Ruptured mycotic cerebral aneurysm development from pseudoocclusion due to septic embolism.

BACKGROUND: Cerebral mycotic aneurysms are rare sequelae of systemic infections that can cause profound morbidity and mortality with rupture. Direct bacterial extension and vessel integrity compromise from septic emboli have been implicated as mechanisms for formation of these lesions. We report the 5-day development of a ruptured mycotic aneurysm arising from a septic embolism that caused a focal M1 pseudoocclusion. CASE DESCRIPTION: A 14-year-old girl developed acute left-sided hemiparesis while hospitalized for subacute bacterial endocarditis that was found after she presented with a 2-wee

Experience with acellular human dura and bovine collagen matrix for duraplasty after posterior fossa decompression for Chiari malformations.

OBJECT: Posterior fossa decompression with duraplasty is routinely used for the treatment of Chiari malformations. It has been traditionally believed that this procedure requires a watertight seal with primary closure of the dura with either pericranium or allograft. In this study, the authors evaluated two synthetic dural substitutes in this patient population for feasibility of use and identification of perioperative morbidity. METHODS: The authors evaluated 56 patients who underwent duraplasty with a synthetic collagen matrix (DuraGen) after suboccipital craniectomy and C-1 laminectomy, an

The supracondylar approach to the jugular tubercle and hypoglossal canal.

BACKGROUND: Circumscribed lesions of the hypoglossal canal and of the jugular tubercle still remain a surgical challenge. So far, transpetrosal, transcondylar suboccipital, and extreme lateral approaches have been used to access this region. These surgical procedures bear a high risk for neurological deficits. Therefore, we introduce a new minimally invasive extradural approach to the hypoglossal canal that also allows access to the lateral aspects of the jugular tubercle. METHODS: After a paramedian retromastoid skin incision, a basal suboccipital craniectomy lateral to the foramen magnum to

Duraplasty with freeze-dried cadaveric dura versus occipital pericranium for Chiari type I malformation: comparative study.

During the period from October 1, 1989 to October 1, 1995 a total of 26 cases of Chiari type I malformation not associated with syringomyelia were attended in our Hospital. All patients underwent cranio-cervical decompression, with occipital craniectomy and removal of the posterior arch of C1. In 3/26 (11.5%) cases an additional C2 laminectomy had to be performed and in 1/26 (3.8%) case the C3 laminae were also removed. A first group of 13 patients underwent dural repair with freeze-dried cadaveric dura sutured with continuous 4-0 Vicryl running stitches, reinforced with fibrin sealant (Tissuc

[Surgery of vertebral aneurysms at the origin of PICA (author's transl)].

Thirteen cases of vertebral aneurysm at the origin of PICA (VA-PICA aneurysm) were operated on at the Department of Neurological Surgery of Kitano Hospital from March, 1970 through July, 1975. Those included 9 cases of saccular aneurysm and 5 cases of fusiform aneurysm (Table 1). The incidence of VA-PICA aneurysms among our whole series of intracranial aneurysms was 4.2%. Patients with subarachnoid hemorrhage were subjected to our routine 4 vessel angiography. For those with suspected vertebral aneurysm vertebral angiography was performed in a transoral projection. In this method, when the ang

Case Demonstrating the Nuances of Acute Cortical Venous Thrombosis Anticoagulation Guidelines.

BACKGROUND: The American Stroke Association and the European Stroke Organization have established guidelines on cerebral venous thrombosis (CVT); however, questions remain when an individual case does not fall within the inclusion criteria on which these guidelines are based. This is relevant when considering the use of anticoagulation in cases of CVT regarding whether or not associated hemorrhage is present and whether the hemorrhage is currently expanding. CASE DESCRIPTION: A 16-year-old right-handed female G2P2 (gravidity 2 [2 pregnancies] and parity 2 [2 live births after at least 24 week

Staged Surgical Resection of Brain Arteriovenous Malformations.

BACKGROUND: Staged treatment of brain arteriovenous malformations (AVMs) is sometimes necessary to minimize risks associated with sudden changes in cerebral hemodynamics. With the increasing availability and optimization of endovascular techniques, multiple surgical resections are rarely necessary, although, due to specific anatomic circumstances, some AVMs still require staged surgery. Here, we describe the largest reported series of staged surgical resections of brain AVMs. METHODS: This is a retrospective review of surgically resected AVMs at a single institution from 1998-2018. Patients w

Acute Posttraumatic Cerebral Venous Sinus Thrombosis-Induced Malignant Increased Intracranial Pressure Treated with Endovascular Dural Sinus Thrombectomy and Stenting.

BACKGROUND: Posttraumatic cerebral venous sinus thrombosis (CVST) is a major complication after head injury. Impaired venous outflow caused by CVST leads to increased intracranial pressure (IICP) refractory to medications and surgical decompression and often results in devastating consequences. Currently, there is no consensus on the treatment strategy. CASE DESCRIPTION: Here we report a case of posttraumatic CVST in a young male motorcyclist involved in a high-speed traffic accident. On admission, the patient immediately underwent decompressive hemicraniectomy. However, refractory malignant

Prophylactic plastic surgery closure of neurosurgical scalp incisions reduces the incidence of wound complications in previously-operated patients treated with bevacizumab (Avastin®) and radiation.

Neurosurgical craniotomy, craniectomy, or other trans-galeal interventions are performed for a variety of indications, including the resection of benign or malignant tumors, hematoma evacuation, and for the management of intractable seizure disorders. Despite an overall low complication rate of intervention, wound healing complications such as dehiscence, surgical site infection, and cerebrospinal fluid leak are not uncommon. A retrospective review was performed of all patients who underwent scalp incision closure at a single institution by a single plastic surgeon between 2006 and 2013. Sixty

Encapsulated acute subdural hematoma mimicking acute epidural hematoma on computed tomography.

An 87-year-old woman presented with an atypical case of acute subdural hematoma (ASDH) manifesting as disturbance of consciousness and left hemiparesis. Computed tomography revealed a high density lentiform lesion in the right convexity, which was thought to be acute epidural hematoma preoperatively. Emergent decompressive craniotomy revealed an encapsulated solid fresh clot in the subdural space and a bleeding small cortical artery under the clot. The arachnoid membrane and the brain parenchyma were intact, and no other abnormal feature such as aneurysm or arteriovenous malformation was obser

Observations on the Cerebral Effects of Refractory Intracranial Hypertension After Severe Traumatic Brain Injury.

BACKGROUND: Raised intracranial pressure (ICP) is a prominent cause of morbidity and mortality after severe traumatic brain injury (TBI). However, in the clinical setting, little is known about the cerebral physiological response to severe and prolonged increases in ICP. METHODS: Thirty-three severe TBI patients from a single center who developed severe refractory intracranial hypertension (ICP > 40 mm Hg for longer than 1 h) with ICP, arterial blood pressure, and brain tissue oxygenation (PBTO2) monitoring (subcohort, n = 9) were selected for retrospective review. Secondary parameters reflec

Assessment of perioperative outcome factors after hemicraniectomy confirms age as the prognostic factor in patients with middle cerebral artery infarction: Evidence from real-world data.

BACKGROUND: Malignant middle cerebral artery (MCA) infarction is a serious condition for which hemicraniectomy can improve survival and outcome. However, it remains unclear to what extent other perioperative factors contribute to the outcome. METHODS: This monocentric, retrospective observational study included patients with MCA infarction and subsequent hemicraniectomy. Perioperative parameters were collected from 68 patients between 01/2012 and 12/2020. Logistic regression analysis was performed to establish determinants for the outcome measured by the Modified Rankin Scale (mRS) (dichotomi

Malignant PRES and RCVS after brain surgery in the early postpartum period.

The management of women with brain tumors in the early post-partum period may be demanding as the patho-physiological changes that occur during pregnancy may also manifest in the early post-partum period. The aim of our paper is to report a case of late-onset post-partum pre-eclampsia after brain tumor surgery, complicated by posterior reversible encephalopathy syndrome (PRES) and reversible cerebral vasoconstriction syndrome (RCVS). Hemicraniectomy and intensive care management were necessary to obtain a favorable neurological outcome. The inherent literature on the subject is also analyzed t

Decompressive Hemicraniectomy for Malignant Middle Cerebral Artery Stroke: South Asian Experience.

BACKGROUND: The randomized trials showed improved outcome and reduced mortality in malignant middle cerebral artery (MMCA) undergoing Decompressive hemicraniectomy (DHC) within 48 hours of stroke onset. Despite high prevalence of stroke, especially in younger individuals, high and short-term mortality from stroke in South Asian and Middle East, there is little published data on DHC in patients with MMCA stroke. METHODS: This is a retrospective, multicenter cross-sectional study to measure outcome following DHC using the modified Rankin Scale (mRS) and dichotomized as favorable (mRS ≤ 4) or un

Decompressive hemicraniectomy for malignant hemispheric stroke in the elderly: comparison of outcomes between individuals 61-70 and >70 years of age.

BACKGROUND: Malignant hemispheric infarction is a life-threatening condition with a high mortality rate. Decompressive hemicraniectomy (DHC) is frequently a life-saving procedure that has shown the highest grade of evidence for patients 18 to 60 years of age. However, the efficacy of DHC in patients>60 years of age has rarely been investigated. METHODS: A retrospective study was conducted in a single academic institution. Surrogates of patients with clinical signs of impending brain herniation despite standard medical therapy were offered the option of DHC regardless of age or the side of the

Effects of posterior fossa decompression with and without duraplasty on Chiari malformation-associated hydromyelia.

OBJECTIVE: The optimal surgical treatment of Chiari malformation is unclear, especially in patients with hydromyelia. Various surgical approaches have included suboccipital craniectomy, syringostomy, obex plugging, syringosubarachnoid shunting, and fourth ventriculosubarachnoid shunting. The purpose of this study is to differentiate extradural and intradural approaches in the treatment of Chiari I malformation. METHODS: We reviewed the medical records and magnetic resonance imaging (MRI) scans of 34 surgical corrections' of Chiari malformation performed at our institution from 1988 to 1998. T

Telovelar Approach for Fourth-Ventricular Epidermoid Cyst: Anatomical Respect, Functional Recovery, and Long-Term Stability.

Background and Clinical Significance: Fourth-ventricular epidermoid cysts are rare intracranial lesions. They account for fewer than 1% of all primary brain tumors. Fourth-ventricular epidermoid cysts grow slowly because they are closely related to brainstem, cerebellum, and major blood vessels, so their treatment requires special caution. Because the cyst capsule attaches to functionally sensitive locations, complete removal is usually not possible without compromising some aspect of brain or spinal cord function. Surgical decision-making always involves weighing the need to remove the entire

[The possibilities and limitations of the neuronavigation method in the treatment of intracranial pathology, on the basis of anatomic registering markers and preoperative magnetic resonance imaging].

INTRODUCTION: The evaluation of technical parameters of the system and the evaluation of the influence of changing intraoperative conditions on the accuracy of the indications of the neuronavigational system has the key meaning in the effectiveness on the work ofa neurosurgeon. On the other hand the employment of neuronavigation during an operation on the brain allows to get an insight into the dynamics and directions of the shifts of individual brain structures in regard to themselves and hard integuments. MATERIAL AND METHODS: In the experimental part of the work an analysis was done of the

Bone Cement Cranioplasty Reduces Cerebrospinal Fluid Leak Rate after Microvascular Decompression: A Single-Institutional Experience.

Background  Microvascular decompression (MVD) is a common surgical treatment for cranial nerve compression, though cerebrospinal fluid (CSF) leak is a known complication of this procedure. Bone cement cranioplasty may reduce rates of CSF leak. Objective  To compare rates of CSF leak before and after implementation of bone cement cranioplasty for the reconstruction of cranial defects after MVD. Methods  Retrospective chart review was performed of patients who underwent MVD through retrosigmoid craniectomy for cranial nerve compression at a single institution from 1998 to 2017. Risk factors for wound-related complications after microvascular decompression.

Retrosigmoid craniotomy for microvascular decompression (MVD) has been traditionally performed via craniectomy. Various closure techniques have been described, yet factors associated with wound-related complications remain undetermined. Accordingly, herein, we sought to identify risk factors associated with wound-related complications after such procedures. An institutional retrospective case-control study was performed; outcomes of interest were cerebrospinal fluid (CSF) leak, wound dehiscence, wound infection, and pseudomeningocele. Univariate analysis was performed using Wilcoxon rank sum t
