Synergistic effect of nitazoxanide with neuraminidase inhibitors against influenza A viruses in vitro.

The emergence of drug-resistant influenza A virus (IAV) strains represents a serious threat to global human health and underscores the need for novel approaches to anti-influenza chemotherapy. Combination therapy with drugs affecting different IAV targets represents an attractive option for influenza treatment. We have previously shown that the thiazolide anti-infective nitazoxanide (NTZ) inhibits H1N1 IAV replication by selectively blocking viral hemagglutinin maturation. Herein we investigate the anti-influenza activity of NTZ against a wide range of human and avian IAVs (H1N1, H3N2, H5N9, H

Viral Respiratory Infections: Influenza.

The influenza virus is a highly contagious respiratory pathogen that causes yearly seasonal epidemics and occasional pandemics, leading to significant health and economic consequences. It spreads through droplets, aerosols, and surfaces. Infected individuals are contagious before symptoms appear, with peak infectivity early in the illness. In the United States, the 2024-2025 influenza season was the most severe since 2017-2018, with an estimated 47 to 82 million cases, up to 1.3 million hospitalizations, and up to 130,000 deaths. The risk of adverse outcomes is higher for vulnerable groups, in

Review of the clinical effectiveness of the neuraminidase inhibitors against influenza B viruses.

Influenza A and B viruses cause significant morbidity and mortality worldwide each year. The neuraminidase inhibitors (NAIs) are the most commonly used class of influenza antiviral drugs for the treatment of infected patients. In vitro studies have shown that influenza B viruses are significantly less susceptible to oseltamivir and other neuraminidase inhibitors compared with influenza A viruses. Following analysis of published clinical studies, we show that oseltamivir does appear to have lower effectiveness in patients infected with influenza B virus compared with influenza A infected patien

Replacement of neuraminidase inhibitor-susceptible influenza A(H1N1) with resistant phenotype in 2008 and circulation of susceptible influenza A and B viruses during 2009-2013, South Africa.

BACKGROUND: Data on the susceptibility of influenza viruses from South Africa to neuraminidase inhibitors (NAIs) are scarce, and no extensive analysis was done. OBJECTIVES: We aimed to determine oseltamivir and zanamivir susceptibility of influenza A and B virus neuraminidases (NAs), 2007-2013, South Africa. PATIENTS/METHODS: We enrolled participants through national influenza-like illness surveillance, 2007-2013. Influenza diagnosis was by virus isolation and quantitative polymerase chain reaction (qPCR). Drug susceptibility was determined by chemiluminescence-based NA-STAR/NA-XTD assay. Sa

Antiviral Susceptibility of Highly Pathogenic Avian Influenza A(H5N1) Viruses Circulating Globally in 2022-2023.

The antiviral susceptibility of currently circulating (2022-2023) highly pathogenic avian influenza (HPAI) A(H5N1) viruses was assessed by genotypic and phenotypic approaches. The frequency was low for neuraminidase (NA) and polymerase acidic (PA) substitutions associated with reduced inhibition by NA inhibitors (21/2698, 0.78%) or the PA inhibitor baloxavir (14/2600, 0.54%). Phenotypic testing of 22 clade 2.3.2.1a and 2.3.4.4b viruses revealed broad susceptibility to NA inhibitors and baloxavir for a conclusion that most contemporary HPAI A(H5N1) viruses retain susceptibility to antiviral dru

Viral RNA polymerase: a promising antiviral target for influenza A virus.

Influenza A virus, which has a high rate of morbidity and mortality, has become a serious threat to human health and society. Because of antigenic variation, the application of influenza vaccination is limited. Till now, the current antivirals are mainly against the M2 protein (such as adamantanes) and the neuraminidase (such as zanamivir and oseltamivir). However, many viral strains have developed resistance to these marketed antiviral drugs, and it is urgent to find new antivirals for the prevention and treatment of influenza. RNA polymerase that is crucial for the replication and transcript

Drug-induced linear immunoglobulin A bullous dermatosis mimicking Stevens-Johnson syndrome: a case report.

Linear immunoglobulin A (IgA) bullous dermatosis (LABD) is a rare autoimmune disorder characterized by vesiculobullous mucocutaneous eruptions. LABD also has been reported as a drug-induced reaction. Idiopathic LABD and drug-induced LABD are clinically indistinguishable and can resemble bullous pemphigoid, dermatitis herpetiformis, or bullous erythema multiforme. LABD is diagnosed with direct immunofluorescence (DIF), and idiopathic LABD can be distinguished from drug-induced LABD with a careful medication history. We present the case of a 54-year-old man with drug-induced LABD after ingestion

Clinical Implications of Antiviral Resistance in Influenza.

Influenza is a major cause of severe respiratory infections leading to excessive hospitalizations and deaths globally; annual epidemics, pandemics, and sporadic/endemic avian virus infections occur as a result of rapid, continuous evolution of influenza viruses. Emergence of antiviral resistance is of great clinical and public health concern. Currently available antiviral treatments include four neuraminidase inhibitors (oseltamivir, zanamivir, peramivir, laninamivir), M2-inibitors (amantadine, rimantadine), and a polymerase inhibitor (favipiravir). In this review, we focus on resistance issue

Neuraminidase inhibitors for the treatment and prevention of influenza.

The impact of influenza virus infection is estimated to run into billions of dollars worldwide. Vaccination plays a key role in prevention; however, vaccines do not provide complete protection against influenza due to the constant mutation of the virus responsible. Unlike amantadine and rimantadine, which are only effective against influenza A, the new neuraminidase inhibitors zanamivir (Relenza), GlaxoSmithKline) and oseltamivir (Tamiflu), Gilead/Roche) are potent and specific inhibitors of influenza types A and B and have minimal side effects. The greatest benefit is derived if treatment com

[Neuraminidase inhibitors in prophylaxis and treatment of influenza].

Influenza is an acute infectious disease which may cause spectacular health, social and economic effects. Influenza may be prevented and treated in a sufficient way. The paper presents general rules of usage of current antiviral drugs--neuraminidase inhibitors--in prophylaxis and treatment of seasonal, avian and pandemic influenza. The paper describes mechanism of action of neuraminidase inhibitors (oseltamivir and zanamivir), their main pharmacokinetic features, dosing and side effects. There was emphasized the necessity of the early introduction of treatment with neuraminidase inhibitors (36

Antiviral Activity of Probenecid and Oseltamivir on Influenza Virus Replication.

Influenza can cause respiratory infections, leading to significant morbidity and mortality in humans. While current influenza vaccines offer varying levels of protection, there remains a pressing need for effective antiviral drugs to supplement vaccine efforts. Currently, the FDA-approved antiviral drugs for influenza include oseltamivir, zanamivir, peramivir, and baloxavir marboxil. These antivirals primarily target the virus, making them vulnerable to drug resistance. In this study, we evaluated the efficacy of the neuraminidase inhibitor, oseltamivir, against probenecid, which targets the h

Screening method for the analysis of antiviral drugs in poultry tissues using zwitterionic hydrophilic interaction liquid chromatography/tandem mass spectrometry.

A screening method for the analysis of seven anti-viral drugs in poultry tissue has been developed. These include anti-influenza drugs (amantadine, rimantadine, zanamivir and oseltamivir and its carboxylate metabolite), anti-herpes drugs (acyclovir and ganciclovir) and an immunomodulator (imiquimod). Poultry tissue was extracted in acetonitrile:water:acetic acid. After sample purification, using a strong cation exchange column, the eluate was split into two fractions. The first portion was dissolved in methanol:water and the second in acetonitrile:methanol:water. Both fractions were analysed o

Influenza antivirals and animal models.

Influenza A and B viruses are among the most prominent human respiratory pathogens. About 3-5 million severe cases of influenza are associated with 300 000-650 000 deaths per year globally. Antivirals effective at reducing morbidity and mortality are part of the first line of defense against influenza. FDA-approved antiviral drugs currently include adamantanes (rimantadine and amantadine), neuraminidase inhibitors (NAI; peramivir, zanamivir, and oseltamivir), and the PA endonuclease inhibitor (baloxavir). Mutations associated with antiviral resistance are common and highlight the need for furt

[Potential antiviral therapeutics for 2019 Novel Coronavirus].

The recent outbreak of respiratory illness in Wuhan, China is caused by a novel coronavirus, named 2019-nCoV, which is genetically close to a bat-derived coronavirus. 2019-nCoV is categorized as beta genus coronavirus, same as the two other strains - severe acute respiratory syndrome coronavirus (SARS-CoV) and Middle East respiratory syndrome coronavirus (MERS-CoV). Antiviral drugs commonly used in clinical practice, including neuraminidase inhibitors (oseltamivir, paramivir, zanamivir, etc.), ganciclovir, acyclovir and ribavirin, are invalid for 2019-nCoV and not recommended. Antiviral agents for influenza, hepatitis C and herpesvirus, enterovirus and rhinovirus infections.

Advances in antiviral therapy have involved both development of new, effective drugs and modification of pre-existing drugs or regimens to increase effectiveness. New agents against influenza virus are the neuraminidase inhibitors zanamivir and oseltamivir, which target specific viral processes and have minimal side effects. New agents for herpesviruses (famciclovir, valaciclovir, valganciclovir) have greater oral bioavailability, allowing less frequent dosing, but mechanisms of action are similar to older agents (aciclovir, ganciclovir). Pleconaril has some activity against enteroviruses and

Mathematical quantification of binding complementarity using intersection surface area (ISA) and geometric sensitivity index (GSI).

The R292K mutation in Influenza Neuraminidase (NA) significantly hinders the efficacy of clinical inhibitors, yet the physical basis of this resistance remains under-explored. In this study, we introduce a novel computational framework integrating Molecular Dynamics (MD) simulations with a geometric Intersection Surface Area (ISA) model. Our results demonstrate that the R292K mutation induces a structural destabilization characterized by a ∼11.5 kcal/mol increase in binding free energy and a 15-21% reduction in intersection binding volume ([Formula: see text]). Notably, Zanamivir (Ligand D) ex
