Nasopharyngeal craniopharyngioma. Case report and literature review.

Craniopharyngioma is an intracranial tumor that occurs rarely in the infrasellar region. Eight patients with craniopharyngioma located within the nasopharynx have been reported previously. These cases originated in the nasopharynx and involved the sella turcica (6), the sphenoid sinus (1), and the vomer (1). Craniopharyngioma usually originates intracranially. When there is no evidence of sellar involvement, the tumor most likely arises along the path of the craniopharyngeal duct. According to the neurosurgical literature, the optimal treatment consists of total surgical excision. Incomplete t

Neonatal septal deviations.

In a review of 1000 consecutive neonates of all races, nasal septal deformity was identified in 29, an incident of 2.9%; significantly fewer cases were found in negroid babies (0.1 greater than P greater than 0.05). Ten cases (35%) underwent manipulation of the deviated septum. Six-monthly review revealed that symptoms associated with the septal deformity are rare. In 7 (44%) of the 16 cases not manipulated, the septum straightened spontaneously during the first few months of life. The appearance of the deviation is not of a dislocation of the caudal edge of the cartilage but a smooth concavit

A salvage procedure in cleft palate repair: suturing nasal mucosa directly to palatine process.

OBJECTIVE: Palatal fistulas are among the complications of cleft palate repair requiring additional surgery. Suturing the nasal mucosa and mucoperiosteal flaps together in a tension-free manner to create a double-layered closure in the hard palate is one of the most important points in prevention of dehiscence and fistula formation. In this report, we describe a salvage procedure to repair nasal mucosa that might be lacerated while being freed from the upper surface of the palatal process. METHOD: To restore the nasal lining, an ipsilateral vomer mucoperiosteal flap or the opposite nasal muco

Influences of different palatoplasties on palatal growth and speech development: comparison between Osada's two-stage palatoplasty and one-stage mucosal flap procedure.

Palatal growth and articulation in two different palatal closure techniques, Osada's two-stage procedure and the conventional one-stage procedure, were evaluated in 12 patients. Osada's procedure (hard palate closure using a vomer flap with complete closure of the raw surface by a full thickness skin graft) is a two-stage operation that consists of early soft palate closure at the time of lip closure (3 to 6 months of age). The palatal sizes and the depth of the palatal arch were significantly greater in the two-stage group than in the one-stage group at 1 year of age and, 3 to 5 years of age.

A Case of Traumatic Nasal Valve Stenosis Successfully Treated with Open Rhinoplasty and Z-Plasty.

External nasal deformity is common and can be treated surgically. It often occurs in conjunction with the development of the nasal septal cartilage, vomer, and vertical plate of the ethmoid bone but may be caused by trauma. Here, we present a case of external nasal deformity caused by trauma. A woman presented with nasal obstruction due to dysfunction of the nasal valve area and was referred to our department for treatment. No chronic rhinosinusitis or nasal septal deviation that causes nasal obstruction was noted at the initial examination. However, trauma-related scarring was observed in the

Premaxillary osteotomy fixation in bilateral cleft lip/palate: Introducing a new technique.

OBJECTIVES: In bilateral cleft lip/palate patients, sometimes the premaxilla is severely protruded and twisted. In such situations premaxillary osteotomy is beneficial. In this article a new technique for fixation of premaxilla after osteotomy is presented. METHODS: The lip-split approach and premaxillary osteotomy and ostectomy from bony nasal septum were carried out. Fixation of osteotomized premaxilla was achieved with low profile miniplate or microplate. Bone grafting of that side was performed during the same session. RESULTS: This procedure was performed in three patients, with good la

Drainage or packing of the sella? Transsphenoidal surgery for primary pituitary abscess: report of two cases.

The detailed surgical procedure of the transsphenoidal surgery for pituitary abscess has scarcely been described previously because it is a very rare clinical entity. The authors reported two cases of primary pituitary abscess. In case 1, the anterior wall of the sella turcica was reconstructed with the vomer bone after irrigating the abscess cavity, but the sella was not packed by fat for fear of the persistent infection by devascularized tissues. This led to the postoperative meningocele, the cerebrospinal fluid leak, and bacterial meningitis despite the successful abscess drainage. In case

[Surgical treatment of space-occupying lesions in the nasal cavity and paranasal sinus involving the skull base].

This paper reports 29 cases of space-occupying lesions in the nasal cavities and the paranasal sinuses. Of them, 12 were benign and 17 malignant. They were all surgically treated, 17 by lateral rhinotomy, 6 by combined cranio-nasal approaches, 3 by maxillectomy and evisceration of the orbit, 1 by transfrontal and 1 by midfacial degloving approaches, respectively. The regional chemotherapy was administered to a case via external carotid artery after neck dissection. Among these cases, 18 of the skull defects were rehabilitated. The repairing materials included the autologous perpendicular plate

Comparison of a third surgical protocol for the treatment of unilateral cleft lip and palate: a multidisciplinary systematic review and meta-analysis.

This systematic review and meta-analysis builds upon our previous publication on the outcomes of patients with unilateral cleft lip and palate (UCLP) treated with Oslo protocols (OP; vomerplasty during lip closure vs delayed hard palate closure protocols (DHPCP), comparing the outcomes of these two protocols with those of one-stage palatoplasty protocols (OSPP). A systematic search of the Embase, MEDLINE/PubMed, Web of Science, Cochrane, and Google Scholar databases was conducted until August 2024. In total, 162 articles (156 study groups) were reviewed, including 4040 UCLP patients following

Endoscopic transeptal surgery for choanal atresia with a stentless folded-over-flap technique.

OBJECTIVE: To analyze long-term results of a new stentless surgical technique for choanal atresia that also used no packing. STUDY DESIGN: Prospective study. METHODS: Ten patients (3 bilateral and 7 unilateral), with ages ranging from 4 days to 30 years, were operated on. The follow-up was 6 months to 36 months. This approach allowed resection of the posterior portion of the vomer, atretic plate, and part of the medial pterygoid process. Flaps were fashioned from septal, nasal, and pharyngeal mucosa from the atretic plates and fixed with fibrin glue. Postoperative control included nasal endo

Surgical Premaxillary Setback with Vomerine Ostectomy for Complicated Bilateral Cleft Lip.

Important factors that affect the initial repair of complete bilateral cleft lip (CBCL) include not only the width of the cleft but also the distance between the premaxilla and alveolar bone. We report a case of a patient with CBCL in whom favorable morphological and aesthetical results were achieved by surgical premaxillary setback. The patient was a 5-month-old boy with CBCL, who had a severely protruding premaxilla. Despite repeated lip adhesion before lip closure surgery, wound dehiscence occurred, and therefore, we performed premaxillary setback with vomerine ostectomy. The bone was remov

Skull anatomy of the endangered Patagonian huemul deer(Hippocamelus bisulcus).

Studies of the normal skull anatomy of the Patagonian Huemul deer are scarce. Currently, the findings of bone lesions in the skull associated with metabolic imbalances are frequent in the literature. The objective of this study was to provide anatomical and morphometric data of the cranium and facies including a morphofunctional interpretation as a reference for clinical, ecomorphological and educational purposes. Five skulls were described, measured, scanned and digitally reconstructed. The presence of a caudal projection of the vomer bone, the absence of the facial tubercle and thin bones fo

The transmission of masticatory forces and nasal septum: structural comparison of the human skull and Gothic cathedral.

This study extrapolates the transmission of masticatory forces to the cranium based on the architectural principles of Gothic cathedrals. The most significant finding of the study, obtained by analysis of coronal CT scans, is the role of the hard palate, and especially the vomer and the perpendicular plate of the ethmoid in masticatory force transmission. The study also confirms, experimentally, the paths of masticatory forces, cited in literature but based purely on morphological observations. Human skulls and Gothic cathedrals have similar morphological and functional characteristics. The lo

Two different hard palate closure techniques in two-stage palatoplasty: effects on velopharyngeal closure and articulation.

Two different hard palate closure techniques, in two-stage palatoplasty, was evaluated in 12 patients. The lip and soft palate were closed at 3 to 7 months of age. The patients were then divided into two groups of six and the hard palate closed at 17 to 23 months of age, either by a vomer flap (VF) with a skin graft (Osada's two-stage palatoplasty) in the VF group, or by the push-back (PB) procedure of the mucoperiosteal flap in the PB group. Velopharyngeal closure (VPC) during a gag reflex before and after hard palate closure, and the articulation of these patients after closure, were evaluat

The embryonic development of sensory organs and the skull in the trisomy 16 mouse, an animal model for Down's syndrome.

The trisomy 16 mouse is a widely accepted animal model for the study of the embryonic development of human trisomy 21. While the development of the brain and heart has been thoroughly studied, there are hardly any data on the development of sensory organs like the eye, nose and ear. By studying scanning electron microscopic pictures and semithin sections from the tenth to the 15th day of development, we found delayed development of the nose, and, in particular, of the vomer. Sensory structures of the otic vesicle also showed a marked developmental delay. Pigmentation of the outer layer of the

[Horseshoe Le Fort I osteotomy].

INTRODUCTION: Performing Le Fort I impaction osteotomy can be compromised or impossible. The intranasal volume of the lower turbinates prevents an important maxillary impaction. In this case, horseshoe osteotomy is an alternative. SURGICAL TECHNIQUE: The first step is to lower the inferior edge of the pyriform aperture and the anterior floor of the nasal fossae with a bur. The palatine alveolar wall is cut through resection of the maxillary sinus anterior and lateral walls. The maxillary dental arch may be impacted without freeing the vomer or cutting through lateral walls of the nasal fossa,

Tongue-bite apparatus highlights functional innovation in a 310-million-year-old ray-finned fish.

Gill-skeleton modifications for processing prey represent a major source of functional innovation in living ray-finned fishes. Here we present the oldest actinopterygian tongue bite, derived from the gill skeleton, in the Middle Pennsylvanian (approx. 310 Ma) †Platysomus parvulus. Unrelated to extant tongue biters, this deep-bodied taxon possesses a large, multipartite basibranchial tooth plate opposing an upper tooth field centred on the vomer. This branchial structure occurs in conjunction with toothed jaws, indicating a role for both the basibranchial plate and jaws in feeding. †P. parvulus
