Steinert syndrome and repercussions in dental medicine.

Steinert syndrome, also called myotonic dystrophy type 1, is a genetic disorder with autosomal dominant transmission characterized by myotonia and a multisystemic clinical picture that affects several tissues of the human body. The most common systemic phenotypes are: muscular, cardiac, respiratory, CNS, ocular, gynecological, digestive, orthopedical, as well as cognitive and psychological symptoms (cognitive decline). Muscles involved in voluntary movement are highly affected by myotonia especially distal muscles of upper limbs. These patients also show changes in face, chewing and pharynx mu

A Survey of the Management of Patients with Temporomandibular Disorders by General Dental Practitioners in Southern Brazil.

PURPOSE: Temporomandibular disorders (TMD) are recognized as one of the most controversial topics in dentistry, despite the fact that both basic science and clinical researchers have currently reached some degree of consensus. This study aimed to conduct a questionnaire-based survey about the management of TMD patients by general dental practitioners (GDPs). MATERIALS AND METHODS: One hundred fifty-one GDPs with a private practice in a city of southern Brazil were included, independent of school of origin, gender, graduation year, and curriculum content. All participants were administered a q

The Singapore Early Psychosis Intervention Programme (EPIP): A programme evaluation.

UNLABELLED: The alarmingly long duration of untreated psychosis (DUP) in Singapore and probable severe consequences were the impetus for establishing the Early Psychosis Intervention Programme (EPIP) in 2001. In 2007, EPIP became a part of the National Mental Health Blueprint. Here, we report some of the programme's outcomes. METHOD: Consecutive patients accepted into EPIP from April 2007 to March 2011 were included. Programme inclusion criteria were: (1) age between 15 and 41 years, (2) first-episode psychotic disorder with no prior or minimal treatment, (3) no current history of substance a

Classifications of vocalic segments from articulatory kinematics: healthy controls and speakers with dysarthria.

PURPOSE: In this study, the authors classified vocalic segments produced by control speakers (C) and speakers with dysarthria due to amyotrophic lateral sclerosis (ALS) or Parkinson's disease (PD); classification was based on movement measures. The researchers asked the following questions: (a) Can vowels be classified on the basis of selected measures of articulatory motions? and (b) Can classification models that are constructed from control productions classify vowels produced by speakers with dysarthria that is related to ALS and PD? METHOD: Nineteen C, 7 PD, and 8 ALS speakers participat

Management of patients with chronic kidney disease.

Chronic kidney disease (CKD) is a silent disease which worsens gradually to end-stage kidney disease (ESKD). US kidney disease outcomes quality initiative (KDOQI) guidelines indicate five stages of CKD based on the severity of kidney function which is assessed by estimating the glomerular filtration rate (GFR) by the modification of diet in renal disease (MDRD) formula. The management of CKD patients with mild renal damage (stage 1-2 KDOQI) is articulated on the reduction of proteinuria (<500 mg/day), reduction of sitting systolic and diastolic blood pressure (<130/80 mmHg), salt restriction d

Occlusal management for a patient with aural symptoms of unknown etiology: a case report.

BACKGROUND: Although the discrepancy between the habitual occlusal position (HOP) and the flat bite plate-induced occlusal position (BPOP) (regarded as the muscular physiological reference position) has been recently reported to be related to symptoms of temporomandibular disorders (TMDs), it still remains unclear whether the occlusal equilibration in the reference position is effective to resolve TMD-related discrepancy and symptoms. Aural symptoms (otalgia, tinnitus, vertigo et cetera) have been included under TMD symptoms. METHODS: To examine the effect of occlusal equilibration for the tr

Embodied harm and health consequences: self-reported physiological symptoms among co-victims of cold-case homicides.

BACKGROUND: Families of victims of unresolved ("cold case") homicides endure not only prolonged emotional devastation but also chronic exposure to stressors linked to institutional inaction and uncertainty. While previous studies have addressed trauma, grief, and mental health consequences among co-victims of homicide, minimal scholarly attention has been devoted to the physical and physiological impact of unresolved loss, particularly when justice remains indefinitely suspended. METHODS: This qualitative study draws on in-depth semi-structured interviews with 13 co-victims of cold-case homic

Art therapy in addiction recovery the face of addiction through artworks based on clinical case studies.

Given the multifaceted nature of addiction, a multidisciplinary and comprehensive therapeutic approach is essential. Art therapy has therefore become an important complementary intervention in both inpatient and outpatient treatment settings. Numerous studies support its applicability and effectiveness among individuals with addiction. Art therapy can be particularly beneficial in reducing pervasive feelings of shame and anxiety, as well as in facilitating the processing of traumatic experiences - phenomena that are common among people with substance use disorder (SUD) and are often difficult

Brain basis of phonological awareness for spoken language in children and its disruption in dyslexia.

Phonological awareness, knowledge that speech is composed of syllables and phonemes, is critical for learning to read. Phonological awareness precedes and predicts successful transition from language to literacy, and weakness in phonological awareness is a leading cause of dyslexia, but the brain basis of phonological awareness for spoken language in children is unknown. We used functional magnetic resonance imaging to identify the neural correlates of phonological awareness using an auditory word-rhyming task in children who were typical readers or who had dyslexia (ages 7-13) and a younger g

New design of a palatal lift prosthesis combined with a palatal bar.

OBJECTIVE: In the past, palatal lift prostheses (PLPs) have been problematic with respect to stability. This stability problem has been addressed by designing a PLP that includes a palatal bar. STRUCTURE: A PLP combined with a palatal bar is functionally characterized by a palatal bar that holds down the posterior palatal plate. By making this location the fulcrum, the force exerted by the soft palate on the lamina actually functions to stabilize the prosthesis instead of acting to overturn it. CASES: This new prosthetic design was applied to patients with cleft palate and those who had unde

Facial animation in patients with Moebius and Moebius-like syndromes.

Moebius syndrome, a rare congenital disorder of varying severity, involves multiple cranial nerves and is characterised predominantly by bilateral or unilateral paralysis of the facial and abducens nerves. Facial paralysis causes inability to smile and bilabial incompetence with speech difficulties, oral incompetence, problems with eating and drinking, including pocketing of food in the cheek and dribbling, as well as severe drooling. Other relevant clinical findings are incomplete eye closure and convergent strabismus. The authors report on 48 patients with Moebius and Moebius-like syndromes

A four-year longitudinal study of palatal plate therapy in children with Down syndrome: effects on oral motor function, articulation and communication preferences.

The orofacial function in 20 children with Down syndrome was evaluated after 4 years of palatal plate therapy in 9 of the children (PPG); the remaining 11 were untreated age-matched controls (CG). All 20 children had received continuous orofacial physical therapy from their speech therapist during the treatment period. A clinical extra- and intraoral examination was performed, including oral motor function, facial expression, the occurrence of malocclusions, and hypertrophic tonsils. A questionnaire requesting data on breathing patterns, drooling, eating problems, and communicative preferences

Elucidating the role of mind-body connection profiles in emotional reactivity and regulation amongst typically developed adults.

BACKGROUND: Adaptive emotional functioning necessitates a strong mind-body connection. The 13-item Body-Mind Connection Questionnaire (BMCQ) was developed to measure attention directed to sensations (Interoceptive Attention), identifying and describing sensations associated with emotions (Sensation-Emotion Articulation), and beliefs regarding mind-body integration and wellbeing (Body-Mind Values). This study aimed to (1) confirm and refine the BMCQ in a new sample of typically developed adults through confirmatory factor analysis (CFA), (2) identify distinct mind-body connection profiles throu

Clinical experience with the AKT1 inhibitor miransertib in two children with PIK3CA-related overgrowth syndrome.

BACKGROUND: PIK3CA-related overgrowth spectrum (PROS) refers to a group of rare disorders, caused by somatic activating mutations in PIK3CA, resulting in abnormal PI3K-AKT-mTOR pathway signalling. Significant associated morbidity is frequently observed, and approved treatments are lacking. Miransertib (ARQ 092) is a novel, orally available, selective pan-AKT inhibitor with proven in vitro efficacy. Following recent results of the use of AKT inhibitors in Proteus syndrome (PS) and AKT-mutant cancers, we investigated its therapeutic use in two patients with severe PROS who had exhausted conventi

Medical profile of the language-delayed child: otitis-prone versus otitis-free.

Delay in language development may be associated with an underlying anatomical, neurosensory, or psychological disorder such as: deafness, cerebral palsy, cleft palate, autism, or mental retardation. A condition called specific developmental language delay may occur in children devoid of any other identifiable disorder or developmental delay. Language delay associated with early onset, severe-to-profound hearing impairment has been well documented. Controversial studies have also appeared in the communicative disorders' literature suggesting that fluctuating conductive hearing loss in early chi

A case of cortical deafness and anarthria.

Generally, cortical deafness is not complicated by anarthria and cortical anarthria does not affect auditory perception. We report a case of simultaneous progressive cortical deafness and anarthria. At the age of 70 years, the patient, a woman, noticed hearing problems when using the telephone, which worsened rapidly over the next 2 years. She was then referred to our hospital for further examinations of her hearing problems. Auditory tests revealed threshold elevation in the low and middle frequencies on pure-tone audiometry, a maximum speech discrimination of 25% and normal otoacoustic emiss

Genetic forms of primary progressive aphasia within the GENetic Frontotemporal dementia Initiative (GENFI) cohort: comparison with sporadic primary progressive aphasia.

Primary progressive aphasia is most commonly a sporadic disorder, but in some cases, it can be genetic. This study aimed to understand the clinical, cognitive and imaging phenotype of the genetic forms of primary progressive aphasia in comparison to the canonical nonfluent, semantic and logopenic subtypes seen in sporadic disease. Participants with genetic primary progressive aphasia were recruited from the international multicentre GENetic Frontotemporal dementia Initiative study and compared with healthy controls as well as a cohort of people with sporadic primary progressive aphasia.
