Patients with patellar instability have one or more anatomical predispositions that are important in pathogenesis of this disease. We present treatment of a patient with valgus deformity of the lower limb and low patellar position with chronic instability. The patient underwent medial closing-wedge distal femoral osteotomy and transposition of tibial tuberosity.

This retrospective case series reports on French bulldogs diagnosed with patellar maltracking without patellar luxation. We describe clinical presentation, joint exploration, and surgical treatment with patellar groove replacement to alleviate maltracking in the first attempt. Three French bulldogs were included. Clinical presentation was similar in all cases, with intermittent skipping lameness of 1 hind limb, absence of patellar luxation, and patellar maltracking. Joint exploration revealed abnormal conflict between the patella and medial ridge of the trochlea, with secondary severe erosion

BACKGROUND: The tibial tubercle to trochlear groove (TT-TG) distance is often utilized for determining the surgical treatment for patients with patellar instability (PI). It is thought to directly represent the position of the TT on the tibia. Recent work has shown that the measurement of the TT-TG distance is multifactorial.
PURPOSE: To investigate the relationship between relative tibial external rotation (rTER) and trochlear dysplasia (TD), as well as the location of the TG and TT in patients with and without PI, and to correlate these and other anatomic measurements with the TT-TG distanc

PURPOSE: The purpose of this study was to determine the utilization and responsiveness of common patient-reported outcomes (PROs) in patients undergoing surgery for patellar instability.
METHODS: Using PRISMA guidelines, a systematic review of studies reporting outcomes following surgical intervention for patellar instability was conducted using Pubmed, Cochrane, OVID Medline, and Google Scholar. Subgroup analysis of articles reporting at least two PROs with baseline and follow-up data were used to evaluate responsiveness of instruments using relative efficiency and effect size.
RESULTS: Fro

First-time patellar luxation appears typically in teenagers and young adults below the age of 16 years, with a prevalence of 45/100,000/year. This luxation is treated with brief limited mobility in a bandage, and with a complementary physiotherapy if the mobility is influenced afterwards. Risk factors for patellar instability are patellofemoral dysplasia, hyperlaxity, patella alta and valgus malalignment in the knee joint. In case of repeated luxation the treatment is surgical, i.e. reconstruction of the medial patellofemoral ligament recreating the medial patella-stabilizing structures. If th

PURPOSE: The quadriceps angle (Q-angle) represents the angle between the vector of action of the quadriceps and the patellar tendon. An increased Q-angle has been associated with an increased risk of patellar instability, although there is disagreement on its reliability and validity as it is affected by the position of the limb and contraction of the quadriceps. Tibial tuberosity-trochlear groove distance (TT-TG) is ascertained by axial CT scanning, with an increased value associated with patellar instability. This study aimed to determine whether the Q-angle correlates with the TT-TG distanc

BACKGROUND: Habitual dislocation of the patella (HDP) is a rare condition. Many surgical procedures using proximal realignment have been reported to treat HDP in children, with around 80% satisfactory results. However, few articles have addressed the treatment of HDP associated with high-grade patellofemoral chondromalacia in adults.
HYPOTHESIS: A combination of proximal and distal realignment procedures of the patella will reduce pain and increase function in adult patients with HDP.
STUDY DESIGN: Case series; Level of evidence, 4.
METHODS: Twelve patients with 13 symptomatic cases of HDP

Patellar misalignment is more common than usually supposed; many patients learn to live with this affection without seeing a doctor. A large number of open surgical procedures were described for treatment of misalignment of extensor mechanism of knee. We want to present our method which consisted in arthroscopic lateral release and augmentation of medial retinaculum. We had 47 patients with femoro-patellar misalignment treated with this method. The results were appreciated by clinical and radiological criteria (patellar glide test, patellar tilt test, Macnab scores, Merchant angle). The follow

We present a case of bilateral patellar component dissociation 6 years after bilateral total knee arthroplasty. The patient had undergone arthroscopic lateral releases bilaterally for patellar maltracking. After repetitive trauma, the patient experienced patellar component dissociation, which was treated arthroscopically by removing the patellar components and leaving the patellae unresurfaced. The patient's symptoms improved substantially. Our case highlights certain features of the etiology as well as the management of patellar component dissociation in the total knee arthroplasty, showing t

PURPOSE: Medial patellofemoral ligament (MPFL) reconstruction and tibial tubercle osteotomy are commonly used to treat recurrent lateral patellar instability, yet there are limited available data on return to sport (RTS) following these procedures. The purpose of this study is to evaluate patient factors associated with excellent functional outcomes, including successful RTS, following surgical stabilization including MPFL reconstruction in competitive athletes with recurrent lateral patellar instability.
METHODS: Athletes undergoing primary MPFL reconstruction for recurrent lateral instabili

BACKGROUND: Patellar complications following endoprosthetic reconstruction can occur as a result of anatomic, physiologic, and surgical reasons. Patellar impingement on tibial polyethylene is a complication of distal femoral replacement, and it is frequently related to inaccurate restoration of the joint line and to soft-tissue contracture. The purpose of our study was to determine the prevalence and type of patellar complications following distal femoral replacements after excisions of bone tumors.
METHODS: The results of reconstruction with use of a rotating-hinge endoprosthesis following e

Introduction: Poor patellar tracking can result in painful contact pressures, patella subluxation, or dislocation. The use of musculoskeletal models and simulations in orthopedic surgeries allows for objective predictions of post-treatment function, empowering clinicians to explore diverse treatment options for patients. Although a promising approach for managing knee surgeries, the high computational cost of the Finite Element Method hampers its clinical usability. In anticipation of minimal elastic deformations in the involved bodies, the exploration of the Multibody Dynamics approach emerge

PURPOSE: The Q-angle has been used for years to quantify lateralization of the patella. The tibial tuberosity-trochlea groove distance (TT-TG distance) was introduced to analyse patellar tracking. Does a significant correlation exist between these two parameters? Do other significant interrelations exist between the Q-angle/TT-TG distance, torsion of the femur and tibia, the frontal axis, overall leg length, gender, former patellar dislocation, BMI?
METHODS: One hundred knees in 55 patients with patellofemoral symptoms were included in a prospective study. All patients underwent clinical exam

BACKGROUND: Medial patellofemoral ligament reconstruction (MPFLR) and tibial tubercular osteotomy (TTO) are common surgical treatments for patellar dislocation. However, the impact of isolated MPFLR and MPFLR combined with TTO (MPFLR+TTO) on vastus medialis oblique (VMO) onset activation remains unclear. This study investigated the VMO onset activation relative to the vastus lateralis (VL) among patients undergoing isolated MPFLR, MPFLR+TTO, and healthy controls during maximum voluntary isometric knee extension.
METHODS: This study included 25 patients who underwent MPFLR, divided into two gr

OBJECTIVE: A rotational osteotomy requires a complete cut of the bone in order to correct maltorsion. An additional correction of the frontal axis can be achieved via an oblique cut of the bone. The osteotomy with bone to bone contact is fixed with an angle stable plate.
INDICATIONS: Symptoms such as anterior knee pain, inwardly pointing knee syndrome, lateral patellar subluxation or dislocation, lateral patellar hypercompression syndrome are a common indication for derivational osteotomy if clinically increased femoral internal rotation and radiologically increased femoral antetorsion is det

PURPOSE: Many studies have reported satisfactory clinical outcomes and low redislocation rates after reconstruction of the medial patellofemoral ligament (MPFL) for the treatment of lateral patellar instability. Despite uncorrected severe trochlear dysplasia (Dejour type B to D) being acknowledged as a major reason for less favourable clinical outcomes and a higher incidence of patellar redislocations after an isolated MPFL reconstruction, the evidence for a deepening trochleoplasty procedure remains scarce in the current literature. BACKGROUND: A laterally tracking patella is commonly seen in patients with chronic recurrent lateral patellar dislocations. Clinical appearance of the J-sign occurs when the patella is congruent with the trochlear groove in flexion and moves over the lateral border of the femoral condyle as the lower leg reaches complete extension. A Fulkerson osteotomy procedure corrects this maltracking of the patella by medially transferring the tibial tubercle. There are many radiographic patellofemoral indices that can be used describe this incongruence about the patelloformal joint. The current literatur

Fractures of the patella account for about 1% of all skeletal injuries and can lead to profound impairment due to its crucial function in the extensor mechanism of the knee. Diagnosis is based on the injury mechanism, physical examination and radiological findings. While the clinical diagnosis is often distinct, there are numerous treatment options available. The type of treatment as well as the optimum timing of surgical intervention depends on the underlying fracture type, the associated soft tissue damage, patient factors (i.e. age, bone quality, activity level and compliance) and the stabi
