thyroid function, cardiac biomarkers, and electrolytes were no

Atrial fibrillation is common after ablation of isolated atrial flutter during long-term follow-up.

BACKGROUND: Previous studies have shown that the incidence of atrial fibrillation after atrial flutter ablation is approximately 20% among patients presenting with typical atrial flutter and no history of fibrillation. However, studies involving this population have been small, with follow-up typically less than 2 years. OBJECTIVE: The purpose of this study was to provide a more accurate perspective on the long-term risk of atrial fibrillation in patients presenting with isolated typical flutter. METHODS: Clinical records of consecutive patients who had flutter ablations at Presbyterian Medi

Electrocardiographic effects of intravenous cocaine: an experimental study in a canine model.

Cocaine abuse causes cardiac dysfunction. Acute intravenous administration of cocaine may lead to development of severe arrhythmias, conduction abnormalities, ST-T changes, and sudden death. Understanding arrhythmogenesis due to cocaine may provide a therapeutic approach to reduce morbidity and mortality. We studied the arrhythmogenic activity and other electrocardiographic abnormalities resulting from an intravenous bolus of cocaine. Baseline and postanesthetic electrocardiographic findings were compared with those after administration of intravenous bolus of various doses of cocaine hydrochl

[The termination of auricular flutter by noninvasive programmed electrical stimulation using a permanent AAI-mode pacemaker].

We describe a case of a 15-year-old boy with an atrial septal defect who three years after the direct closure of the defect presented with atrial flutter type I according to Wells's classification. He was then electrically cardioverted but presented immediately sinus bradycardia and a ventricular escape rhythm that required epicardial pacing (Intermedics 292-03 DASH) in the AAI mode. We report here the successful termination of a new episode of atrial flutter into sinus rhythm with a noninvasive programmed stimulation using his previously implanted pacing system.

Significance of supraventricular arrhythmias induced by electrophysiologic studies.

The clinical history and the findings on Holter monitoring of 767 patients (without evidence of ventricular pre-excitation on the ECG) were compared with the results of electrophysiological studies to assess the significance of supraventricular arrhythmias induced by intracardiac study. The studies were undertaken to determine the cause of syncope, conduction disturbances or tachycardia. In 570 patients (group 1) supraventricular arrhythmias were not induced. On Holter monitoring, 36 (6%) had evidence of supraventricular tachycardias, including atrial flutter, atrial fibrillation and paroxysma
