Hip pain is a commonly reported primary symptom with many potential causes. The causal entity can remain elusive, even after clinical history review, physical examination, and diagnostic imaging. Although there are many options for definitive treatment, many of these procedures are invasive, are associated with risk of morbidity, and can be unsuccessful, with lengthy revision surgery required. Percutaneous musculoskeletal intervention is an attractive alternative to more invasive procedures and an indispensable tool for evaluating and managing hip pain. US is an ideal modality for imaging guid

Background/Objectives: There is a gap between the very positive opinion of patients and doctors regarding knee viscosupplementation (VS) and the contrasting results of controlled studies. The objective of this study was to evaluate the overall satisfaction and predictors of satisfaction with VS in patients with knee osteoarthritis treated with VS. Methods: Post-hoc analysis of a cross-sectional study in patients with knee OA treated with one injection of a mannitol-modified cross-linked HA (HANOX-M-XL). The primary outcome was satisfaction, self-assessed semi-quantitatively by the patients. De

Hip arthropathy in haemophilic patients is disabling for hip and other common target joints. Even if bleedings in the hip are not frequent, femoroacetabular alterations may affect the functional ability of patients at a very young age. A haematologic prophylaxis combined with an adequate lifestyle and regular and low-traumatic physical activity are the keys to preventing such arthropathy. In the early stages of arthropathy, anti-inflammatory drugs and physical therapy may be sufficient to limit its progression. In cases of recurrent symptoms, viscosupplementation with hyaluronic acid, and chem

Hylan G-F 20 viscosupplementation can be used to treat knee osteoarthritis pain. This study evaluated time to knee arthroplasty (KA), KA risk factors, and health care resource utilization in patients aged ≥18 years with claims in the Optum Clinformatics Data Mart database (2006-2016) for knee osteoarthritis treated with at least one course of hylan G-F 20. Kaplan-Meier analysis estimated KA risk from osteoarthritis diagnosis and first hylan G-F 20 treatment. KA risk factors were determined using multivariate Cox regression. Among 62,033 patients treated with hylan G-F 20 and/or hylan G-F 20 si

Mechanical overload and oxidative inflammation jointly drive the progression of friction-induced, age-related joint degeneration. Current clinically employed viscosupplementation strategies, such as hyaluronic acid, only offer transient lubrication, and under high-shear conditions, it undergoes rapid viscoelasticity deterioration, greatly limiting lubrication property. Here, we reported a multifunctional nanoplatform system (6S-NP), which integrated stimuli-responsive, self-renewable, supramolecular-synergistic, super-lubricating, sustained-retaining, and senescence-reversible features for ach

Hip osteoarthritis (OA) is usually managed with systemic treatments such as nonsteroidal anti-inflammatory drugs (NSAIDs) and/or symptomatic slow-acting drugs. Unfortunately, many patients either cannot tolerate NSAIDs or suffer serious, even fatal, NSAID-induced side effects, predominantly gastrointestinal ulceration and bleeding. Viscosupplementation, which aims to restore physiological and rheological features of the synovial fluid, is a well-accepted therapeutic option in knee OA patients, but limited data exist in the literature about its potential benefit for the treatment of hip OA. Intra-articular platelet-rich plasma (PRP) is among the most frequently administered orthobiologic treatments for symptomatic knee osteoarthritis. PRP is not a single intervention. Preparations differ in platelet concentration, total platelet dose, leukocyte and erythrocyte content, injection volume, activation method, and injection interval, such that 2 products sharing the acronym may constitute fundamentally different biologic treatments. The randomized literature supports a small symptomatic advantage of PRP over hyaluronic acid in mild-to-moderate disease. That advantage appears to emerge

BACKGROUND: Tendinopathies are overuse tendon injuries showing load-dependant pain, stiffness, weakness of movement in the affected area, and impairment in the movements. The scientific interest on the role of Hyaluronic Acid (HA) for the management of tendinopathies has been increased due to its anti-inflammatory and lubricative properties.
OBJECTIVE: To collect evidence regarding the effectiveness and safety of HA injections in reducing pain in patients affected by tendinopathies.
METHODS: A scientific literature search was conducted using the PubMed, Medline and PEDro electronic databases

The development of non-cellularized injectable suspensions of viscous chitosan (CHI) solutions (1.7⁻3.3% (w/w)), filled with cellulose nanofibers (CNF) (0.02⁻0.6% (w/w)) of the type nanofibrillated cellulose, was proposed for viscosupplementation of the intervertebral disc nucleus pulposus tissue. The achievement of CNF/CHI formulations which can gel in situ at the disc injection site constitutes a minimally-invasive approach to restore damaged/degenerated discs. We studied physico-chemical aspects of the sol and gel states of the CNF/CHI formulations, including the rheological behavior in rel

There are multiple reports in the literature of chondrolysis following arthroscopic shoulder surgery. Although the etiology of these cases is not known for certain, there has been speculation that radiofrequency devices, young patient age, instability surgery, intra-articular pain pumps, and type of anesthetic may be precipitating factors. This article describes a case of a 37-year-old law enforcement officer who injured both shoulders and ultimately underwent nearly identical bilateral procedures: arthroscopic superior labrum anteroposterior (SLAP) repair, Bankart repair, capsulorrhaphy, acro

Based on the published literature available so far, it appears that naturally derived hyaluronic acid (HA) and newer formulations available on the market belong to the pharmacologic class of slow-acting drugs for the treatment of osteoarthritis. These compounds seem to have the potential to modulate the painful symptoms of osteoarthritis as well as to improve the function of the osteoarthritis joint. Positive clinical consequences are based on direct and indirect effects of viscosupplementation associated with a normalization of the rheologic properties of the osteoarthritic synovial fluid, de

Post-traumatic osteoarthritis (PTOA) is a common cause of lameness in the horse. There is no cure, therefore treatments are aimed at reducing pain and improving the joint environment by modifying inflammatory pathways or by viscosupplementation. Here, we report the safety and efficacy of the biolubricant (poly(2-methacryloyloxyethyl phosphorylcholine; pMPC) to mitigate the physical, gross, histological, and biochemical effects of arthritis. We created an osteochondral fragment in the middle carpal joint of one limb in 16 horses to induce PTOA; the contralateral limb served as a sham-operated j

Osteoarthritis (OA) is a multifactorial joint disorder characterized by inflammation, cartilage homeostasis disruption, and extracellular matrix degradation. Although hyaluronic acid (HA) viscosupplementation is widely used, available formulations mainly provide symptomatic relief without targeting underlying inflammatory and catabolic processes. Here, two innovative HA derivatives covalently functionalized with lipoic acid and formate (SHLF0.5% and SHLF1%) were developed to enhance viscoelastic properties and modulate the oxidative joint microenvironment. Biological performance was evaluated

Introduction/Objectives: Temporomandibular joint osteoarthritis (TMJOA) causes pain, mandibular dysfunction, fibrocartilage degradation, and synovial inflammation. Current therapies are mainly palliative and limited by rapid intra-articular clearance and insufficient disease-modifying effects. This review summarizes injectable hydrogels for TMJOA as regenerative scaffolds, drug delivery systems, joint lubricants, and mechanical buffers. Methods: Relevant studies were identified from PubMed/MEDLINE, Web of Science and Google Scholar using terms related to TMJOA, injectable hydrogels, intra-arti

OBJECTIVE: Transcutaneous vocal cord injection is a popular treatment choice for patients suffering from unilateral vocal cord paralysis. Transcutaneous transcartilaginous (through the thyroid cartilage) approach abolishes any anatomical constraint to reach the vocal cord. However it is a largely blind procedure as the needle should stay submucosal. Herein we report our experience in employing ultrasound guidance to circumvent this problem.
METHODS: Retrospective review of a series of 8 patients who underwent combined ultrasound/endoscopy-assisted vocal cord injection for unilateral vocal cor
