Epiploic bypass flap: a new method of limb salvage. Anatomic basis and clinical application.

BACKGROUND: The incidence of critical limb ischemia increases with the aging of the population. Two-thirds of patients with critical limb ischemia present with trophic disorders. Revascularization decreases the rate of amputation. Infected wounds with exposure of the tendons, bones, or points of articulation cannot heal in spite of bridging and local debridement. Surgery associated with a distal venous bypass or recanalization and a free flap makes it possible to cover major tissue loss and offers a hemodynamic advantage by increasing the flow of the bypass, thanks to the vascular bed added by

Relationship between occlusal findings and orofacial myofunctional status in primary and mixed dentition: part IV: interrelation between space conditions and orofacial dysfunctions.

AIM: The aim of this study was to provide basic representative data on the prevalence of malocclusions involving space deficiency in both primary and early mixed dentition and to examine the relationship between these malocclusions and orofacial dysfunctions. The results should be viewed from an orthodontic prevention and early treatment perspective. SUBJECTS AND METHODS: Orthodontic findings in the maxilla and mandible as well as the myofunctional status of 766 children in primary dentition and 2,209 children in mixed dentition were examined clinically in a cross-sectional study. The followi

Elevated carcinoembryonic antigen tumour marker caused by head and neck cancer: a case report and literature study.

Carcinoembryonic antigen is a tumour marker commonly increased in gastrointestinal and pulmonary cancers. We report a case of a 46-year-old man with a mucoepidermoid carcinoma of the base of tongue with an elevated and traceable serum carcinoembryonic antigen level. This antigen proved to be a valuable marker in the treatment follow-up. When a raised carcinoembryonic antigen level is found, salivary gland malignancies should be taken into the differential diagnosis and clinical examination of the head and neck region should not be overlooked.

Speech outcomes of early palatal repair with or without intravelar veloplasty in children with complete unilateral cleft lip and palate.

We compared the early speech outcomes of 40 consecutive children with complete unilateral cleft lip and palate (UCLP) who had been treated according to different 2-stage protocols: the Malek protocol (soft palate closure without intravelar veloplasty at 3 months; lip and hard palate repair at 6 months) (n=20), and the Talmant protocol (cheilorhinoplasty and soft palate repair with intravelar veloplasty at 6 months; hard palate closure at 18 months) (n=20). We compared the speech assessments obtained at a mean (SD) age of 3.3 (0.35) years after treatment by the same surgeon. The main outcome me

Relapse of anterior open bites treated with orthodontic appliances with and without orofacial myofunctional therapy.

INTRODUCTION: Closure and long-term retention of anterior open bites are significant concerns for orthodontists and their patients. In this study, we investigated the efficacy of orofacial myofunctional therapy (OMT) for maintaining closure of open bites in conjunction with orthodontic treatment. METHODS: The sample included 76 subjects with dental anterior open bites referred for OMT before, during, or after relapse of orthodontic treatment. The experimental cohort consisted of 27 subjects who received OMT and orthodontic treatment or retreatment. The control cohort comprised 49 subjects who

Reproducibility of proton MR spectroscopic imaging (PEPSI): comparison of dyslexic and normal-reading children and effects of treatment on brain lactate levels during language tasks.

BACKGROUND AND PURPOSE: We repeated a proton echo-planar spectroscopic imaging (PEPSI) study to test the hypothesis that children with dyslexia and good readers differ in brain lactate activation during a phonologic judgment task before but not after instructional treatment. METHODS: We measured PEPSI brain lactate activation (TR/TE, 4000/144; 1.5 T) at two points 1-2 months apart during two language tasks (phonologic and lexical) and a control task (passive listening). Dyslexic participants (n = 10) and control participants (n = 8) (boys and girls aged 9-12 years) were matched in age, verbal

Reproducibility of 2 methods to locate centric relation in healthy individuals and TMD patients.

No conclusive evidence exists for any maxillomandibular relationship as the preferable treatment position. Measurement reliability of 2 different methods to attempt to locate centric relation in control and TMD patients was assessed to determine if both methods lead to the same position. A group of 27 controls and 91 TMD patients were examined using the Research Diagnostic Criteria for TMD (RDC/TMD). Three patient groups were recruited: 27 patients with myofascial pain (MYO), 34 patients with disc displacement without reduction (ID), and 30 patients with osteoarthritis (OA). For each study par

The role of plasticity-related functional reorganization in the explanation of central dyslexias.

This investigation explored the hypothesis that patterns of acquired dyslexia may reflect, in part, plasticity-driven relearning that dynamically alters the division of labour (DOL) between the direct, orthography → phonology (O → P) pathway and the semantically mediated, orthography → semantics → phonology (O → S → P) pathway. Three simulations were conducted using a variant of the triangle model of reading. The model demonstrated core characteristics of normal reading behaviour in its undamaged state. When damage was followed by reoptimization (mimicking spontaneous recovery), the model repr

A research study review of effectiveness of treatments for psychiatric conditions common to end-stage cancer patients: needs assessment for future research and an impassioned plea.

BACKGROUND: Rates of psychiatric conditions common to end-stage cancer patients (delirium, depression, anxiety disorders) remain unchanged. However, patient numbers have increased as the population has aged; indeed, cancer is a chief cause of mortality and morbidity in older populations. Effectiveness of psychiatric interventions and research to evaluate, inform, and improve interventions is critical to these patients' care. This article's intent is to report results from a recent review study on the effectiveness of interventions for psychiatric conditions common to end-stage cancer patients;

Infantile Anorexia and Co-parenting: A Pilot Study on Mother-Father-Child Triadic Interactions during Feeding and Play.

Infantile Anorexia (IA), defined by the Diagnostic Classification of Mental Health and Developmental Disorders of Infancy and Early Childhood Revised (DC: 0-3R, Zero To Three, 2005), occurs when the child (a) refuses to eat adequate amounts of food for at least 1 month, and shows growth deficiency, (b) does not communicate hunger and lacks interest in food, and (c) the child's food refusal does not follow a traumatic event and is not due to an underlying medical illness. IA usually emerges during the transition to self-feeding, when the child issues of autonomy are played out daily in the feed

[Post-traumatic stress, post-traumatic depression and major depressive episode: literature].

Although they are likely to add their effects, physical and psychic traumata (or traumas) can provoke in different ways the appearance of depressive symptoms sometimes common. Post-traumatic depression, reactional depression, major depressive disorder and post-traumatic stress disorder represent different clinical and nosographic disorders in despite of their occasionally common symptomatic core. Historically, it is interesting to note during the XXth century the true semantic change of the terms of trauma from the somatic field to the psychic sphere. Physical traumatism is often represented b

Towards a Better Definition of Patient-Centered Therapeutic Strategy of Parkinson's Disease: An Opinion Paper.

Recent Parkinson's disease (PD) treatment guidelines remain predominantly drug- and stage-centric and only inconsistently articulate a patient-centered medication strategy. We aimed to identify this gap and propose pragmatic, consensus-based principles for fully individualized, preference-sensitive pharmacological care across the patient journey. Seventeen movement disorder experts from eight European countries convened (March 2026) and generated key clinical questions spanning early to advanced PD. Panelists completed an anonymized questionnaire; semantic AI supported identification of areas

Differentiating upper- and lower motor neuron diseases using automated acoustic analysis.

OBJECTIVE: Motor neuron diseases (MNDs) result in a spectrum of motor impairments, including considerable effects on speech function, which manifest as dysarthria-a motor speech disorder. Speech metrics are increasingly recognized as critical biomarkers with potential utility in disease diagnosis and phenotyping. This study aimed to (1) characterize acoustics of upper motor neuron (UMN) and lower motor neuron (LMN) dysarthria presentations in MNDs, and (2) identify relationships between bulbar disease severity scores and acoustic features, as these could collectively enable personalized approa
