Exercise-entrained breathing and non-invasive determination of anaerobic threshold.

Incremental cycle exercise was performed by 17 subjects under two breathing conditions: spontaneous breathing (Non-ENT run) and deliberate entrainment of breathing frequency (f) to cycling rate at preferred coupling ratios up to possibly high work rates (ENT run). The purpose was to investigate the influence of entrainment on two non-invasive determinations of anaerobic threshold (AT): the break points of VE/VO2 (VAT) and VCO2 (V-slope AT) changes against VO2. Compared to the Non-ENT run, VAT in the ENT run was greater in 12 subjects entrained at higher f accompanied by relative hyperventilati

The Effect of Waistbands on Intra-Abdominal Pressure and Respiratory Mechanics During Bodyweight Exercise.

Leahy, MG, Kipp, S, and Sheel, AW. The effect of waistbands on intra-abdominal pressure and respiratory mechanics during bodyweight exercise. J Strength Cond Res 38(12): 2047-2054, 2024-Athletic apparel is often designed with high-waisted, elastic bands around the abdomen to provide comfort and support during functional exercises. The abdominal musculature aids in both active respiration and trunk stability during the exercise, but whether high-waistband athletic garments affect pulmonary function and respiratory mechanics is unknown. Ten healthy women participated in a single-visit study. Sub

Mandibular advancement devices for the control of snoring.

Patients presenting with the complaint of antisocial snoring have very few options available to them of proven efficiency. Mandibular advancement devices worn intra-orally at night, have recently been shown in controlled trials to help mild to moderate obstructive sleep apnoea. However, there are no properly controlled studies with objective measurements on the use of such appliances for the management of antisocial snoring. Fifteen patients, already established on mandibular advancement devices for the control of snoring, were asked to participate in this study. They were studied over two nig

Airway obstruction and gastroesophageal reflux: an experimental study on the pathogenesis of this association.

Gastroesophageal reflux (GER) frequently complicates the clinical course of children suffering from conditions leading to upper airway obstruction (UAO) (choanal atresia, tracheomalacia, esophageal atresia, vascular rings, etc). In an attempt to explore whether partial airway obstruction causes changes in the normal thoracoabdominal pressure gradients, we measured end-inspiratory intrathoracic and intraabdominal pressures in anesthetized rats under spontaneous breathing conditions, after tracheostomy, and under upper airway obstruction induced by tracheal intubation with three progressively na

Effect of surgical repair on respiratory mechanics in congenital diaphragmatic hernia.

To determine whether surgical repair of congenital diaphragmatic hernia (CHD) results in improvement in respiratory mechanics, we measured respiratory system compliance in nine patients (five survivors and four nonsurvivors) before and after operation. In all nine infants, CHD was diagnosed within 6 hours of life, and surgical repair was through an abdominal approach after a period of stabilization. Measurements were made noninvasively, using the passive expiratory flow-volume technique. In only one of the nine infants did compliance immediately improve after surgical repair, and in another it

Assessment of Respiratory Drive in the Mechanically Ventilated Patient.

Respiratory drive is a critical yet often underappreciated determinant of outcome during mechanical ventilation. Both insufficient and excessive drive can cause harm: Low drive promotes diaphragm atrophy, whereas high drive may contribute to lung and diaphragm injury. A key clinical challenge is the potential dissociation between drive and effort in patients with respiratory muscle weakness, where high drive may not translate to adequate muscular output. Simple bedside tools such as airway-occlusion pressure (P0.1) and whole-breath occlusion pressure (POCC) can identify patients at risk and gu

Theophylline increases oxygen consumption during inspiratory resistive loading.

The effect of theophylline on diaphragmatic blood flow (Qdi) and oxygen consumption (VO2di) was studied in eight lightly anesthetized dogs during quiet breathing and inspiratory resistive loading. Qdi was determined with the radioactive microsphere tracer technique, and VO2di was calculated as the product of Qdi and the diaphragmatic arterio-venous oxygen difference. During quiet breathing, theophylline increased minute ventilation (9.3 +/- 1.7 versus 5.1 +/- 0.4 L/min), mean inspiratory flow (547 +/- 60 versus 378 +/- 56 ml/s), and duty cycle (0.270 +/- 0.042 versus 0.192 +/- 0.024) but did n

Bi-level positive airway pressure ventilation reduces the oxygen cost of breathing in long-standing post-polio patients on invasive home mechanical ventilation.

BACKGROUND: Today, patients with chronic respiratory failure are commonly treated with non-invasive bi-level positive airway pressure ventilation, supporting spontaneous breathing. However, in conformity with previous clinical routine, many post-polio patients with chronic respiratory failure are still treated with invasive (i.e. via a tracheostomy) controlled mechanical ventilation (CMV). The aim of the study was to investigate the effect of invasive bi-level positive airway pressure ventilation on the work of breathing compared with that during the patients' ordinary CMV and spontaneous brea

Comparison of maximal oxygen consumption with oral and nasal breathing.

The major cause of exercise-induced asthma (EIA) is thought to be the drying and cooling of the airways during the 'conditioning' of the inspired air. Nasal breathing increases the respiratory system's ability to warm and humidity the inspired air compared to oral breathing and reduces the drying and cooling effects of the increased ventilation during exercise. This will reduce the severity of EIA provoked by a given intensity and duration of exercise. The purpose of the study was to determine the exercise intensity (%VO2 max) at which healthy subjects, free from respiratory disease, could per

A hyperventilation theory of job stress and musculoskeletal disorders.

BACKGROUND: There is evidence of a link between job stress and upper extremity work-related musculoskeletal disorders. However, the biobehavioral mechanisms by which psychosocial stress factors contribute to the development of musculoskeletal disorders are uncertain. METHODS: Based on established principles of breathing and job stress and the relevant empirical literatures, a hyperventilation theory of job stress and work-related musculoskeletal disorders was developed. RESULTS: Hyperventilation (overbreathing) refers to a drop in arterial CO2 caused by ventilation that exceeds metabolic dem

[Variation in inspiratory gas flow in pressure support ventilation. The effect on respiratory mechanics and respiratory work].

UNLABELLED: During pressure support ventilation (PSV), the timing of the breathing cycle is mainly controlled by the patient. Therefore, the delivered flow pattern during PSV might be better synchronised with the patient's demands than during volume-assisted ventilation. In several modern ventilators, inspiration is terminated when the inspiratory flow decreases to 25% of the initial peak value. However, this timing algorithm might cause premature inspiration termination if the initial peak flow is high. This could result not only in an increased risk of dyssynchronization between the patient

Inspiratory pressure support prevents diaphragmatic fatigue during weaning from mechanical ventilation.

Persistent inability to tolerate discontinuation from mechanical ventilation is frequently encountered in patients recovering from acute respiratory failure. We studied the ability of inspiratory pressure support, a new mode of ventilatory assistance, to promote a nonfatiguing respiratory muscle activity in eight patients unsuccessful at weaning from mechanical ventilation. During spontaneous breathing, seven of the eight patients demonstrated electromyographic signs of incipient diaphragmatic fatigue. During ventilation with pressure support at increasing levels, the work of breathing gradual

Physiologic Effects of High-Flow Nasal Cannula in Healthy Subjects.

BACKGROUND: High-flow nasal cannula (HFNC) is increasingly used in the management of acute and chronic respiratory failure. Little is known about the optimal settings for HFNC. This study was designed to assess the dose effect of HFNC on respiratory effort indexes and respiratory patterns in spontaneously breathing adults. METHODS: A randomized controlled crossover study was conducted in 10 healthy subjects. Five experimental conditions were evaluated: baseline with no therapy; 5 L/min with conventional nasal prongs; and HFNC at 20, 40, and 60 L/min. The primary outcomes were the indexes of r

Identification of Breathing Patterns through EEG Signal Analysis Using Machine Learning.

This study was to investigate the changes in brain function due to lack of oxygen (O2) caused by mouth breathing, and to suggest a method to alleviate the side effects of mouth breathing on brain function through an additional O2 supply. For this purpose, we classified the breathing patterns according to EEG signals using a machine learning technique and proposed a method to reduce the side effects of mouth breathing on brain function. Twenty subjects participated in this study, and each subject performed three different breathings: nose and mouth breathing and mouth breathing with O2 supply d

The distribution of blood flow, oxygen consumption, and work output among the respiratory muscles during unobstructed hyperventilation.

An animal model was developed to describe respiratory muscle work output, blood flow, and oxygen consumption during mechanical ventilation, resting spontaneous ventilation, and the increased unobstructed ventilatory efforts induced by CO2 rebreathing. Almost all of the work of breathing was inspiratory work at all ventilatory levels; thus, only blood flows to the diaphragm and external intercostals increased in the transition from mechanical to spontaneous ventilation, and they further increased linearly as ventilatory work was incrementally augmented ninefold by CO2 rebreathing. No other musc

[Pathophysiology of dyspnea].

The sensation of dyspnea is related to pathologically increased breathing work as a result of hyperventilation and/or increased airway resistance. Low cardiac output resulting in tissue hypoxia provokes hyperventilation. In diffuse interstitial lung diseases, hyperventilation is combined with severely reduced lung compliance. In severe airway obstruction, hypoventilation is the result of excessively enhanced breathing work. Habituation and acute changes of situation have an important bearing on the subjective sensation of dyspnea.
