[Classification of dysphonias based on the primary etiologic factor (part II)].

INTRODUCTION: Phonation is a complex integral function of an organism. Regular phonation is characterized by: clearness and adequate pitch. Dysphonia is a disorder of phonation. It can have many acoustic forms, but hoarseness is the best known symptom of dysphonia. Acoustic phenomena in regard to voice are caused by: irregularities in vocal cord vibration, turbulent airflow in the glottis and obstruction of glottis. PREVIOUS CLASSIFICATIONS OF DYSPHONIAS: The best known classification of dysphonias was introduced by Perello. There are two groups: 1. organic dysphonias and 2. functional dyspho

Pseudomembranous supraglottitis with airway compromise in a patient with recurrent tonsillar carcinoma.

PURPOSE: In this report, we discuss a patient with acute pseudomembranous supraglottitis complicating recurrent tonsillar carcinoma and describe the ramifications of these disorders on perioperative management. CLINICAL FEATURES: The patient was an acutely ill man with a history of right tonsillar carcinoma originally treated with chemoradiation therapy and a radical neck dissection who presented with a brief history of fever, dyspnea, and stridor. The soft tissue of his neck was very stiff, his neck mobility was limited, and his mouth opening was restricted by pain and radiation-induced fibr

Tonsillar Post-transplant Lymphoproliferative Disorder Presenting With Hemoptysis After Heart Transplantation: A Case Report and Review of the Literature.

Post-transplant lymphoproliferative disorder (PTLD) is a potentially fatal complication of solid organ transplantation. Heart transplant recipients are among those at elevated risk for developing PTLD. Tonsillar involvement is a recognized manifestation, typically presenting as recurrent tonsillitis or progressive tonsillar hypertrophy, although hemorrhagic presentations are rare. We present the case of a 44-year-old man with nonischemic cardiomyopathy, status post-orthotopic heart transplantation in December 2024, who developed recurrent tonsillitis with progressive bilateral tonsillar hypert

Update on pneumococcal infections of the respiratory tract.

Respiratory illnesses are the leading reason for seeking medical care here in the United States. Streptococcus pneumoniae is the most common bacterial pathogen causing acute otitis media (AOM), sinusitis, and community-acquired pneumonia in both the pediatric and adult populations. The continued development of antibiotic resistance to an increasing number of different antibiotic classes by this organism has made the treatment of some of these infections more difficult. Recently, a heptavalent pneumococcal conjugate vaccine was approved for infants and toddlers, beginning at 2 months of age. Wi

Contemporary Indications and Technical Limits of Transoral Laser Microsurgery for Laryngeal Squamous Cell Carcinoma.

Transoral laser microsurgery (TOLMS) is an established organ-preserving treatment for early-stage and selected intermediate-stage laryngeal squamous cell carcinoma (SCC). Initially indicated for Tis-T1 lesions, its use has progressively expanded to carefully selected T2 and T3 tumors, providing oncologic outcomes comparable to open surgery and (chemo)radiotherapy [(C)RT] while offering favorable functional results. The CO₂ (carbon dioxide) laser is the preferred device for TOLMS due to its precision and minimal thermal injury, allowing accurate tumor resection with optimal margin control. Succ

Case Report: Sedation crisis caused by drug-induced sleep endoscopy in a pediatric patient with symptomatic multilevel airway obstruction.

BACKGROUND: Unexplained life-threatening events during pediatric sedation examinations are a major clinical challenge. Drug-induced sleep endoscopy (DISE) enables dynamic assessment of the site, severity, and pattern of airway collapse by simulating sleep through pharmacologically induced sedation. However, limited studies have utilized DISE as a diagnostic approach for elucidating the etiology of sedation-related events. CASE PRESENTATION: A male patient aged 10 years was admitted with a cough for >30 days and a history of cardiopulmonary resuscitation (CPR) 2 days earlier. The patient was d

Impaired pharyngeal reflex responses to negative pressure: a novel cause of sleep apnea in multiple sclerosis.

Obstructive sleep apnea (OSA) is common in people with multiple sclerosis (MS). However, people with MS often do not have "typical" anatomical risk factors (i.e., nonobese and female predominance). Accordingly, nonanatomical factors such as impaired upper-airway muscle function may be particularly important for OSA pathogenesis in MS. Therefore, this study aimed to investigate genioglossus (largest upper-airway dilator muscle) reflex responses to brief pulses of upper-airway negative pressure in people with OSA and MS. Eleven people with MS and OSA and 10 OSA controls without MS matched for ag

Serial evaluation of resting and exercising overground endoscopic examination results in young Thoroughbreds with no treatment intervention.

BACKGROUND: We hypothesised that grade/appearance for upper respiratory tract (URT) disorders identified at the first overground endoscopy (OGE) examination would vary at subsequent examinations. OBJECTIVES: To compare OGE examinations from horses evaluated on at least two occasions under similar exercise conditions without treatment intervention. STUDY DESIGN: Retrospective cohort. METHODS: Pre-exercise and exercising OGE recordings from Thoroughbred horses undergoing multiple examinations under similar exercise conditions were reviewed, with the first two recordings for each horse statist

Effect of wake-sleep transitions and rapid eye movement sleep on pharyngeal muscle response to negative pressure in humans.

1. Genioglossus (GG) activation in response to upper airway negative pressure may be an important mechanism in the maintenance of airway patency. This reflex occurs during wakefulness but is diminished during stable non-rapid eye movement (NREM) sleep. Since obstructive events occur more commonly at wake-sleep transitions and during rapid eye movement (REM) sleep than during stable NREM sleep, we assessed the GG reflex during these two vulnerable states. 2. Seventeen healthy adults were studied throughout one evening and overnight. Electroencephalograms (EEGs), electro-oculograms (EOGs), subme

Factors associated with occult ipsilateral and contralateral nodal metastasis in resectable lateralized laryngeal and hypopharyngeal malignancies.

OBJECTIVE: The incidence of occult lymph node metastasis is high in cT4aN0 laryngeal and lateral hypopharyngeal cancers, and bilateral selective neck dissection (levels II–IV) is routinely performed for oncologic clearance. However, the risk, distribution, and determinants of occult contralateral nodal involvement remain incompletely characterised. This retrospective cohort study evaluates the incidence of occult lymph node metastasis, the nodal stations commonly involved, and clinicopathological factors associated with contralateral occult nodal metastasis in patients with cT4aN0 laryngeal an

Enhanced recovery after surgery: a prospective cohort study on ibSLN preservation in transoral laser surgery for pyriform sinus carcinoma.

BACKGROUND: Transoral endoscopic head and neck surgery is an important modality in hypopharyngeal carcinoma therapy. Minimally invasive techniques and nerve protection are important components of Enhanced Recovery After Surgery principles. However, the limited surgical space and the challenges in identifying the internal branch of the superior laryngeal nerve (ibSLN) pose difficulties during this procedure. This study investigated the effects of preserving the ibSLN during transoral laser surgery on the postoperative swallowing rehabilitation of patients diagnosed with pyriform sinus carcinoma

The diagnostic accuracy of point-of-care ultrasound parameters for airway assessment in patients undergoing intubation in emergency department-an observational study.

BACKGROUND: Endotracheal intubation is an essential resuscitative procedure in the emergency setting. Airway assessment parameters such as the Mallampati classification are difficult to perform in an emergency setting. As point-of-care ultrasound (POCUS) assessment of airway parameters does not require patients to perform any mandatory action, ultrasound may become the potential first-line noninvasive airway assessment tool in the emergency department (ED). The use of POCUS in the ED has not been sufficiently studied. Using POCUS in airway assessment for predicting difficult intubation may be

The dysphonic videolaryngoscopy with stroboscopy paradox and challenge of acquired subglottic stenosis after laryngotracheal reconstruction.

OBJECTIVE: There's no greater challenge in pediatric laryngology than diagnosis and treatment of chronic dysphonia following laryngotracheal reconstruction of acquired subglottic stenosis. Videolaryngoscopy with stroboscopy provides incomparable diagnostic information to fiberoptic endoscopy. Unfortunately, this pediatric subpopulation which would benefit the most from videolaryngoscopy with stroboscopy infrequently does. We present the unique videolaryngostroboscopic patterns with their diagnostic and treatment implications in this complex population. METHODS: From a total database of 113 ch

A prospective study of upper aerodigestive tract manifestations of mucous membrane pemphigoid.

We conducted a prospective study between 1995 and 2002 to investigate nose and throat (NT) manifestations of mucous membrane pemphigoid (MMP). One hundred ten consecutive patients with clinical, histologic, and immunologic criteria of MMP were seen in 2 referral centers for bullous diseases. They were systematically asked about the existence of persistent NT symptoms. Patients who had any were examined with a flexible nasopharyngolaryngoscope by the same otorhinolaryngologist. When possible, NT mucous membrane (MM) biopsies were taken for direct immunofluorescence (IF) assays to determine lesi

[New clinico-therapeutic directions in upper airway pathology].

The main issues regarding the pathology of the primary airways in children, such as persistent rhinitis, recurrent disease of the pharynx and laryngitis, have been re-examined and updated on the basis of the latest findings regarding immunology and respiratory physiopathology as well as recent technological progress in the field of diagnosis. As far as rhinitis is concerned, recently demonstrations that nasal mucosa, like bronchial mucosa, responds to different kinds of stimuli with the same type of inflammation, have led to a new nosographic viewpoint: apart from classic allergic rhinitis, mu
