Patient-Reported Feedback Suggests an Alternative Sweet Spot for Deep Brain Stimulation Programming in Essential Tremor.

BACKGROUND: Deep brain stimulation (DBS) of the ventral intermediate nucleus (VIM) and caudal zona incerta is an established therapy for essential tremor (ET). Clinical outcomes depend on precise electrode placement and optimal stimulation parameters. Effective programming must balance tremor suppression with side-effect risk, yet systematic incorporation of patient-reported feedback remains limited. OBJECTIVE: The aim was to assess whether subjective patient feedback, quantified via a Visual Analogue Scale (VAS), can guide DBS programming for effective tremor control. METHODS: In 15 VIM-DBS

Parabrachial neuropeptide Y Y1 receptor-expressing neurons govern the sensory dimension of neuropathic pain.

Pain perception involves complex sensory and emotional processes, yet the supraspinal circuits that sustain neuropathic pain remain incompletely understood. Here, we demonstrate that parabrachial nucleus (PBN) neurons expressing the inhibitory G-protein-coupled neuropeptide Y Y1 receptor (Npy1r) are critical modulators of neuropathic pain. Approximately 19% of lateral PBN neurons express Npy1r. Stimulus-evoked Fos activation and calcium dynamics of these neurons increases in mice with spared nerve injury (SNI) compared to sham controls. Pharmacological activation of Y1 receptors or chemogeneti

Dual regulatory mechanisms of the ZI-LHb circuit in SPS&S model mice: dissociable control of aversion and anxiety through distinct downstream targets.

Post-traumatic stress disorder (PTSD) involves maladaptive aversion processing and anxiety disorders; however, the correlation between these components and their underlying neural circuit mechanisms remains unclear. The single prolonged stress and shock (SPS&S) model reliably induced PTSD-related aversive behaviors in mice through functional reorganization of the zona incerta (ZI)→lateral habenula (LHb) neural pathway. The ZI primarily projected to the lateral subregion of the LHb (LHbL) and consisted predominantly of GABAergic neurons. During the SPS&S expression phase, mice exhibit weakened

Optimising DBS in essential tremor: clinical and neurophysiological considerations for targeting and programming.

Deep brain stimulation (DBS) is an established treatment for essential tremor (ET), with the ventral intermediate nucleus (Vim) of the thalamus and the posterior subthalamic area (PSA), including the zona incerta and dentato-rubro-thalamic tract (DRTT), serving as the primary stimulation targets. These structures lie along the cerebello-thalamo-cortical pathway but differ in anatomical location and fibre composition, potentially influencing clinical effects and side effect profiles. We aim to compare the anatomical and physiological considerations of Vim and PSA targeting in ET, explore the im

Possible rodent equivalent of the posterior cingulate cortex (area 23) interconnects with multimodal cortical and subcortical regions.

Posterior cingulate cortex (area 23, A23) in human and monkeys is a critical component of the default mode network and is involved in many diseases such as Alzheimer's disease, autism, depression, attention deficit hyperactivity disorder and schizophrenia. However, A23 has not yet identified in rodents, and this makes modeling related circuits and diseases in rodents very difficult. Using a comparative approach, molecular markers and unique connectional patterns this study has uncovered the location and extent of possible rodent equivalent (A23~) of the primate A23. A23 ~ but not adjoining are

Flexible vs. standard subthalamic stimulation in Parkinson disease: A double-blind proof-of-concept cross-over trial.

BACKGROUND: Deep brain stimulation (DBS) of the subthalamus (STN) is effective for the treatment of cardinal motor signs of Parkinson disease (PD). Structures around the STN can suppress dyskinesia and tremor (zona incerta) and improve gait and balance (substantia nigra pars reticulata). OBJECTIVE: Is the newer 8-contact linear lead connected to a 'flexible' DBS system superior to standard 4-contact stimulation in PD patients receiving STN DBS? METHODS: After 3 months of open label programming, 10 patients were randomized to standard or flexible stimulation before crossing over to the other

The alpha 1 subunit of soluble guanylyl cyclase is expressed prenatally in the rat brain.

The mRNA encoding the alpha 1 subunit of soluble guanylyl cyclase (alpha 1sGC) was identified in a different-display screening for genes spatially and temporally regulated during the development of fetal rat brain. The initially isolated fragment of the 3' untranslated region was used for in situ hybridization and to produce full-length cDNA clones by hybridization screening of cDNA libraries and by RACE (rapid amplification of cDNA ends), respectively. In situ hybridization analysis that alpha 1sGC was absent at embryonic day 12 (E12), but by E14-E15, the forebrain exhibited dense expression

Long-range inputome of cortical neurons containing corticotropin-releasing hormone.

Dissection of the neural circuits of the cerebral cortex is essential for studying mechanisms underlying brain function. Herein, combining a retrograde rabies tracing system with fluorescent micro-optical sectional tomography, we investigated long-range input neurons of corticotropin-releasing hormone containing neurons in the six main cortical areas, including the prefrontal, somatosensory, motor, auditory, and visual cortices. The whole brain distribution of input neurons showed similar patterns to input neurons distributed mainly in the adjacent cortical areas, thalamus, and basal forebrain

Interleaving Stimulation in Parkinson's Disease, Tremor, and Dystonia.

BACKGROUND/AIMS: Interleaving stimulation (ILS) in deep brain stimulation (DBS) provides individualized stimulation of 2 contacts delivered in alternating order. Currently, limited information on the utility of ILS exists. The aims of this study were to determine the practical applications and outcomes of ILS DBS in Parkinson's disease (PD), tremor, and dystonia. METHODS: We performed a single-center, unblinded, retrospective chart review of all patients with DBS attempted on ILS at our referral center assessing for rationale and outcomes. RESULTS: Fifty patients (PD, n = 27; tremor, n = 7;

Deep Brain Stimulation for Tremor: Is There a Common Structure?

BACKGROUND: Subthalamic nucleus (STN) stimulation has been recognized to control resting tremor in Parkinson disease. Similarly, thalamic stimulation (ventral intermediate nucleus; VIM) has shown tremor control in Parkinson disease, essential, and intention tremors. Recently, stimulation of the posterior subthalamic area (PSA) has been associated with excellent tremor control. Thus, the optimal site of stimulation may be located in the surrounding white matter. AIMS: The objective of this work was to investigate the area of stimulation by determining the contact location correlated with the b

Deep brain stimulation and tremor.

Deep brain stimulation (DBS) has been used to treat various tremor disorders for several decades. Medication-resistant, disabling essential tremor (ET) is the most common tremor disorder treated with DBS. The treatment has been consistently reported to result in significant benefit in upper extremity, as well as head and voice tremor, all of which were improved more dramatically with bilateral procedures. These benefits have been demonstrated to be sustained for up to 7 years. DBS has also been shown to be beneficial for the tremor associated with multiple sclerosis and post-traumatic tremor;

Brainstem motor loops in the control of movement.

In recent years, the role of the area around the upper brainstem, particularly the pedunculopontine (PPN) region and the zona incerta (ZI), in the initiation and control of movement has generated much clinical interest. Using electrophysiological and pharmacological methods, we have further explored these structures and their influence in motor control in the nonhuman primate and in patients with proximal tremor. We have found that lesioning the PPN and electrical stimulation at high frequencies of the PPN region in the normal-behaving primate induces akinesia, and low frequency stimulation ca

Patterns of increased and decreased ingestive behavior after injections of lithium chloride and 2-deoxy-D-glucose.

In previous research, 750 mg/kg 2DG yielded a number of effects suggesting a postdrug nausea; and the present experiments revealed that the illness-inducing agent LiCl similarly produced taste aversion learning at 7.5 mg/kg, inhibited drinking in water-deprived animals at 30 mg/kg, depressed feeding in hungry rats at 60 mg/kg, and evoked food intake and pica at 120 mg/kg. The appearance of eating and pica at the same dosage suggested that rats may eat food as well as a nonnutritive substance as a species-specific reaction to illness and that postdrug feeding, including that observed after 2DG,

Thalamic and subthalamic deep brain stimulation for essential tremor: where is the optimal target?

BACKGROUND: The ventrolateral thalamus (ventral intermediate nucleus [Vim]) is the traditional target for neurosurgical treatment of essential tremor. The target, however, has varied substantially among different neurosurgeons. OBJECTIVE: To evaluate the effect of deep brain stimulation in the thalamus and posterior subthalamic area (PSA) in relation to electrode location. METHODS: Thirty-six (17 Vim/19 PSA) patients with 44 deep brain stimulation electrodes were included in this retrospective study. The effect of stimulation was evaluated with standardized settings for each contact using it

Long-term effects of unilateral deep brain stimulation on voice tremor in patients with essential tremor.

INTRODUCTION: Voice tremor (VT) is a common symptom of Essential tremor (ET). Deep brain stimulation (DBS) is an established treatment for ET overall, however, its effect on VT is less clear. The aim of this study was to evaluate long-term effects of DBS on VT and to investigate how VT symptoms develop over time in patients with ET. METHODS: VT scores for the cohort of 81 ET patients that had undergone DBS surgery in the caudal zona incerta (cZi) were analyzed retrospectively. Thirty-four patients had preoperative VT and long-term evaluations were available for 19 patients. Longitudinal effec

Deep Brain Stimulation for Essential Tremor: A Comparison of Targets.

BACKGROUND: Deep brain stimulation (DBS) is an established treatment for refractory essential tremor (ET). Initially, the target of choice was the thalamic ventralis intermedius nucleus (VIM). However, the zona incerta (ZI) has been suggested as a superior target. Both targets are considered safe and effective, but a direct comparison between these targets is lacking. METHODS: We analyzed a single-center cohort of 44 patients with ET treated with DBS between 1998 and 2017, targeting the VIM and/or ZI. Sweet spot for resting-state functional MRI effect of deep brain stimulation in dystonia lies in the lower pallidal area.

INTRODUCTION: Deep brain stimulation (DBS) of the internal globus pallidus (GPi) is a well-established, effective treatment for dystonia. Substantial variability of therapeutic success has been the one of the drivers of an ongoing debate about proper stimulation site and settings, with several indications of the notional sweet spot pointing to the lower GPi or even subpallidal area. METHODS: The presented patient-blinded, random-order study with cross-sectional verification against healthy controls enrolled 17 GPi DBS idiopathic, cervical or generalised dystonia patients to compare the effect

Individual Mesopontine Neurons Implicated in Anesthetic Loss-of-consciousness Employ Separate Ascending Pathways to the Cerebral Cortex.

The mesopontine tegmental anesthesia area (MPTA) is a small brainstem nucleus that, when exposed to minute quantities of GABAA receptor agonists, induces a state of general anesthesia. In addition to immobility and analgesia this state is accompanied by widespread suppression of neural activity in the cerebral cortex and high delta-band power in the electroencephalogram. Collectively, MPTA neurons are known to project to a variety of forebrain targets which are known to relay to the cortex in a highly distributed manner. Here we ask whether ascending projections of individual MPTA neurons coll

Post-Traumatic Tremor and Thalamic Deep Brain Stimulation: Evidence for Use of Diffusion Tensor Imaging.

BACKGROUND: Deep brain stimulation (DBS) is a well-established treatment to reduce tremor, notably in Parkinson disease. DBS may also be effective in post-traumatic tremor, one of the most common movement disorders caused by head injury. However, the cohorts of patients often have multiple lesions that may impact the outcome depending on which fiber tracts are affected. CASE DESCRIPTION: A 20-year-old man presented after road traffic accident with severe closed head injury and polytrauma. Computed tomography scan showed left frontal and basal ganglia hemorrhagic contusions and intraventricula

[The use of neuromodulation for the treatment of tremor].

BACKGROUND: Tremor may be a disabling disorder and pharmacologic treatment is the first-line therapy for these patients. Nevertheless, this treatment may lead to a satisfactory tremor reduction in only 50% of patients with essential tremor. Thalamotomy was the treatment of choice for tremor refractory to medical therapy until deep brain stimulation (DBS) of the ventral intermedius nucleus (Vim) of the thalamus has started being used. Nowadays, thalamotomy is rarely performed. METHODS: This article is a non-systematic review of the indications, results, programming parameters and surgical tech
