Emergency Medical Responders and Adolescents With Autism Spectrum Disorder.

OBJECTIVES: Because of the high prevalence of Autism Spectrum Disorder (ASD) and wandering behavior, emergency medical responders (EMRs) will likely encounter children and adolescents with ASD. The objectives were to describe interactions between EMRs and children and adolescents with ASD, to evaluate EMRs' ability to recognize ASD in a simulated trauma setting, and to determine if EMRs' demographic characteristics affected their interactions with ASD youth. METHODS: A study of 75 videos of a simulated school bus crash was performed. The simulation included an adolescent with ASD portrayed by

[Clinical study of chronic subdural hematoma and hygroma in two hospitals for elderly patients with dementia].

The authors have investigated the incidence of chronic subdural hematoma and hygroma (CSH) in two hospitals for elderly patients with dementia syndrome over a period of three years and nine months. All observed patients had been in the hospital for 2 weeks or more and had had brain computed tomography (CT) on admission. The total of 613 patients included 212 males and 401 females. Based on the CT findings observed in patients on admission, the authors found 47 patients with CSH. This was equivalent to 7.7% of all patients observed. After admission, almost all patients had brain CT twice per ye

Case Report: Ischemic brain infarction and cognitive dysfunction syndrome in an aged dog.

This case report describes a rare occurrence of canine cognitive dysfunction syndrome (CDS) accompanied by ischemic brain infarction, providing insights into the relationship between neurodegeneration and cerebrovascular pathology in aged dogs. A 19-year-old neutered male miniature poodle exhibited progressive behavioral changes over three years, including nocturnal restlessness, inappropriate urination, and aimless wandering. Neurological examination revealed mild disorientation, decreased proprioception, and weakened postural reactions in the hind limbs, with a cognitive dysfunction rating (

Mediational Patterns of Parenting Styles Between Cognitive Disengagement Syndrome Difficulties and Youth Psychopathology.

BACKGROUND/OBJECTIVES: Cognitive Disengagement Syndrome (CDS) is a clinical condition primarily characterized by inattention, hypoactivity, and mind-wandering, which has not yet been recognized as an official diagnostic category. Although there are overlaps between CDS and ADHD, evidence supports the semi-independence of CDS from the ADHD-Inattentive subtype. Importantly, while the impact of ADHD on parenting styles has been studied, no previous research has investigated the potential influence of CDS difficulties on parenting behaviors. Both CDS and ADHD are associated with internalizing and

Early diagnosis of anti-N-methyl-D-aspartate receptor encephalitis in a young woman with psychiatric symptoms.

A previously healthy 21-year-old woman, transported to our medical emergency center for excluding organic brain disease, had undergone medical examination 9 days before for trembling in her left hand, which was caused by stress. The patient exhibited fever and strange behaviors, e.g., wandering around, babbling, and making smoking gestures; hence, psychiatric examination was performed. The patient's Glasgow Coma Scale score was 4-3-5, and involuntary movement was observed. Cerebrospinal fluid examination revealed increased cell count; hence, we suspected anti-N-methyl-d-aspartate (NMDA) recept

Chaos in nonequilibrium two-temperature (Tx, Ty) Nosé-Hoover cell models.

We revisit a two-temperature Nosé-Hoover wanderer particle embedded in a two-dimensional periodic 2 × 2 cell with four smooth repulsive corners at (x, y) = (±1, ±1) to explore chaos with anisotropic thermostatting. The model employs separate thermostats in the x and y directions, enabling controlled deviations from equilibrium. By integrating the full six-dimensional equations of motion and computing the complete Lyapunov spectrum, we confirm chaos and quantify phase-space contraction from the fully resolved six-dimensional Lyapunov spectrum. The total contraction rate, interpreted as entropy

Albatross movement suggests sensitivity to infrasound cues at sea.

The ways in which seabirds navigate over very large spatial scales remain poorly understood. While olfactory and visual information can provide guidance over short distances, their range is often limited to 100s km, far below the navigational capacity of wide-ranging animals such as albatrosses. Infrasound is a form of low-frequency sound that propagates for 1,000s km in the atmosphere. In marine habitats, its association with storms and ocean surface waves could in effect make it a useful cue for anticipating environmental conditions that favor or hinder flight or be associated with profitabl

Characterizing lift, drag, and pressure differences across wandering salamanders (Aneides vagrans) with computational fluid dynamics to investigate aerodynamics.

Wandering salamanders (Aneides vagrans), known to occupy the crowns of old growth coast redwood trees, have recently been found to decelerate and engage in controlled, nonvertical descent while falling. Closely related, nonarboreal species with seemingly minor morphological differences exhibit far less behavioral control while falling; however, the influence of salamander morphology on aerodynamics remains to be tested. Here, we examine differences in morphology and aerodynamics of two salamander species, A. vagrans and the nonarboreal ensatina salamander (Ensatina eschscholtzii), using a comb

Modern-Day Relics of Psychiatry.

Constantly shifting cultural views influence public perceptions of psychiatric diagnoses, sometimes accommodated by changes in diagnostic terminology. Evolving scientific knowledge of the era is at times used to justify and support mental illnesses. Too often, however, remasked nomenclatures fail to alter social stigma, in part because political arguments are used. Scientific validations of variant behaviors as symptoms with a pathologic status are unfortunately overshadowed. Examples of cultural bias effects on recurring diagnostic challenges illustrate a need for scientific validation. Renam

Service inputs and costs of care related to outcomes among cognitively impaired nursing home residents.

BACKGROUND: There are over 17 000 nursing homes in the United States. Within these, special care units (SCUs) provide a separate residential and/or activity locus for residents, and are expected to provide more staff time and more specialized staff assignments. This paper addresses a fundamental issue relating to the nature, quality and quantity of resident care inputs: what impacts of SCUs are associated with added service inputs, and thus with personnel costs, recognizing that personnel account for the majority of costs associated with nursing home care? AIMS OF THE STUDY: The aim of this as

Patient safety considerations in the rehabilitation of the individual with cognitive impairment.

Deficits in cognitive functioning are associated with many safety concerns, including difficulties performing activities of daily living, medication errors, motor vehicle accidents, impaired awareness of deficits, decision-making capacity, falls, and travel away from home. Preventing adverse safety outcomes is particularly relevant in rehabilitation patients. Integration of information and recommendations stemming from allied disciplines, such as rehabilitation medicine, physical therapy, occupational therapy, speech therapy, and neuropsychology, is the most effective way to limit poor outcome

Development of a Thai tool for assessing behavioral and psychological symptoms of dementia: A confirmatory factor analysis.

INTRODUCTION: The early recognition and management of the behavioral and psychological symptoms of dementia (BPSD) are important to inform treatment decisions. Current BPSD screening tools are time-consuming and require advanced skills, limiting their application in routine clinical practice. An easier and quicker tool for use by nonphysician healthcare personnel is needed. METHODS: A 14-item, Thai-language, BPSD scoring system for dementia (BPSD-T) was developed, based on clinical surveys and modifications after a pilot study. The Neuropsychiatric Inventory (NPI), BPSD-T, Thai Mental State E

Nodding syndrome: recent insights into etiology, pathophysiology, and treatment.

Nodding syndrome is an enigmatic neuropsychiatric and epileptiform disorder associated with psychomotor, mental, and physical growth retardation. The disorder affects otherwise previously normal children aged 3-18 years, with a slight preponderance for the male child. Nodding syndrome has been described in rural regions of some low-income countries in sub-Saharan Africa including northern Uganda, South Sudan, and a mountainous region of southern Tanzania. The cause of the disorder has hitherto eluded scientists. Neuroimaging studies show involvement of the nervous system with associated severe

Disrupted functional connectivity of cerebellar default network areas in attention-deficit/hyperactivity disorder.

Attention-deficit/hyperactivity disorder (ADHD) is increasingly understood as a disorder of spontaneous brain-network interactions. The default mode network (DMN), implicated in ADHD-linked behaviors including mind-wandering and attentional fluctuations, has been shown to exhibit abnormal spontaneous functional connectivity (FC) within-network and with other networks (salience, dorsal attention and frontoparietal) in ADHD. Although the cerebellum has been implicated in the pathophysiology of ADHD, it remains unknown whether cerebellar areas of the DMN (CerDMN) exhibit altered FC with cortical

Internally directed cognition and mindfulness: an integrative perspective derived from predictive and reactive control systems theory.

In the present paper, we will apply the predictive and reactive control systems (PARCS) theory as a framework that integrates competing theories of neural substrates of awareness by describing the "default mode network" (DMN) and anterior insula (AI) as parts of two different behavioral and homeostatic control systems. The DMN, a network that becomes active at rest when there is no external stimulation or task to perform, has been implicated in self-reflective awareness and prospection. By contrast, the AI is associated with awareness and task-related attention. This has led to competing theor

Mucopolysaccharidosis Type IIIA presents as a variant of Klüver-Bucy syndrome.

Mucopolysaccharidosis Type IIIA (MPS IIIA) is a neurodegenerative disease with behavioral symptoms unique among the mucopolysaccharidoses. Children with MPS IIIA reportedly mouth things, explore novel environments almost continuously, disregard danger, and empathize/socialize and comply less with parents. These characteristics resemble Klüver-Bucy syndrome (K-Bs). To test the K-Bs hypothesis, 30 children with MPS IIIA were compared to 8 "posttransplant" mucopolysaccharidosis Type IH patients in an experimental "risk room." The room contained attractive and mildly frightening objects, exposure

Caregiver-reported use of physical restraints among community-dwelling older adults with severe dementia in Singapore.

BACKGROUND: Older adults with severe dementia are at increased risk of being physically restrained in nursing homes and acute care settings, but little is known about restraint use among those cared for at home. This study explores caregiver-reported use of restraints among community-dwelling older adults with severe dementia. METHODS: Using cross-sectional data from 215 family caregivers, we describe restraint use among older adults with severe dementia living at home. We then use multivariable logistic regression to identify factors associated with restraint use. RESULTS: Nearly half (47%)
