Otorhinolaryngological problems in medical support of space flights.

At present the main trends among the most important problems of otorhinolaryngology in space medicine have become defined as vestibulology, audiology and clinical aspects (prophylaxis, diagnosis and treatment of ENT diseases in flight). The principal result of recent vestibular studies has probably been the establishment of an apparent relationship between the resistance of the vestibular system to adequate ground-based stimulation and tolerance to space flight. The findings of the studies formed the basis for the development of a new system of vestibular selection, as well as demonstrated the

Centrifugation as a countermeasure during actual and simulated microgravity: a review.

This paper summarizes what has been learned from studies of the effects of artificial gravity generated by centrifugation in actual and simulated weightless conditions. The experience of artificial gravity during actual space flight in animals and humans are discussed. Studies using intermittent centrifugation during bed rest and water immersion, as a way to maintain orthostatic tolerance and exercise capacity, are reviewed; their results indicate that intermittent centrifugation is a potential countermeasure for maintaining the integrity of these physiological functions in extended space miss

Neck-vein thrombosis during spaceflight.

Spaceflight imposes unique environmental challenges, including weightlessness, increased radiation exposure and confinement, which can lead to unexpected health effects. One such example is neck-vein thrombosis, a condition rarely seen on Earth without predisposing factors such as venous catheters or infections. Although spaceflight has not previously been associated with thromboembolic events, recent cases of neck-vein thrombosis in astronauts aboard the International Space Station have raised concern. These events were detected incidentally and showed no obvious symptoms, complicating diagno

Remains Containment Considerations for Death in Low-Earth Orbit.

BACKGROUND: Maintenance and disposition of decedent remains during spaceflight require the isolation of biohazardous products of decomposition in microgravity and in the absence of refrigeration. Containment and isolation options would preferably offer sufficient time to enable crew and ground support teams to determine appropriate disposition of remains and even potentially return remains to the Earth. The pilot study described herein undertook an effort to develop a postmortem containment unit for the isolation and maintenance of decedent remains in a microgravity environment.METHODS: Commer

The search for the ultimate exercise countermeasure to preserve crew health and ensure mission success in long-duration spaceflight.

The current understanding of crew health maintenance is founded upon decades of physiological research conducted in terrestrial spaceflight analogues and in low Earth orbit, particularly on the International Space Station. However, as we progress towards the Lunar Gateway and interplanetary missions, it is imperative that the tools employed to maintain crew health are redefined, including the utilisation of exercise countermeasures. The successful implementation of exercise countermeasures for deep space missions must address a number of challenges, including those posed by new environments wi

Bone mineral measurement: Skylab experiment M-078.

The observation that bone mineral is lost in patients who are either immobilized or remain in bed for extended periods of time formed the basis for the concern that large amounts of bone mineral may be lost during long periods of weightlessness. This concern was magnified when early X-ray densitometry studies suggested that rather large amounts of mineral could be lost during rather short periods of weightlessness (4-14 days). Even though these Gemini results have recently been modified, they still reflect substantial losses in the upper extremity. This led to a series of prolonged bed-rest st

Neurobiology of fish under altered gravity conditions.

In vertebrates (including man), an altered gravitational environment such as weightlessness can induce malfunction of the inner ear, based on an irregular dislocation of the otoliths from the corresponding sensory epithelia. This dislocation leads to an illusionary tilt, since the otolithic inputs are not in register with other sensory organs. This results in an intersensory conflict. Vertebrates in orbit therefore face severe orientation problems. In humans, the intersensory conflict may additionally lead to a malaise, commonly referred to as space motion sickness (SMS). During the first days

Humans in space.

Many successful space missions over the past 40 years have highlighted the advantages and necessity of humans in the exploration of space. But as space travel becomes ever more feasible in the twenty-first century, the health and safety of future space explorers will be paramount. In particular, understanding the risks posed by exposure to radiation and extended weightlessness will be crucial if humans are to travel far from Earth.

Establishment and Validation of a Novel Microgravity Simulation Platform for Ground-Based Animal Experiments.

Ground-based analog models, such as the hindlimb unloading (HU) technique, have been used for several decades in simulating microgravity effects on musculoskeletal studies. However, this model has seen limited technological advancement in several aspects. Here, the design and validation of the Microgravity Simulated Platform (MSP), a refined evolution of the classic HU model, is presented. The multipurpose engineered MSP apparatus is designed to overcome key limitations of existing models by enabling simultaneous study of a larger number of mice in compact space, with fewer lesions, less infla

Gravitational effects on intraocular pressure and ocular perfusion pressure.

Changes in the gravitational vector by postural changes or weightlessness induce fluid shifts, impacting ocular hemodynamics and regional pressures. This investigation explores the impact of changes in the direction of the gravitational vector on intraocular pressure (IOP), mean arterial pressure at eye level (MAPeye), and ocular perfusion pressure (OPP), which is critical for ocular health. Thirteen subjects underwent 360° of tilt (including both prone and supine positions) at 15° increments. At each angle, steady-state IOP and MAPeye were measured, and OPP calculated as MAPeye - IOP. How spaceflight challenges human cardiovascular health.

The harsh environmental conditions in space, particularly weightlessness and radiation exposure, can negatively affect cardiovascular function and structure. In the future, preventive cardiology will be crucial in enabling safe space travel. Indeed, future space missions destined to the Moon and from there to Mars will create new challenges to cardiovascular health while limiting medical management. Moreover, commercial spaceflight evolves rapidly such that older persons with cardiovascular risk factors will be exposed to space conditions. This review provides an overview on studies conducted

Exposure of biological preparations to radiofrequency electromagnetic fields under low gravity.

There is interest as to whether the electromagnetic fields used in mobile radiotelephony might affect biological processes. Other weak fields such as gravity intervene in a number of physical and biological processes. Under appropriate in vitro conditions, the macroscopic self-organization of microtubules, a major cellular component, is triggered by gravity. We wished to investigate whether self-organization might also be affected by radiotelephone electromagnetic fields. Detecting a possible effect requires removing the obscuring effects triggered by gravity. A simple manner of doing this is

Physiological changes during spaceflight.

Before human beings first explored space, many dire predictions from the scientific community suggested that weightlessness was not compatible with life. Although these prophesies have been proven false, many physiological alterations do occur in microgravity conditions. Other systems are altered as well, but the cardiovascular, musculoskeletal, and immune systems undergo adaptations that may pose problems for the space traveler, especially on returning to gravity.

Design and validation of an electroporation-capable random positioning machine for the study of cell membrane permeabilization in simulated microgravity.

Biomanufacturing is being explored as a potential solution for improved space logistics in austere environments. One potentially useful tool is electroporation-the delivery of exogenous DNA into cells using pulsed electric fields to increase cell membrane permeability. Electroporation has the potential to enable tunable bioreactors in a space environment through the targeted delivery of nucleic acids encoding proteins of interest and may also present a solution for disinfection, purification, and biomolecule extraction without requiring disposable reagents. Unfortunately, electroporation has n

The exercise and environmental physiology of extravehicular activity.

Extravehicular activity (EVA), i.e., exercise performed under unique environmental conditions, is indispensable for supporting daily living in weightlessness and for further space exploration. From 1965-1996 an average of 20 h x yr(-1) were spent performing EVA. International Space Station (ISS) assembly will require 135 h x yr(-1) of EVA, and 138 h x yr(-1) is planned for post-construction maintenance. The extravehicular mobility unit (EMU), used to protect astronauts during EVA, has a decreased pressure of 4.3 psi that could increase astronauts' risk of decompression sickness (DCS). Exercise

[The pharmacological regulation of physiological functions in space medicine].

There are considered the physiological systems of the body which are the most sensitive to the action of weightlessness and are subject to pharmacological correction at different stages of a space flight. Particular emphasis is given to the development and practical use of the cardiovascular drugs, the agents for prevention of movement disease and also the agents influencing the metabolic processes and mineral saturation of the bony tissue. The possibility of using the drugs intended for the application during space flights and in medical practice is emphasized.

Comprehensive assessment of physiological responses in women during the ESA dry immersion VIVALDI microgravity simulation.

Astronauts in microgravity experience multi-system deconditioning, impacting their inflight efficiency and inducing dysfunctions upon return to Earth gravity. To fill the sex gap of knowledge in the health impact of spaceflights, we simulate microgravity with a 5-day dry immersion in 18 healthy women (ClinicalTrials.gov Identifier: NCT05043974). Here we show that dry immersion rapidly induces a sedentarily-like metabolism shift mimicking the beginning of a metabolic syndrome with a drop in glucose tolerance, an increase in the atherogenic index of plasma, and an impaired lipid profile. Noninvasive indicators of intracranial pressure before, during, and after long-duration spaceflight.

Weightlessness induces a cephalad shift of blood and cerebrospinal fluid that may increase intracranial pressure (ICP) during spaceflight, whereas lower body negative pressure (LBNP) may provide an opportunity to caudally redistribute fluids and lower ICP. To investigate the effects of spaceflight and LBNP on noninvasive indicators of ICP (nICP), we studied 13 crewmembers before and after spaceflight in seated, supine, and 15° head-down tilt postures, and at ∼45 and ∼150 days of spaceflight with and without 25 mmHg LBNP. We used four techniques to quantify nICP: cerebral and cochlear fluid pre

The effect of parabolic flight on perceived physical, motivational and psychological state in men and women: correlation with neuroendocrine stress parameters and electrocortical activity.

Previous findings of decreased mental and perceptual motor performance during parabolic flights have been attributed mainly to the primary effects of weightlessness rather than the accompanying effects of stress and altered mood. Although recent studies have alluded to the possible negative effects of stress on performance, there has been no attempt to investigate this during parabolic flights. Over a period of 3 years, 27 human participants (male n = 18, mean age +/- SD 34.67 +/- 7.59 years; female n = 9, 36.22 +/- 9.92 years) were recruited with the aim to evaluate if, and to what extent, pa
