Obsessive-compulsive spectrum disorders: a multidimensional approach.

One advantage of conceptualizing two disorders as related lies in the possibility that a similar treatment is effective for both conditions. The approach taken in this paper is somewhat different, however. The authors have argued that although a particular psychobiologic process may be relevant to OCD and obsessive-compulsive conditions, it may be disrupted indifferent ways in various conditions that fall along a spectrum. If so, differ-ent obsessive-compulsive spectrum disorders may well require different treatments. Thus, although OCD and trichotillomania can be conceptualized in terms of th

The demography, phenomenology, and family history of 22 persons with compulsive hair pulling.

This study reports on the demographics, phenomenology, and family history of 22 compulsive hair pullers. Subjects were recruited through a psychiatric outpatient clinic as well as newspaper advertisements. Subjects completed a semistructured interview focused on hair-pulling behavior and demographics, the Diagnostic Interview Schedule to assess major (Axis I) mental disorders, and the Structured Interview for DSM-III-R Personality Disorders to assess Axis II disorders. The typical subject was a 33-year-old woman who had completed 1 or more years of college and had been pulling her hair for 19

Dermatopsychosomatics: classification, physiology, and therapeutic approaches.

Psychosomatic dermatology is practiced in some manner by every dermatologist. In spite of this, there has been a virtual void in the literature from the middle 1950s until the present time. The relationship to physiologic phenomena, as well as a classification of psychosomatic dermatology, is reviewed. Dermatologic patients are divided into three groups: the primary group in which the emotional disorder is the primary disease and the cutaneous disorder merely part of its expression; the secondary group in which the basic cause is organic but does not affect the person emotionally in various de

[The dialectic of the obsessionality and the dissociativity: toward a psychopathology of binge-eating].

Binge-eating is one of the most paradoxical phenomena in the symptomatology of anorexia nervosa and bulimia nervosa. The patients seem to be struggling to control their eating behaviors as strictly as possible, and at the same time, they repeat chaotic patterns of binge-eating. The author investigated the descriptive psychopathology of binge-eating in this paper. The author recognized the impulsive nature of binge-eating at first, and compared it with the clinical features of so-called impulse control disorders, such as kleptomania, trichotillomania, and so on. According to the literature revi

Recurrent Rapunzel syndrome in a young woman: Diagnostic pitfalls and surgical strategy.

INTRODUCTION AND IMPORTANCE: Trichobezoars are typically localized to the stomach but can rarely extend into the small bowel, causing a condition known as Rapunzel syndrome. This rare entity primarily affects young females with psychiatric conditions such as trichotillomania and trichophagia. The recurrent nature of this syndrome and its association with psychological disorders necessitate a multidisciplinary approach. PRESENTATION OF CASE: A 21-year-old female presented to the emergency department with abdominal pain, nausea, and vomiting persisting for four days. On physical examination, sh

Frequency of Types of Alopecia in a Single-centre Hair Referral Clinic Over a Ten Years Period.

BACKGROUND: The frequencies of the different types of alopecia in hair referral centres have so far been reported in a single multicentre study at multiple specialist hair clinics over a time period of one month. AIM: Single center studies over a longer time-frame offer a more representative and homogeneous study population with regard to ethnic, demographic, and climatic factors, and seasonality of hair growth and shedding than multicenter studies over a short time frame. MATERIALS AND METHODS: Retrospective study of patient data at a single centre over 10 years. RESULTS: A total of 15'211

Variable clinical presentations of secondary delusional infestation: an experience of six cases from a psychodermatology clinic.

OBJECTIVE: Delusional Infestation (DI) is a relatively rare condition with a fixed belief of being infested with living organisms, despite a lack of medical evidence of such infestation. Although it seems to be a psychiatric disease, patients commonly are admitted to dermatology clinics because of skin findings. Psychiatrists can underestimate its prevalance, whereas dermatologists can miss the diagnosis. It should be managed as a psychodermatological disease. Our aim in the study was to evaluate six patients with different clinical presentations of DI and to emphasize some clinical features.

Fifty Years of Lithium in Obsessive-Compulsive Disorder and Related Disorders: A Systematic Review of Clinical Evidence Challenging a Theoretical Assumption.

INTRODUCTION: Obsessive-compulsive disorder (OCD) and related disorders (OCRDs) represent a group of conditions in which lack of response to first-line treatments, such as serotonin reuptake inhibitors (SRIs), is common. Therefore, exploring alternative strategies is necessary. We conducted a systematic review of the use of lithium in OCRDs. METHODS: This systematic review was conducted in accordance with PRISMA guidelines. We included studies evaluating the use of lithium for OCRDs in adults. Eligible designs included case reports, case series, observational studies, and clinical trials. Fou

Complementary medicine, self-help, and lifestyle interventions for obsessive compulsive disorder (OCD) and the OCD spectrum: a systematic review.

BACKGROUND: In Obsessive Compulsive Disorder (OCD) current standard pharmacotherapies may be of limited efficacy. Non-conventional interventions such as Complementary and Alternative Medicine (CAM), self-help techniques, and lifestyle interventions are commonly used by sufferers of OCD, however to date no systematic review of this specific area exists. METHODS: We conducted a systematic review of studies using CAM, self-help, and lifestyle interventions for treatment of OCD and trichotillomania (TTM). Gastric trichobezoar: abdominal mass in a child with sickle cell disease.

Abdominal pain is a frequent occurrence among the pediatric population and can be a diagnostic challenge. Trichobezoar is a differential diagnosis that is often neglected. Different from previously reported cases, we present a 3-year-old girl with sickle cell disease with complaints of acute abdominal pain, suspecting sickle cell splenic sequestration. The child presented to the emergency department with sharp epigastric pain and an associated palpable upper abdominal mass. This case illustrates a large obstructing gastric trichobezoar and summarizes both the diagnostic modalities and treatmen

A case report of an autistic girl with an extra bisatellited marker chromosome.

Cytogenetic examination of a 14-year-old severely retarded girl revealed a karyotype of 47 chromosomes with an extra bisatellited chromosome, a translocation between No. 22 and a chromosome in the D group. The girl had presented an early autistic syndrome beginning about 6 months of age during plastering for a congenital luxation of the hips and receding from the age of 5. In addition, she was hyperkinetic with various aggressive and auto-aggressive traits and had atypical minor epileptic fits. Data from child psychiatric examinations at 5 and 14 years are presented. The importance of giving p

A preliminary survey of rhinotillexomania in an adolescent sample.

BACKGROUND: Rhinotillexomania is a recent term coined to describe compulsive nose picking. There is little world literature on nose-picking behavior in the general population. METHOD: We studied nose-picking behavior in a sample of 200 adolescents from 4 urban schools. RESULTS: Almost the entire sample admitted to nose picking, with a median frequency of 4 times per day; the frequency was > 20 times per day in 7.6% of the sample. Nearly 17% of subjects considered that they had a serious nose-picking problem. Other somatic habits such as nail biting, scratching in a specific spot, or pulling
