Patellar stabilization surgeries in cases of recurrent patellar instability: a retrospective clinical and radiological audit.

BACKGROUND: Patellar instability is a disabling condition that limits the functional ability and physical aspirations of patients. There are multiple anatomical structures which stabilize the patella and surgical treatment is tailored to repair the underlying aetiology. AIMS: To evaluate the clinical and radiological outcome of patellar stabilization procedures in patients with recurrent patellar instability. METHODS: We analysed 34 patients (36 knees) (mean age, 26.6) with recurrent patellar instability who underwent patellar stabilization surgery from June 2009 to September 2014. Type of p

Isolated versus combined medial patellofemoral ligament reconstruction for lateral instability of the patella.

INTRODUCTION: Medial patellofemoral ligament reconstruction (MPFLR) is regularly combined with a tibial tuberosity transfer (TTT) in cases of recurrent patellar instability with underlying structural deformity. However, these indications for a TTT have recently come into question. This study aimed to assess the traditional indications by comparing the outcomes of isolated and combined MPFLR for the treatment of recurrent lateral patellar dislocation. METHODS: A systematic review was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines.

Histopathological and electron microscopic study in dogs with patellar luxation and skin hyperextensibility.

Patellar luxation is abnormal displacement of the patella from the femoral trochlear groove. It is seen primarily in small breed dogs and causes pain and limited mobility of the stifle joint. This study aimed to investigate the relationship among patellar luxation, skin extension, and skin collagen fibril diameter. Nine dogs with patellar luxation and five clinically normal dogs were enrolled in the study. We measured the skin extension and investigated the ultrastructure of the skin and patellofemoral ligament by histopathology and transmission electron microscopy. The mean skin extension in

Non traumatic dislocation of the patella in children: the case for a dysplastic aetiology.

Congenital dislocation of the patella may be a phenotype, which has several genotypes, i.e. it may be congenital or acquired. This hypothesis is suggested with three illustrative case studies. Risk factors predictive of the development of irreducible dislocation of the patella in infancy are suggested. As there is a spectrum of possible aetiologies, 'late' diagnosis and a number of 'risk' factors but a common pathology, the word 'congenital' is not always appropriate. We suggest 'Developmental dysplasia and dislocation of the patella (DDDP)' is a more appropriate term for this rare condition.

The management of superior dislocation of the patella with interlocking osteophytes--an update on a rare problem.

The superior dislocation of the patella with interlocking osteophytes is a rare condition. A review of the English literature revealed only 12 reported cases. The purpose of reviewing these case reports is to highlight the unusual presentation and the injury mechanism in 2 of our patients, and to present our treatment algorithm. Closed reduction with manipulation of the patella, with or without anaesthesia, was performed without difficulty. We recommend an intermediate step of trying a regional nerve block before proceeding to general anaesthesia. Our patients had full range-of-motion after re

Medial patellofemoral ligament reconstruction: a novel technique using the patellar ligament.

In patients with chronic patellofemoral instability, more than 2 episodes of dislocation, and an anterior tuberosity trochlear groove of less than 20 mm as measured on computed tomography or nuclear magnetic resonance imaging, we have developed a technique for medial patellofemoral ligament reconstruction that uses a medial strip of the patellar ligament (PL). The incision started proximally at the level of the superior margin of the patella, centrally between the patellar medial margin and the medial epicondyle. A descending incision was then made, directed toward the superomedial margin of t

ACR Appropriateness Criteria Acute Trauma to the Knee.

More than 500,000 visits to the emergency room occur annually in the United States, for acute knee trauma. Many of these are twisting injuries in young patients who can walk and bear weight, and emergent radiographs are not required. Several clinical decision rules have been devised that can considerably reduce the number of radiographs ordered without missing a clinically significant fracture. Although a fracture is seen on only 5% of emergency department knee radiographs, 86% of knee fractures result from blunt trauma. In patients with a fall or twisting injury who have focal tenderness, eff

Medial patellofemoral ligament: anatomy, injury and treatment in the adolescent knee.

PURPOSE OF REVIEW: Traumatic and nontraumatic patellofemoral instability (PFI) in children and adolescents is a complex problem. It is determined by a large number of mechanical and pathomorphologic conditions, mainly seen in nontraumatic dislocations. RECENT FINDINGS: Although conservative treatment with a short immobilization, followed by early passive motion and isometric quadriceps strengthening, can be considered in real traumatic dislocations without any cartilaginous injury, a surgical intervention should be considered in atraumatic cases. As 90% of PFI are nontraumatic and correlated

Robotic-Arm Assisted Total Knee Arthroplasty Demonstrated Soft Tissue Protection.

INTRODUCTION: While total knee arthroplasty (TKA) procedures have demonstrated clinical success, occasionally intraoperative complications can occur. Collateral or posterior cruciate ligament injury, instability, extensor mechanism disruption, and tibiofemoral or patellofemoral dislocation are among a few of the intraoperatively driven adverse events prevalently ranked by The Knee Society. Robotic-arm assisted TKA (RATKA) provides a surgeon the ability to three-dimensionally plan a TKA and use intraoperative visual, auditory, and tactile feedback to ensure that only the desired bone cuts are m

Rehabilitation and Return to Sport After Medial Patellofemoral Complex Reconstruction.

The medial patellofemoral complex (MPFC) consists of the medial patellofemoral ligament and medial quadriceps tendon femoral ligament, which play a critical role stabilizing the patella against lateral translation. After a patellar dislocation, athletes with recurrent dislocations have functional limitations that may limit their return to their prior level of competition, requiring surgical reconstruction. Although ample literature exists delineating return-to-play (RTP) considerations after anterior cruciate ligament reconstruction, there is a paucity of evidence specific to MPFC reconstructi

Medial or lateral dislocation (snapping) of a portion of the distal triceps: a biomechanical, anatomic explanation.

Medial and lateral snapping (dislocation) of the distal triceps over the epicondyle during elbow flexion has been reported but is frequently misdiagnosed and is not well understood. In this study a mathematical model was designed to simulate the effect that bony abnormalities at the distal humerus and soft tissue variations of the distal triceps have on the line of pull of the triceps. The predictions were then tested on prefabricated and fabricated plastic elbow models, as well as 8 cadaveric elbows. When the bony alignment was altered, varus angulation had the greatest effect: 30 degrees var

Patients with major depressive disorder and undiagnosed depression are at risk for inferior outcomes after patellar stabilization surgery.

INTRODUCTION/OBJECTIVES: Medial patellofemoral ligament reconstruction (MPFL-R) is commonly performed in patients with recurrent patellar dislocations and associated instability. However, there is limited information on the effects of underlying mental health conditions, such as major depressive disorder (MDD), on operative outcomes. This study assesses how mental health disorders, such as MDD or unrecognized depression, can affect pre- and post-operative outcomes in patients undergoing MPFL-R. METHODS: A retrospective analysis of patients who underwent MPFL-R from 2015 to 2023 was performed.
