Rural and urban differences in patients with a dual diagnosis.

OBJECTIVES: To evaluate the differences between two cohorts of patients with severe mental illness (schizophrenia-spectrum or bipolar disorder) and co-occurring substance-use disorders, living in either predominantly rural areas or urban areas. METHODS: Two study groups of patients with a dual diagnosis, recruited using the same criteria, were evaluated, including 225 patients from New Hampshire and 166 patients from two cities in Connecticut. The two study groups were compared on demographic characteristics, housing, legal problems, psychiatric and substance use diagnoses, substance use and

Correlates of crack abuse among drug-using incarcerated women: psychological trauma, social support, and coping behavior.

This investigation examines the relationship between psychological trauma and crack abuse among 158 women with a recent history of drug use who were incarcerated in a New York City jail facility. Interviewers obtained data on demographics, drug use, psychological trauma history, criminal history, social support, and coping behavior variables. Three-fourths of the total sample had used crack three or more times a week for a month in the past; a quarter had used other drugs, predominantly heroin, three or more times a week for a month in the past. Multiple logistic regression analysis was used t

Correlates of codisorders in homeless and never homeless indigent schizophrenic men.

We recently completed a case-control study of 100 literally homeless and 100 never homeless indigent schizophrenic men in New York City, in which concurrent substance abuse and antisocial personality disorder were widespread. In this paper we probe the correlates of 'pure' schizophrenia (single disorder, N = 60), schizophrenia and substance abuse (double disorder, N = 89), and schizophrenia, substance abuse, and antisocial personality disorder (triple disorder, N = 50), across the homeless/never homeless distinction. Subjects were recruited from a homeless shelter and mental health service pro

The importance of lead management in the transcatheter tricuspid valve intervention era.

BACKGROUND: Percutaneous transcatheter tricuspid valve replacement (TTVR) has emerged as an effective treatment for severe tricuspid regurgitation (TR). Management of TR in the setting of transvenous leads is complex, requiring a multidisciplinary team-based approach. Transvenous lead extraction (TLE) may improve or worsen lead-related TR and is important when considering TTVR to avoid "jailing" leads between the transcatheter and native tricuspid valve. Data and guidance in these settings are limited. OBJECTIVE: This study aimed to describe the outcomes of a strategy using TLE in patients wi

Social determinants of health associated with initiation of evidence-based psychotherapy for posttraumatic stress disorder in a national sample of veterans.

Although the Veterans Health Administration (VHA) has strived to make posttraumatic stress disorder (PTSD) treatment widely available, the vast majority of veterans do not initiate evidence-based psychotherapy (EBP) for PTSD, which includes Prolonged Exposure (PE) and Cognitive Processing Therapy (CPT). One prominent gap in the literature on PTSD EBP initiation is research on the role of social determinants of health (SDoH). In the current study, we used national survey and VHA data to investigate relationships between SDoH factors and PTSD EBP initiation. We used data collected in 2018 from a

Exploring chronic pain and other clinical characteristics in drug treatment court participants.

BACKGROUND: Three out of four participants in Drug Treatment Courts (DTCs) present with co-occurring substance use disorders and mental health disorders (COD). Among individuals with COD outside criminal-legal settings, 71% report chronic pain. However, chronic pain has not been examined in the context of DTC participation. This secondary analysis compared intake treatment needs of DTC participants with COD, stratified by presence or absence of chronic pain history. METHODS: Participants with COD from five DTCs were compared: 61 with chronic pain history (36%) and 109 without (64%). Chronic p

Drug-coated balloons for coronary bifurcation lesions.

Coronary bifurcation lesions (CBLs) represent a common and challenging subset of coronary artery disease requiring percutaneous coronary intervention (PCI). While drug-eluting stents (DES) remain the cornerstone of treatment, their use is associated with risks such as restenosis, thrombosis, side branch (SB) jailing and the need for prolonged dual antiplatelet therapy. Drug-coated balloons (DCBs) have emerged as a promising alternative, delivering antiproliferative drugs without permanent implants, thereby reducing the risk of late complications and preserving native vessel geometry. This revi

New Opportunities for Health and Resilience Measures for Suicide Prevention (NO HARMS): protocol to investigate suicidal behaviours using linked multisystem administrative data.

INTRODUCTION: Suicide is a leading cause of death in the USA, and its incidence has increased substantially in recent years. Observational studies have used large, linked databases to investigate risk factors for suicide, but data generally originate from a small number of similar sources. Prevention efforts may be strengthened by linking data from diverse sources across multiple sectors. The New Opportunities for Health and Resilience Measures for Suicide Prevention project aims to develop a resource for characterising key risk and protective factors for suicide outcomes by linking state-leve

Structural Racism and Black-White Disparities in Homelessness in the United States.

BACKGROUND: Homelessness is a growing crisis consistently linked to adverse health. At the same time, race is strongly associated with homelessness risk. Thus, understanding and addressing racial disparities in homelessness is imperative for health equity. This study explores geographic variation in Black-to-White disparities in homelessness across the United States and examines how measures of local structural racism are associated with these inequities. METHODS: The study sample included 259 urban and suburban communities. The dataset was constructed by merging estimates of sheltered, unshe

The empirical clustering of crimes.

Despite wide use among researchers and law enforcement, crime classification structures have received little empirical study to validate their theoretical models. Specifically, few studies have examined the empirical structure of crime categories across a large range of offenses to examine adherence to traditional classification systems (e.g., violent vs. non-violent; crimes against persons, property, and society). This study first identified how crimes clustered together empirically in a sample of Florida jail inmates (Aim 1; N = 832, 68% men, 72% White, 83% non-Hispanic) and then tested the

Developing a conceptual framework for post-incarceration care for sexual and gender minoritized adults: a concept mapping study.

Mass incarceration in the United States is a social-structural driver of health for sexual and gender minoritized adults (SGMA). The aim of this study was to develop a conceptual framework for post-release healthcare to use in adapting a mobile health unit (MHU) for SGMA returning from jail. We used concept mapping to answer a focal prompt regarding health-related needs for SGMA post-release. We recruited SGMA with recent incarceration experience and relevant community partners to participate in 4 concept mapping sessions. Multidimensional scaling produced a two-dimensional point map of all st

Initial Experience with the New DERIVO® Mini Embolisation Device for the Treatment of Intracranial Aneurysms.

The aim of this study is to present the outcomes of cerebral aneurysm treatment with the DERIVO® mini Embolisation Device (DMD), which is compatible with microcatheters with 0.021-inch inner diameters. Consecutive patients treated with DMD were identified retrospectively. Patient and aneurysm characteristics, procedural findings, clinical outcomes and follow-up imaging results were evaluated. A total of 44 target aneurysms in 30 patients were treated with DMD. The mean age of the patients was 49.9 (range, 4-77 years). Four patients with five aneurysms presented with acute subarachnoid hemorrha

Pediatric Health and System Impacts of Mass Incarceration, 2009-2020: A Matched Cohort Study.

OBJECTIVE: The US has the highest incarceration rate in the world; incarceration's direct and indirect toll on the health and health care use of youth is rarely investigated. We sought to compare the health of youth with known personal or family justice involvement and a matched cohort of youth without known personal/family justice involvement. METHODS: A cross-sectional matched parallel cohort study was conducted. We queried electronic health records on youth (<21 years) with a visit in a large Midwestern pediatric hospital-based institution from January 2009 to December 2020. Youth were loc

Criminal legal system engagement among people who use drugs in Oregon following decriminalization of drug possession.

BACKGROUND: In February 2021, Measure 110 (M110) in Oregon decriminalized noncommercial possession of drugs. We examined criminal legal system (CLS) involvement of people who use drugs (PWUD) 2 years after decriminalization. METHODS: We conducted a quantitative survey of PWUD (N=468) in eight Oregon counties between March and November 2023. We ran multivariable models to examine predictors of CLS involvement and law enforcement stops. RESULTS: The majority of PWUD (74 %) reported any past year CLS involvement; 67 % had at least one law enforcement stop (mean of 11.4 and median of 3 law enfor

Incarceration history and HIV testing among people who inject drugs in the Boston metro area: a pooled cross-sectional study.

BACKGROUND: The persistent incidence of HIV among people who inject drugs (PWID) underscores the urgency for HIV prevention efforts to end the HIV epidemic. Little is known about the role carceral settings play as touchpoints for HIV testing in this population. METHODS: Secondary analysis of cross-sectional survey data of PWID in the Boston metro area from the 2015 and 2018 cycles of the National HIV Behavioral Surveillance (NHBS). Among self-reported HIV-negative participants, we examined incarceration and HIV testing histories and used a multivariable modified Poisson regression model to ev

Hypothermic sepsis in time since death estimation - a case report.

Both hyper- and hypothermia are problematic in temperature based forensic time since death estimation. Hyperthermia may occur in infection, traumatic brain injury, and intoxication. Hypothermia is encountered predominantly in exposure. Sepsis may present itself clinically as hypothermic. Sepsis is not uncommon in the forensic setting and mostly occurs in the context of malpractice accusations. There is usually little overlap between sepsis and typical forensic time since death estimation scenarios of violent or otherwise suspicious deaths. In the presented case, hypothermia and time since deat

Incarceration status at buprenorphine initiation and OUD treatment outcomes during pregnancy.

INTRODUCTION: Opioid use disorder (OUD) is a leading cause of pregnancy-associated deaths. OUD treatment with buprenorphine (BUP) reduces overdose risk and improves perinatal outcomes. Incarceration can be a barrier to receipt of OUD treatment during pregnancy and postpartum. The objective of this study was to examine differences in BUP continuation at delivery by patients' incarceration status at the time of BUP initiation. METHODS: This is a secondary analysis of a retrospective cohort study of pregnant patients with OUD who delivered at an academic medical center and initiated BUP between

Impact of San Francisco's New Street crisis response Team on Service use among people experiencing homelessness with mental and substance use disorders: A mixed methods study protocol.

Mobile crisis services for people experiencing distress related to mental health or substance use are expanding rapidly across the US, yet there is little evidence to support these specific models of care. These new programs present a unique opportunity to expand the literature by utilizing implementation science methods to inform the future design of crisis systems. This mixed methods study will examine the effectiveness and acceptability of the Street Crisis Response Team (SCRT), a new 911-dispatched multidisciplinary mobile crisis intervention piloted in San Francisco, California. Trends and Factors Affecting Knowledge of and Stigma and Violence Towards Female Sex Workers in Morocco.

BACKGROUND: We investigate factors affecting HIV knowledge, stigma, and violence among female sex workers (FSW) in Agadir, Rabat, Fes, and Tangier, Morocco, over three rounds of HIV surveillance surveys (2012, 2016, and 2019) conducted using respondent-driven sampling (RDS). Multivariable analyses from RDS studies examining the relationships between variables are under-utilized, particularly analyses that combine multiple locations and years of data together in a principled manner.
