Sulcus fixation without capsular support in children.

PURPOSE: To evaluate long-term follow-up in eyes of children who had sulcus fixation of an intraocular lens (IOL) without capsular support. SETTING: St. Eriks Eye Hospital/Karolinska Institute, Stockholm, Sweden. METHODS: This retrospective study included 21 eyes of 13 children. Seven eyes had Marfan's syndrome, 7 essential lens dislocation, 2 perforation with lens injury, and 5 spherophakia. The IOL implantation was primary in 16 eyes and secondary in 5 eyes. Lensectomy was performed with a limbal approach. An IOL with holes in the haptics was sutured in the sulcus, with the knots buried in

Results following secondary intraocular lens implantation in children.

BACKGROUND: Placement of a secondary intraocular lens (IOL) in a child may be considered in children with congenital monocular cataracts who have had complete opacities removed early in life and who later become contact lens intolerant, in eyes that have received trauma which precluded placement of a primary IOL, and in young adults who have bilateral aphakia who become resistant to use of contact lenses or spectacles. METHODS: Clinical records of all children in our practice who received a secondary IOL between January 1988 and December 1994 were reviewed. Indications, biometry, type of proc

Visual outcome of secondary lens implantation after trauma or complicated retinal detachment surgery.

PURPOSE: Visual outcome of intraocular lens (IOL) implantation as a separate surgical procedure after severe trauma of the anterior or posterior segment or after vitreoretinal surgery for complicated retinal detachment resulting in aphakia was evaluated. METHODS: The charts of all patients undergoing secondary IOL implantation for the above reasons between 1988 and 1992 were retrospectively reviewed. A special transcleral suturing technique was developed for the posterior chamber lenses, where needles are primarily passed from the outside through the sclera to enhance correct positioning of t

Penetrating keratoplasty with anterior or posterior chamber intraocular lens implantation.

OBJECTIVE: To compare the outcome of anterior chamber (AC) intraocular lens (IOL) implantation with the outcome of posterior chamber (PC) IOL implantation at the time of penetrating keratoplasty in the absence of capsular support. DESIGN: Retrospective study of 122 eyes that underwent secondary IOL implantation without capsular support at the time of penetrating keratoplasty. Ninety eyes received an AC IOL and 32 received a PC IOL sutured to the iris or the ciliary sulcus. RESULTS: Survival analysis showed the probability of a clear graft to be 86% for eyes with an AC IOL and 90% for eyes wi

Posterior chamber intraocular lens implantation in the absence of capsular support.

To avoid the complications associated with anterior chamber intraocular lenses (IOLs), we have developed a technique for the implantation of a posterior chamber IOL in the absence of capsular support. The IOL is secured in the ciliary sulcus by suturing the haptics to the sclera at the ciliary sulcus inferiorly and to the sclera or iris superiorly. We have used this technique for secondary IOL implantation in 16 contact lens-intolerant patients with aphakia with a mean follow-up of 9 months (range, 5 to 20 months) and in eight eyes at the time of IOL removal. All eyes with secondary implants h

Epikeratophakia in children.

In the past two years 29 patients (ages 9 months to 15 years) received 37 epikeratophakia grafts. 91.1 percent of the original surgeries were successful. The average change in refractive error was 16.7 diopters, and the average spectacle overcorrection was +0.2 diopters. We have obtained good visual results in our very young patients were follow-up is already over 1 year. We demonstrated also some improvement in visual acuity in older children especially those with traumatic or secondary cataracts. The study demonstrates that we can add an average of +17.1 diopters with an epikeratophakia graf

[Evaluation of the performance of an automatic objective refractometer on 3,618 eyes].

The RM 5000 Auto-refractor was evaluated for one year on 3,618 eyes of 2,037 non selected patients undergoing refraction. The objective automatic refractor showed a high degree of reliability and repeatability in determining the needed refractive correction. The performances were at least equivalent to the results given by keratometry and retinoscopy with an appreciable saving of time (about 4 minutes per patient) turned to account for clinical examination. The results were especially useful among aphakic patients and children under atropine. Automatic refractors are expected to become an esse

Prevalence of cataracts in a population-based study of persons with diabetes mellitus.

Persons with diabetes mellitus have been found to be at increased risk of developing cataracts when compared with nondiabetic persons. This report describes the study of characteristics which may be related to this problem in a population-based sample of diabetic persons. Prevalence of surgical aphakia and cataract increased with increasing age in both younger and older onset diabetic persons. Females had higher rates than males. In multivariate analyses, for younger onset persons, longer duration of diabetes, older age at examination, increased severity of retinopathy, diuretic usage and high

Helping the aphakic neonate to see.

The treatment of the infant who is born with sensory deprivation amblyopia is one of the most difficult therapeutic problem sets facing the ophthalmologist today. This group of anomalies has many forms, the most common of which seems to be unilateral and bilateral cataracts. Substantial advances have been made, however, in many aspects of therapy for infants who have this set of problems. We have come to realize that it is essential to address these conditions at the first moment that is consistent with the safety of the child. New techniques in cataract extraction have reduced the time course

[The first experience of iris-fixated intraocular lens implantation in patients with open-angle glaucoma].

The results of iris-claw intraocular lens (IOL) implantation in patients with lens dislocation, deficiency of capsule support in combination with glaucoma are presented. Iris-claw "Artizan" IOL was used in 16 eyes. IOL was implanted through 5 mm corneal incision into the posterior chamber with haptic fixated to iris stroma. Implantation of this IOL model is minimally traumatic, provides favorable visual outcome and does not lead to intraocular pressure decompensation in patients with glaucoma.

Adherence to occlusion therapy in the first six months of follow-up and visual acuity among participants in the Infant Aphakia Treatment Study (IATS).

PURPOSE: Achieving good vision in infants born with a unilateral cataract is believed to require early surgery and consistent occlusion of the fellow eye. This article examines the relationship between adherence to patching and grating acuity. METHODS: Data came from the Infant Aphakia Treatment Study, a randomized clinical trial of treatment for unilateral congenital cataract. Infants were either left aphakic (n = 53) or had an intraocular lens implanted (n = 55). Patching was prescribed 1 hour per day per month of age until 8 months of age and 50% of waking hours thereafter. Adherence was m
