OBJECTIVE: There is a void in the literature describing reliable surgical landmarks that aid in the dissection of the facial recess in the absence of skeletonizing the mastoid segment of the facial nerve. The posterior ligament of the incus is a readily distinguishable "white dot" along the incus buttress that has been used to guide dissection in a safe and efficient manner. The goal of our study is to describe a surgical approach that utilizes this surgical landmark to drill the facial recess and to take anatomical measurements demonstrating the safety and reliability of this approach.

HYPOTHESIS: Round window velocity measurements should correlate closely with vibration measurements taken at proximal points along an intact chain over a set frequency range. These round window vibration measurements should be similar to the vibration measurements taken of the ossicles if mechanical energy is conserved through the vestibular organ.
BACKGROUND: To date there has not been a study which compares vibratory velocity measurements through an intact ossicular chain to the level of the round window. This study attempted to quantify the degree of mechanical energy transmission and susp

INTRODUCTION: Cochlear implants are, for many reasons, expirable electronic devices and occasionally may have to be replaced. The surgical cochlear reimplantation technique is not substantially different from the initial implantation but does have some peculiarities. These peculiarities are analyzed in this paper not only from the point of view of surgical technique but also with respect to the complications involved and the functional auditive outcome obtained.
OBJECTIVES: An accurate description of the surgical findings, peculiarities of the surgical technique and complications found during

INTRODUCTION: Candidiasis is usually a superficial infection of the moist areas of the body and is generally caused by Candida albicans. Visceral infections occur in diabetes, lymphomas and leukemias, malnutrition, avitaminosis and they are associated with antibiotic, corticosteroid and immunosuppressive therapy. Candida albicans was isolated from middle ear inflammation. The diagnosis is made on the basis of microscopic appearance of colonies and characteristic smell. Candidiasis is successfully treated with nystatin, imidazol derivatives (fluconazole, ketoconazole and intraconazole), amphote

OBJECTIVE: To detect the presence of bacteria in mastoid granulations and compare its prevalence in both types of chronic suppurative otitis media (CSOM). To find out if stage of disease activity, age, duration of disease, and aditus patency relate to obtaining positive cultures.
STUDY DESIGN AND SETTING: A prospective, parallel group study done at a tertiary care referral centre. Mastoid granulations from 79 patients with CSOM undergoing mastoidectomy were processed for anaerobic and aerobic bacteria.
RESULTS: Aerobes were isolated from 57.55 per cent of the tubotympanic and 74.4 per cent o

UNLABELLED: Acute mastoiditis (AM) is an uncommon but serious complication of acute otitis media (AOM). In the pre-antibiotic era, AM was seen more frequently than it is today, but it was rare in infants. However, in the last two decades an increase in the incidence of AM in infancy has been reported in the literature. During the years 1990-2002, we treated 113 patients with 128 episodes of AM; of them, 24 were infants (median age 6 months; 18 males) who suffered from 26 episodes of AM. Twenty developed AM as a complication of their first episode of AOM. One of the four infants with a prior hi

Canal wall-down (CWD) mastoidectomy creates a radical cavity that modifies the anatomy and physiology of the middle ear, thus preventing it from being self-cleaning and causing epidermal stagnation in the posterior cavities. Canal wall-down tympanomastoidectomy with reconstruction (CWDTwR) can obliterate such radical cavities. The main objective of this study was to compare postoperative results after CWDTwR by using either bone allografts or 45S5 bioactive glass as a filling tissue with an 18-month follow-up. This was a single-center observational trial including all patients undergoing CWDTw

OBJECTIVE: Severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2), a novel coronavirus, causes coronavirus disease 2019 (COVID-19). Otologic surgeries with drilling by powered instruments induce significant aerosols, which may induce SARS-CoV-2 transmission to medical staff if SARS-CoV-2 exists in the middle ear and mastoid cavity. During a COVID-19 pandemic, therefore, confirming a negative COVID-19 test prior to otologic surgery is recommended. However, previous coronavirus studies demonstrated that coronavirus was detected in the middle ear in some patients even though the polymerase

INTRODUCTION: Injuries to the peripheral auditory system are among the most common results of high intensity impulsive acoustic exposure. Prior studies of high intensity sound transmission by the ossicular chain have relied upon measurements in animal models, measurements at more moderate sound levels (i.e. < 130 dB SPL), and/or measured responses to steady-state noise. Here, we directly measure intracochlear pressure in human cadaveric temporal bones, with fiber optic pressure sensors placed in scala vestibuli (SV) and tympani (ST), during exposure to shock waves with peak positive pressures

INTRODUCTION: Paragangliomas are usually benign tumours stemming from the neural crest which may have neuroendocrine activity. Because of hypervascularisation they usually lead to pulsatile tinnitus. Local expansion initially causes middle ear and later inner ear hearing loss, and cranial nerve dysfunction.
PATIENTS/METHODS: Signs and symptoms, demographic data, tumour stages, treatments and outcomes were analysed for the 21 patients treated at our clinic between 1992 and 1999. Among 16 surgically treated patients 3 type A and 4 type B tumours were treated by mastoidectomy and middle ear reco

Background Middle ear surgeries such as tympanoplasty and mastoidectomy demand a bloodless operative field because even minimal bleeding under high-magnification microscopy can significantly impair visualization, prolong operative time, and increase the risk of iatrogenic injury. Controlled hypotension is widely accepted for minimizing bleeding; however, the optimal pharmacological agent in otological surgery remains debated. Esmolol, a cardio-selective beta-1 blocker, reduces heart rate (HR) and cardiac output without vasodilation, whereas nitroglycerin (NTG) frequently induces reflex tachyca

BACKGROUND: In chronic suppurative ear disease, conductive hearing loss with ossicular chain erosion, poses a serious challenge to the otologist. To overcome this hurdle, various ossiculoplasty techniques have been developed to date. Little modifications in the ossicular reconstruction technique, combined with cavity obliteration in selected cases, can yield predictably good results.
OBJECTIVE: A) To study the hearing outcome in 50 patients of Austin group A ossicular defect, using a piece of notched autologous conchal cartilage over the stapes suprastructure, combined with incus interpositio

INTRODUCTION AND IMPORTANCE: Gradenigo syndrome is a rare but potentially serious complication of suppurative otitis media, characterized by the classic triad of suppurative otorrhea, abducens nerve (VI) paralysis, and pain in the trigeminal nerve (V) territory. Although the advent of antibiotic therapy has significantly reduced its incidence, this condition remains relevant, particularly in children, requiring early recognition and appropriate multidisciplinary management.
CASE PRESENTATION: We report the case of a 13-year-old girl who presented with a painful swelling behind her right ear t

INTRODUCTION: Children with Down syndrome (DS) have a high incidence of chronic middle ear disease. Surgery to manage this disease is challenging due to the severity of illness and narrow ear canal dimensions. Endoscopic ear surgery is used to manage tympanic membrane and middle ear disease with the advantages of improved visualization and avoidance of post-auricular incisions. However, its application in children with DS has not been reported. We aim to compare the outcomes of endoscopic versus microscopic ear surgery in children with DS.
METHODS: All patients with DS who underwent tympanopl
