Vertebral Artery to Middle Cerebral Artery Bypass for Flow Augmentation.

BACKGROUND AND IMPORTANCE: Extracranial-intracranial bypass remains an enduring procedure for a select group of patients suffering from steno-occlusive cerebrovascular disease. Although the superficial temporal artery (STA) to middle cerebral artery (MCA) bypass is most familiar among neurosurgeons, particular circumstances preclude the use of an STA donor. In such cases, alternative revascularization strategies must be pursued. CLINICAL PRESENTATION: A 63-year-old female presented with symptoms of hemodynamic insufficiency and was found to have left common carotid artery occlusion at the ori

Ruptured vertebral artery dissecting aneurysm associated with parent artery occlusion.

A 63-year-old hypertensive man presented with vertebral artery (VA) dissection manifesting as subarachnoid hemorrhage located mainly in the posterior fossa. Left vertebral angiography on the day of hemorrhage showed complete occlusion of the left VA. Right vertebral angiography showed retrograde filling of the distal portion of the left VA and the left posterior inferior cerebellar artery, and a "double lumen"-like finding in the left VA. He was managed conservatively. Follow-up angiography on Day 29 showed spontaneous recanalization of the occlusive lesion and an almost normal arterial config

Vertebral artery pseudoaneurysm. A rare complication of subclavian artery catheterization.

The present case report details a previously unreported complication of subclavian vein catheterization, vertebral artery pseudoaneurysm. Attention to this problem was brought about in the patient by the development of stridor and dysphagia noted 5 days following placement of the subclavian catheter. A computed tomographic scan of the neck revealed a superior mediastinal, contrast-enhancing mass in the region of the right subclavian artery with a "bull's-eye" sign suggestive of a pseudoaneurysm. Arteriography subsequently proved the pseudoaneurysm to be of vertebral origin. The pseudoaneurysm

A traumatic pseudoaneurysm of the vertebral artery.

Although uncommon, vertebral artery pseudoaneurysms harbour significant risk of embolic stroke and their presence should be considered in cases of blunt cervical trauma. We illustrate a case of a traumatically ruptured vertebral artery pseudoaneurysm treated with coil embolisation.

[Aplasia of both vertebral arteries].

An angiographically verified case of aplasia of both vertebral arteries is described. Blood was supplied to the whole vertebro-basilar territory by the hypertrophied left occipital artery the direct continuation of which was the basilar artery of the brain. The article also deals with other anomalies of the vertebral arteries which are important in surgery.

Spontaneous dissections of the vertebral arteries.

Clinical and angiographic features and outcome in 25 patients with spontaneous dissections of the vertebral arteries are described. Most patients were in their fourth or fifth decade of life, and women predominated. Forty-eight percent of the patients were hypertensive. Angiographic evidence of fibromuscular dysplasia was noted in one only. Brainstem ischemic symptoms (usually a lateral medullary syndrome) and ipsilateral occipital headache and neck pain (often preceding but sometimes associated with or following the brainstem ischemic event) were the most common clinical findings. The angiogr

A dissecting aneurysm of the vertebral artery.

A rare case of dissecting aneurysm of the vertebral artery evidenced by ischemic attacks is reported. The aneurysm was treated by proximal ligation of the artery. There have been no repeated ischemic attacks and bleeding for 20 months. Analysis of 51 cases of vertebral artery aneurysm failed to disclose any comparable case. A review of the literature also revealed that clinically diagnosed dissecting aneurysms of the vertebrobasilar system have been reported rarely. Most of these cases were found at autopsy.

Bilateral Vertebral Artery Dissection - Multiple Artery Affection or Early Recurrence?

In a certain percentage of patients with craniocervical artery dissection, dissections affect multiple arteries. Some investigators consider that the dissections diagnosed as multiple might have occurred sequentially within a short time frame. We describe an oligosymptomatic patient with bilateral progressive vertebral artery dissection. Careful history taking added new data on transient left arm weakens two months earlier, as a possibility of the earlier disease onset.

Does Variation of the Vertebral Artery (VA) Origin Affect Vertebral Artery Dominance? A Retrospective, Single Centre Cohort Study.

INTRODUCTION: The vertebral arteries (VA), usually arising from the subclavian arteries, supply the posterior circulation of the brain. Anatomical variations in VA origin occur in up to 5.8% of cases, most commonly with direct aortic origin of the left VA. As paired vessels, the sizes of VAs are often mismatched, a phenomenon known as VA dominance. This results in unequal haemodynamic stress, which has been associated with increased risks of atherosclerosis, posterior circulation strokes and vertebrobasilar dissection. Despite their recognised clinical significance, limited studies have examin

An association between vertebral artery dominance and steno-occlusion of the vertebral artery segments: A retrospective angiographic study.

OBJECTIVES: This study investigated whether there was a relationship between steno-occlusion of the vertebral artery (VA) segments and the dominant VA side. MATERIALS AND METHODS: Angiography results of 215 patients (146 men; 69 women) were retrospectively analyzed in this study. The patients were divided into three groups: dominant, non-dominant and co-dominant. These groups were compared according to the presence of steno-occlusion in the vertebral artery segments. The results were evaluatedusing X2, Mann-Whitney U and Kruskal-Wallis tests. For correlation analysis, Spearman's Rho test was

Percutaneous transluminal angioplasty of the vertebral artery. A therapeutic alternative to operative reconstruction of proximal vertebral artery stenoses.

Percutaneous transluminal angioplasty (PTA) of the proximal vertebral artery was performed in 13 patients with stenosis of the proximal vertebral arteries. All of these patients had symptoms indicating vertebrobasilar insufficiency. PTA was performed only if an extreme reduction of the total diameter of both vertebral arteries was present. Only 13 patients have fulfilled the strict selection criteria in the last 3 years. All patients were monitored during the procedure by means of continuous-wave Doppler ultrasound and electrophysiological techniques. After PTA their neurological and vascular

Vertebral artery-posterior inferior cerebellar artery bypass using a radial artery graft for hemorrhagic dissecting vertebral artery aneurysms: surgical technique and report of 2 cases.

Endovascular occlusion of hemorrhagic dissecting aneurysms of the vertebral artery (VA) is not possible when the posterior inferior cerebellar artery (PICA) originates from the dissecting aneurysm or when the contralateral VA provides inadequate collateral blood flow to the distal basilar circulation. The authors introduce a VA-PICA bypass with radial artery interposition graft and aneurysm trapping as an alternative approach and describe 2 cases in which this bypass was used to treat hemorrhagic dissecting VA aneurysms. The VA-PICA bypass is performed via a standard far lateral approach. An e

Vertebral Artery Stenting for Acute Multiple Cerebral Infarctions Caused by Vertebral Artery Dissection After Massage: A Case Report.

Vertebral artery dissection is a rare pathology that can cause ischemic stroke in young people. Cervical massage, especially improper pulling manipulation, is a cause of vertebral artery dissection. We present a case of 32-year-old woman who developed acute multiple posterior circulation ischemic cerebral infarctions as a result of left vertebral artery V4 segment dissection after receiving neck massage. She underwent emergency vertebral artery stent implantation at the site of the dissection. Symptoms were relieved the day after treatment. The patient recovered without adverse complications o

Iatrogenic vertebral artery injury with concomitant contralateral idiopathic vertebral artery dissection managed with intravascular intervention: a case report.

BackgroundVertebral artery injury (VAI) is a known potential complication of posterior cervical fusion surgery. Pre-operative imaging is used to determine the patency of bilateral vertebral arteries during the planning and execution of surgery. This case illustrates an example of a staged anterior/posterior cervical reconstruction in which an iatrogenic VAI combined with a contralateral idiopathic vertebral artery dissection not identified on pre-operative imaging resulted in absent basilar artery anterograde flow.Case descriptionA 61-year-old female underwent planned staged anterior cervical
