Combined Distal Femoral Osteotomy and Medial Patellofemoral Ligament Reconstruction for Patellar Instability and Genu Valgus: A Case Report and Literature Review.

BACKGROUND: Medial patellofemoral ligament (MPFL) reconstruction alone is not effective for patellar instability associated with anatomic abnormalities of lower limbs. In this article, we report a case of complex lower limb malformations, including genu valgus, lower limb shortening, and increased femoral anteversion angle. In addition to MPFL reconstruction, we performed a rare osteotomy named combined distal femoral osteotomy (CDFO), which combined the characteristics of lateral opening wedge distal femoral osteotomy (LOWDFO) and derotational distal femoral osteotomy (DDFO). CASE PRESENTATI

Correlation Analysis between Tibial Tuberosity-Trochlear Groove Distance and Other Patellar Stability Parameters in Young and Middle-aged Populations.

OBJECTIVE: The exact mechanism of patellofemoral instability has not been clearly clarified. The current study aims to explore the correlation between the tibial tuberosity-trochlear groove (TT-TG) distance and other patellar stability parameters. METHODS: A total of 60 individuals aged 18 to 40 years who underwent knee computed tomography (CT) examination between September 2014 and December 2017 were retrospectively recruited. Five reference sites were selected on the femoral trochlear articular surface in every CT image. The TT-TG distance and the trochlear groove angles (TGA) at the five r

[Diagnosis and therapy of chronic patellofemoral instability].

The chronic patellofemoral instability is a multifactorial disease, which is mostly congenital. Luxation of the patella is mainly atraumatic. Sole conservative treatment is often unsuccessful. Surgical treatment is needed in most cases and requires consideration of all pathologic changes of the patella-stabilizing anatomic structures. Rupture of the medial patellofemoral ligament is almost pathognomonic. In addition, the frontal mechanical axis, rotation of femur and tibia, the trochlear shape, the distance from the tibial tuberosity to the trochlear sulcus and the patellar height play an impo

Systematic Review of Medial Patellofemoral Ligament Reconstruction Techniques: Comparison of Patellar Bone Socket and Cortical Surface Fixation Techniques.

PURPOSE: To compare patellar bone socket and cortical surface fixation techniques for isolated medial patellofemoral ligament (MPFL) reconstruction and determine whether there was a difference in (1) complication rates, including fracture of the patella; (2) redislocation rates; or (3) patient-reported outcomes. METHODS: A literature search was conducted following Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. We included patients who underwent isolated MPFL reconstruction for recurrent patellar instability. Patients with confirmed concomitant or prior

Guidelines for Treatment of Lateral Patella Dislocations in Skeletally Mature Patients.

The incidence of lateral patella dislocations is high, particularly in young females. Beside traumatic cases, many patients present with specific anatomical factors that predispose to lateral patella dislocations (torsional abnormalities of the femur or the tibia, trochlea dysplasia, patella alta, etc). It is of utmost importance to correct those pathologic factors during concomitant procedures as isolated reconstructions of the medial patellofemoral ligament would fail in the presence of severe anatomic risk factors. This article provides a comprehensive instruction on how to analyze the risk

Are the fabellae bisected by the femoral cortices in a true craniocaudal pelvic limb radiograph?

OBJECTIVES: To assess whether the bisection of the fabellae by their respective femoral cortices is a useful criterion for assessing positioning of craniocaudal pelvic limb radiographs, and whether this is consistent in limbs affected or unaffected by medial patella luxation. METHODS: Computed tomography multi-planar reconstructions were used to determine the relative positions of the fabellae with respect to the femoral cortices. RESULTS: Only 36% of fabellae overall were bisected by the femoral cortex. There was no significant difference between limbs affected or unaffected by medial patel

Results of medial patellofemoral ligament reconstruction compared with trochleoplasty plus individual extensor apparatus balancing in patellar instability caused by severe trochlear dysplasia: a systematic review and meta-analysis.

PURPOSE: Many studies have reported satisfactory clinical outcomes and low redislocation rates after reconstruction of the medial patellofemoral ligament (MPFL) for the treatment of lateral patellar instability. Despite uncorrected severe trochlear dysplasia (Dejour type B to D) being acknowledged as a major reason for less favourable clinical outcomes and a higher incidence of patellar redislocations after an isolated MPFL reconstruction, the evidence for a deepening trochleoplasty procedure remains scarce in the current literature. The hypothesis of this systematic review and meta-analysis w

Prospective assessment of functional and clinical results of surgical patellar stabilization in rural and urban populations. Equal chance to success?

INTRODUCTION AND OBJECTIVE: Patella dislocation represents 3.3% of all knee injuries often leading to persistent instability. Medial patello-femoral ligament(MPFL) reconstruction is the standard method of treatment in the patellar instability. Rehabilitation after MPFL-R is a long and demanding procedure. The hypothesis presented reflects the idea that despite relatively good access to hospital care and surgical options, the post-operative rehabilitation care system is still inferior in rural areas versus the one offered in major cities and towns. MATERIAL AND METHODS: Between January 2015 -

New standardization method of tibial tubercle-posterior cruciate ligament distance according to patient size in patients with patellofemoral instability.

BACKGROUND: Large differences in tibial tubercle-posterior cruciate ligament (TT-PCL) distance were described in several reports between countries, suggesting that abnormal TT-PCL distance is variable due to individual patient size. This study aimed to clarify the relationship between TT-PCL distance and patient size, and to determine a method for describing individualized TT lateralization. METHODS: We analyzed 41 patients with recurrent patellar dislocation (RPD) and 41 age-matched patients without patellar instability who underwent primary anterior cruciate ligament reconstruction (control

Computed tomography of the normal patellofemoral joint.

Radiographic assessment of the patellofemoral joint (PFJ) is problematic because conventional views are cumbersome, difficult to standardize, and not reliable for displaying this joint with flexion of the knee less than about 30 degrees. We used computed tomography (CT) to obtain conveniently a direct transaxial view of the PFJ in ten normal human volunteers at different 0 degree, 20 degrees, and 45 degrees of knee flexion and during both contraction and relaxation of the quadriceps muscle. From the CT images we measured femoral trochlear angle, patellar centralization, femoral trochlear depth

Evaluation of the Roux-Elmslie-Trillat procedure for knee extensor realignment.

One method for correction of patellofemoral malalignment in acute and recurrent dislocations and subluxations of the patella involves a lateral retinacular release, medial capsular reefing, and medial displacement of the infrapatellar tendon insertion. The procedures was originally described by Roux and later modified and popularized by Elmslie and Trillat. It allows adequate evaluation and easy adjustment of the extensor mechanism at the time of surgery. This current report is an evaluation of 116 of these procedures followed for at least one year and examined by an orthopaedic surgeon. This

Anatomical Variation in Quadriceps Angle With Regard to Different Anthropometric Parameters in a Tertiary Care Center in Northern India: A Descriptive Study.

BACKGROUND: The quadriceps angle, commonly known as the Q-angle, is the angle formed between the vectors of the pull of the quadriceps muscle and the patellar tendon. The literature varies in terms of the values of Q angles measured by various researchers. It is well appreciated that the normal Q-angle should fall between 12° and 20°, with males being at the lower end of this range and females having higher measurements. An increase in Q-angle beyond the normal range has been associated with knee extensor dysfunction leading to patellar instability. Keeping in mind the clinical and biomechanic

Patient-Reported Favorable Outcomes of a Minimum Five-Year Follow-Up After Medial Patellofemoral Ligament (MPFL) Reconstruction: A Systematic Review and Meta-Analysis.

Medial patellofemoral ligament (MPFL) reconstruction is a procedure performed to address patellofemoral instability. The primary objective of this paper is to evaluate studies reporting the outcomes of MPFL reconstruction at a minimum five-year follow-up through analysis of patient-reported outcomes (PROs), re-dislocations, and range of motion. We hypothesize that MPFL reconstruction will demonstrate excellent PROs, low redislocation rates, and full range of motion. A search following guidelines established by the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) was
