What Are the Complications of Allograft Reconstructions for Sarcoma Resection in Children Younger Than 10 Years at Long-term Followup?

BACKGROUND: Preservation of limb function after resection of malignant bone tumors in skeletally immature children is challenging. Resection of bone sarcomas and reconstruction with an allograft in patients younger than 10 years old is one reconstructive alternative. However, long-term studies analyzing late complications and limb length discrepancy at skeletal maturity are scarce; this information would be important, because growth potential is altered in these patients owing to the loss of one physis during tumor resection. QUESTIONS/PURPOSES: At a minimum followup of 10 years after reconst

External fixation or flexible intramedullary nailing for femoral shaft fractures in children. A prospective, randomised study.

We report the outcome of 19 children aged 5.2 to 13.2 years with 20 fractures of the femoral shaft requiring surgery, who were randomly assigned to have external fixation (EF) or flexible intramedullary nailing (FIN) (10 fractures each). The duration of the operation averaged 56 minutes for the EF group with 1.4 minutes of fluoroscopy, compared with 74 minutes and 2.6 minutes, respectively, for the FIN group. The early postoperative course was similar, but the FIN [corrected] group showed much more callus formation. The time to full weight-bearing, full range of movement and return to school w

Femoral internal torsion greater than twenty-five degrees and/or external tibial torsion greater than thirty degrees as measured by computed tomography are threshold values for axial alignment correction in patellofemoral instability.

OBJECTIVES: Patellofemoral instability (PFI) has multiple predisposing anatomic factors, including ligamentous hyperlaxity, coronal and axial malalignment, patella alta, trochlea dysplasia, excessive lateral patellar tilt, and excessive lateral Q vector. Yet, few studies have analyzed surgical thresholds for performing axial alignment corrective osteotomies in the treatment of PFI and patella maltracking. The objective of this systematic literature review was to determine if there is a threshold for axial plane alignment that triggers surgical correction for the treatment of patellar instabili

A Case Report and Literature Review to Aid in the Management of Trunnion Failure in Hip Arthroplasty Patients: Can Trunnionosis and Prosthetic Joint Infection Co-Exist?

Trunnionosis is a type of corrosion and wear at the head-neck taper junction of the femoral implant, and it can be a slow and silent catastrophe. Simultaneous prosthetic joint infection (PJI) is occasionally possible based on the fulfillment of a few of the minor criteria from the Musculoskeletal Infection Society (MSIS), but the existing literature lacks adequate evidence to support that the infection actually exists. We are presenting a case of an 82-year-old man with right total hip arthroplasty performed over a decade prior to presenting to the emergency room with a sudden-onset pop follow

[Surgical treatment of the Charcot foot : long-term results and systematic review].

BACKGROUND: Because of extensive bone loss and the associated complex instability and deformity of the feet, Charcot arthropathy or neuroosteoarthropathy is a challenge for the orthopedic surgeon. Ulcerations offer entry of various bacteria; thus, infections are a frequent serious issue and complication. The careful choice of treatment is not only very important for the correct alignment of the foot and the loading capacity, but also contributes significantly to the prevention of skin irritation or the healing of existing ulcers. Above all, chronic plantar ulcerations, caused by prominent exos

Development of a standardized method of assessment of radiographs and radiographic change in juvenile idiopathic arthritis: introduction of the Dijkstra composite score.

OBJECTIVE: To evaluate the sensitivity to change of a newly developed radiologic assessment tool, the Dijkstra score, and to develop a numeric composite score and progressor classification scheme to apply in juvenile idiopathic arthritis (JIA) trials. METHODS: A placebo-controlled trial of sulfasalazine (SSZ) in patients with oligoarticular- and polyarticular-onset JIA yielded the data for this study. Data were obtained from 418 sets of radiographs of the clinically involved and contralateral joints (at study entry and at 6 months' followup) from 66 JIA patients. The Dijkstra score assesses t

Functional Outcome of Radius and Ulnar Shaft Fractures Treated With Intramedullary Kirschner's Wire Fixation in Children.

INTRODUCTION: Fractures are a common occurrence in childhood, with approximately one-third of boys and girls sustaining at least one fracture before the age of 17. Both-bone forearm fractures, particularly those involving the radius and ulna, are more common in the non-dominant hand and in boys and usually involve the distal portions of both bones. If not properly treated, these injuries can have a significant impact on limb function. OBJECTIVE: This study aimed to determine the functional outcomes of radius and ulnar shaft fractures in children treated with intramedullary Kirschner's wire fi

Total knee replacement in the fixed valgus deformity using a lateral approach: role of the automatic iliotibial band release for a successful balancing.

The purpose of this work was to document eleven years of experience in knee replacement for fixed knee valgus through a lateral approach with special emphasis on the balancing procedures. At a mean follow-up of seven years, only one revision for sepsis was required in this series of 63 knee replacements. The mean knee score improved from 37 (range 20-45) to 91 (range 65-100) at the last review (p < 0.01) while the function score increased from 29.5 (range 0-50) to 78.7 (range 10-100) (p = 0.01). The mean mechanical axis (HKA) was 14.7 degrees of valgus preoperatively and 1 degrees of valgus po

Comparative Study of Post-operative Pain and Functional Outcomes in Robotic-assisted Total Knee Replacement versus Cruciate-retaining Manual Total Knee Replacement.

INTRODUCTION: Robotic-assisted total knee replacement (TKR) has emerged as an important technological advancement in arthroplasty surgery, designed to overcome the classical limitations of conventional jig-based instrumentation. Traditional manual techniques rely heavily on standard mechanical alignment systems and visual estimation, which introduce a higher margin for component malalignment and suboptimal soft-tissue balancing. By utilizing precise image-dependent or image-free navigation and haptic feedback loops, robotic arthroplasty provides real-time adjustments and micro-millimetric bone

A Comparative Analysis of Medial Versus Lateral Unicompartmental Arthroplasty: A Case-Matched Analysis of Patient-Reported Outcomes.

Introduction Unicompartmental knee arthroplasty (UKA) is a bone-preserving alternative to total knee arthroplasty (TKA) for isolated compartmental osteoarthritis (OA). The medial compartment is most frequently affected due to varus malalignment, while lateral disease, associated with valgus alignment, is less common. Although UKA offers faster recovery and improved function compared to total knee replacement, direct comparisons between medial and lateral UKA outcomes are limited. This study aims to compare patient-reported outcomes between medial and lateral UKA using a retrospective case-matc

Long-term results of remodeling of lateral condylar prominence after lateral closed-wedge osteotomy for cubitus varus.

BACKGROUND: Despite the ability of lateral closed-wedge osteotomy for cubitus varus deformity to restore carrying angle and preserve elbow motion, there have been reports of poor cosmetic appearance of the elbow because of lateral condylar prominence. METHOD: We evaluated long-term results of remodeling of lateral condylar prominence after osteotomy in 11 patients (7 of the prepuberty group, 4 of the postpuberty group). The follow-up period ranged from 4.7 to 14.2 years (average, 10). RESULTS: The mean preoperative and postoperative lateral condylar prominence index (LCPI) of the affected el
