Civilian gunshot wounds to the spinal cord and cauda equina.

We evaluated 42 patients with neurological deficits after civilian gunshot wounds to the spine. Thirty-five of these patients (the study population presented here) received their initial and follow-up care at Louisiana State University Medical Center in Shreveport over a 4-year period. Each patient had incurred a single gunshot wound to the spinal cord or cauda equina with an accompanying neurological deficit. The patient population was divided into three groups. Group 1 patients had incurred a complete motor and sensory loss below the injury (20 patients (57%]. Group 2 patients had incurred i

Evaluation of Prognostic Factors in Patients with Cranial Gunshot Wounds.

BACKGROUND: In recent years, cranial gunshot wounds become a growing concern for neurosurgeons due to factors such as the increase in civil wars, terrorist activities, and the global rise in civilian gun ownership. AIM: To evaluate the prognostic factors in patients with cranial gunshot wounds. METHODS: The current study included 124 patients who received either conservative or surgical treatment for cranial gunshot wounds between October 2014 and April 2024. The data including age, gender, clinical parameters (Glasgow coma scale (GCS), pupillary examination, admission time, high or low-velo

Fracture Patterns in Craniofacial Gunshot Wounds: A Seven-Year Experience.

OBJECTIVE: To characterize facial fracture patterns and understand predictors of surgical repair and LOS with the objective of assisting providers in managing and understanding these complex injuries. STUDY DESIGN: This is a retrospective cohort chart review study. METHODS: A retrospective review was conducted for patients admitted with gunshot wounds (GSWs) to the head, neck, or face between January 2013 and March 2020 at a level one trauma tertiary care hospital. Univariate and multivariate analysis were performed to identify associations with surgical repair and LOS. RESULTS: Of the 578

Scoring system for unstable spinal gunshot wounds: the study protocol.

UNLABELLED: Morphology of injuries following gunshot wounds requires specific treatment approaches. Currently, there are no similar classifications for assessing fracture stability with subsequent tactical recommendations. Taking into account diagnostic limitations (contraindications for MRI due to implantable metal fragments, limitations of functional radiography of the spine in seriously injured patients), we make decisions considering CT data. In this study, we will determine severity of vertebral damage and effect of these damages on mechanical stability of spinal motion segments. In the f

Retained bullet fragments after nonfatal gunshot wounds: epidemiology and outcomes.

BACKGROUND: With no consensus on the optimal management strategy for asymptomatic retained bullet fragments (RBF), the emerging data on RBF lead toxicity have become an increasingly important issue. There are, however, a paucity of data on the magnitude of this problem. The aim of this study was to address this by characterizing the incidence and distribution of RBF. METHODS: A trauma registry was used to identify all patients sustaining a gunshot wound (GSW) from July 1, 2015, to June 31, 2016. After excluding deaths during the index admission, clinical demographics, injury characteristics,

Complex craniomaxillofacial gunshot wounds A step-by-step multidisciplinary approach.

BACKGROUND: Gunshot injuries of the viscerocranium are rarely reported. Penetrating wounds to the cranio-maxillofacial region pose a significant challenge for surgeons as they often comprise serious soft tissue, bone and cerebral defects. We present a case report of a 42-year old female with a gunshot wound to the viscerocranium after suicidal attempt. Series of image of the disease course are available. CASE DESCRIPTION: A 42-year-old female presented with gunshot wound to the viscerocranium after a suicidal attempt. At the arrival GCS was 8/15 and general examination showed the inlet wound

Functional Outcomes After Gunshot Wounds to the Foot and Ankle.

The peer-reviewed, clinical data focusing on foot and ankle gunshot wounds are limited. The present study aimed to evaluate functional outcomes in a case series according to the area of injury, articular involvement, and the presence of infection. From January 2003 through February 2011 (8 years), 37 patients treated at Sinai-Grace Hospital (Detroit, MI) for civilian gunshot wounds localized to the foot and/or ankle were reviewed. Of these, 27 (72.97%) met the inclusion criteria. All acute wounds were thoroughly irrigated in the emergency room (8 of 27, 29.63%) or operating room (19 of 27, 70.

Transmediastinal gunshot wounds in a mature trauma centre: changing perspectives.

BACKGROUND: Transmediastinal gunshot wounds are associated with a high mortality and frequently require operative intervention. The purpose of this study was to identify the diagnostic and therapeutic challenges of these injuries in a mature trauma system with decreasing prehospital time intervals. METHODS: Patients admitted to a large urban Level 1 trauma centre between 1/2006 and 12/2010 sustaining a firearm injury to the torso were identified. Transmediastinal gunshot wounds were defined as missile tracts traversing the mediastinum identified on CT images, operative notes or autopsy report

Spine injuries are common among asymptomatic patients after gunshot wounds.

BACKGROUND: Spine injuries after gunshot wounds are thought to be rare among asymptomatic patients. The occurrence of spine injuries among asymptomatic patients with gunshot wounds was studied to determine the necessity for mandatory spine immobilization and radiographic imaging. METHODS: In this retrospective cohort study, initial physical examination, radiographic findings, and final diagnosis and treatment were reviewed. Patients were included if they were admitted to the authors' level 1 trauma center with gunshot wounds to the head, neck, or trunk during a 10-year period. Spine injuries

Transcatheter arterial embolization for intractable epistaxis secondary to gunshot wounds.

PURPOSE: To evaluate the efficacy of transcatheter arterial embolization for intractable epistaxis secondary to gunshot wounds. MATERIALS AND METHODS: Seven patients with intractable epistaxis secondary to penetrating trauma (gunshot wounds) were studied with angiography and subsequently underwent embolization with particles (polyvinyl alcohol, gelatin sponge) and/or microcoils. Clinical follow-up included standard hemodynamic monitoring, serial hematocrit determinations, and clinical observation for recurrent bleeding. RESULTS: Diagnostic angiography demonstrated evidence of acute arterial

Gunshot wounds traversing the mediastinum: guidelines for evaluation and management.

When there is suspicion that a penetrating would to the chest has crossed the midline, it is incumbent upon the medical staff to actively exclude injuries to mediastinal structures such as the heart, aortic arch, bronchial tree, and esophagus, even though injury to such structures may not be immediately clinically apparent. The successful management of a patient with a gunshot wound apparently traversing the anterior mediastinum is presented; an esophagram identified an unsuspected through-and-through injury to the esophagus. The evaluation of this patient is discussed, and an algorithm for th

Characteristics of firearms and gunshot wounds as markers of suicide.

Data from a total of 1,200 cases that the Medical Examiner of Dallas County, Texas, ruled as suicide with a firearm have been compared with the results of forensic laboratory examinations and tests; 75% are with handguns and 25% are with long guns. The incidence of suicides by gender yields 82% males and 18% females, with the most common entrance site being the head. With regard to racial distribution of persons committing suicide, > 88% are white in the study population group, which is 76% white. Data are presented for the incidence of blood detected inside and on the muzzle end of the barrel

[Endoprosthesis of the elbow joint in sequelae of gunshot wounds].

5 patients have been implanted the endoprostheses of Sivash K. M. to restore functions of the upper limb after the gunshot injury in the area of elbow joint with the clinico-roentgenologic bone defect detection. Coarse cicatricial changes in the area of an articulation with presence of foreign bodies are characteristic for those patients. There have been achieved significant improvement of restoration of active movement of elbow joint and upper limb in whole with 4 of them in spite of high danger of possible post-operative inflammation. In one case due to hard physical labour there have been n

Management of nonpenetrating stab and gunshot wounds of the chest.

Records of 102 patients with gunshot and stab wounds to the chest but with normal roentgenogram of the chest upon admission were reviewed. There were no delayed hemothoraces or pneumothoraces. The three complications probably resulted from admission to the hospital and would not have occurred if the patients had maintained their normal daily activities. The cost of these admissions was $104,319, and no benefit resulted from them. A significant number of routine studies were done with an extremely low yield. These patients need not be admitted to the hospital unless there is some other compelli

Simple wound closure compared with surgery for civilian cranial gunshot wounds.

OBJECTIVE: A carefully selected subset of civilian cranial gunshot wound (CGSW) patients may be treated with simple wound closure (SWC) as a proactive therapy, but the appropriate clinical scenario for using this strategy is unknown. The aim of this study was to compare SWC and surgery patients in terms of their neurological outcomes and complications, including infections, seizures, and reoperations. METHODS: This was a single-center, retrospective review of the prospectively maintained institutional traumatic brain injury and trauma registries. Included were adults who sustained an acute CG
