Risk Factors for Epidemic Cholera in Lusaka, Zambia-2017.

On October 6, 2017, the Zambia Ministry of Health declared a cholera outbreak in Lusaka. By December, 1,462 cases and 38 deaths had occurred (case fatality rate, 2.6%). We conducted a case-control study to identify risk factors and inform interventions. A case was any person with acute watery diarrhea (≥ 3 loose stools in 24 hours) admitted to a cholera treatment center in Lusaka from December 16 to 21, 2017. Controls were neighbors without diarrhea during the same time period. Up to two controls were matched to each case by age-group (1-4, 5-17, and ≥ 18 years) and neighborhood. Surveyors int

Electrodiagnostic consultations in Zambia: Referral characteristics and neuromuscular disorders.

INTRODUCTION: Research on neuromuscular disorders in sub-Saharan Africa is scarce. We aimed to delineate referral characteristics and the neuromuscular disorders observed among electrodiagnostic (EDX) consultations in a tertiary care setting in Zambia. METHODS: EDX records were reviewed for all specialist-performed studies after the establishment of the laboratory. The frequency of demographic, medical characteristics, and final EDX impressions are presented. RESULTS: Among 108 referrals, 52% were male, 84% were adults (mean age 44 years). Referrals were predominantly outpatients (85%) and s

Diversity of Mycobacterium tuberculosis genotypes circulating in Ndola, Zambia.

BACKGROUND: Tuberculosis (TB) is one of the major public health problems in Zambia. However, information about lineages of M. tuberculosis complex (MTBC) isolates useful for epidemiology investigations is unknown. In this study, we investigated the diversity of MTBC isolates from Ndola, a typical Zambian urbanized city with a documented high HIV prevalence. METHODS: This was part of a prospective cohort study in subjects with sputum smear-positive pulmonary TB. Spoligotyping was used to genotype the MTBC isolates and establish the circulating lineages. The 15-locus Mycobacterial Interspersed

A multisite inventory study of neonatal syringe pumps in Zambia.

BACKGROUND: The global neonatal mortality rate is due to miss the third Sustainable Development Goal of 12 deaths per 100 live births by 2030. However, medical devices can play a crucial role in improving the quality of care given to neonates, helping to reduce the rate further. METHODS: This study focuses on quantifying the availability of syringe pumps to newborns at key hospital locations in Zambia. Inventories of syringe pumps on neonatal intensive care units (NICUs) were conducted at 7 different hospitals. RESULTS: Syringe pumps were only available on 3 of the 8 NICUs. Only 1 of the 13

Seroepidemiology of selected transboundary animal diseases in goats in Zambia.

Transboundary pathogens of goats present significant constraints to the livelihoods of millions of farmers in countries such as Zambia. Consequently, this study aimed to investigate the seroprevalence of Mycoplasma capricolum subsp. capripneumoniae (Mccp), foot and mouth disease virus (FMDV), Brucella spp., Crimean Congo haemorrhagic fever virus (CCHFV), and Rift Valley fever virus (RVFV) in Zambian goats. Another aim was to identify associations between seroprevalence and different predictor variables, such as trade and border proximity. From September to October 2019, 962 serum samples were

Taenia solium control in Zambia: The potholed road to success.

The taeniosis/cysticercosis neglected zoonotic disease complex is caused by Taenia solium, and is associated with significant economic and public health impacts. This paper reviews the current knowledge on T. solium in Zambia and the control strategies already studied, covering almost 20 years of research, and explores the way forward. Studies on occurrence of porcine cysticercosis indicated very high prevalences, ranging from 15 to 34% based on detection of circulating antigens, and of 46% to 68% based on full carcass dissection in slaughter age pigs. Taeniosis prevalences have been reported

Schistosomiasis is associated with incident HIV transmission and death in Zambia.

BACKGROUND: We examined relationships between schistosome infection, HIV transmission or acquisition, and all-cause death. METHODS: We retrospectively tested baseline sera from a heterosexual HIV-discordant couple cohort in Lusaka, Zambia with follow-up from 1994-2012 in a nested case-control design. Schistosome-specific antibody levels were measured by ELISA. Associations between baseline antibody response to schistosome antigens and incident HIV transmission, acquisition, and all-cause death stratified by gender and HIV status were assessed. In a subset of HIV- women and HIV+ men, we perfor

An analysis of trypanocidal drug use in the Eastern Province of Zambia.

As part of the development of a strategy for the control of bovine trypanosomosis in Zambia, a survey was conducted to quantify and qualify the current use of trypanocidal drugs (diminazene aceturate and isometamidium chloride) in a tsetse-controlled and a tsetse-infested area of the Eastern Province. A total of 207 trypanocide users were interviewed. Questions were posed on herd structure, trypanocidal drug preference, treatment strategy, reason for treatment, method of treatment and treatment frequency. The majority of the cattle owners preferred to use diminazene aceturate rather than isome

Health sector reform and tuberculosis control: the case of Zambia.

SETTING: Zambia, 1995-1997. OBJECTIVE: To describe the process leading to the collapse of Zambia's National Tuberculosis Programme NTP). DESIGN: A descriptive analysis of health sector reform in Zambia and its effects on the NTP during the period 1995-1997. RESULTS: By the end of 1997 the NTP had stopped functioning. The main reason was that external support had ended, while the National Strategic Health Plan 1995-1999 had no budget for special programmes according to the policy to integrate these into the general health services. As a consequence, technical support for tuberculosis control

Barriers to care for patients with neurologic disease in rural Zambia.

The awesome burden of treatable yet untreated neurologic disease in the developing world presents a humanitarian crisis to those of us with neurologic expertise from more privileged situations. Although increased economic resources are critically needed, a shortage of personnel to care for these patients is as great a problem. It is neither feasible nor desirable to propose training neurologists to work in these regions. However, COs could be selected to receive additional training and return to their home regions to serve as resources for referrals and as community educators. Such a training
