Complement-mediated thrombotic microangiopathy treated with anticomplement protein 5 therapy, a retrospective study.

BACKGROUND: Complement-mediated thrombotic microangiopathy (CM-TMA), also called atypical hemolytic uremic syndrome (aHUS), is a difficult-to-diagnose rare disease that carries severe morbidity and mortality. Anti-C5 monoclonal antibodies (aC5-mab) are standard treatments, but large studies and long-term data are scarce. Here, we report our single institution experience to augment the knowledge of CM-TMA treated with aC5-mab therapy. METHODS: We aimed to assess the short and long-term effects of aC5-mab in patients diagnosed with CM-TMA treated outside of a clinical trial. This was a retrospe

Clinical outcomes and quality of life associated with eculizumab in patients with late-onset myasthenia gravis.

INTRODUCTION: Late-onset myasthenia gravis (LOMG) features prominent immunosenescence, high myasthenic crisis risk, and refractoriness due to comorbidities limiting conventional immunosuppressants, leading to poor prognosis. Eculizumab, which blocks complement C5 activation, may provide rapid symptom improvement and has shown an acceptable safety profile in previous studies, holding potential for LOMG management. Although the REGAIN trial and some real-world studies have demonstrated the efficacy of eculizumab, evidence for its use in patients with refractory LOMG is scarce. This study aimed t

Brachial Plexus Ultrasound and MRI in Children with Brachial Plexus Birth Injury.

BACKGROUND AND PURPOSE: Brachial plexus birth injury is caused by traction on the neck during delivery and results in flaccid palsy of an upper extremity commonly involving C5-C6 nerve roots. MR imaging and MR myelography help to assess the anatomic location, extent, and severity of brachial plexus injuries which influence the long-term prognosis along with the surgical decision making. Recently, sonography has been increasingly used as the imaging modality of choice for brachial plexus injuries. The aim of this study was to assess the degree of correlation among brachial plexus sonography, MR

A new in vitro model for the study of pig-to-human vascular hyperacute rejection.

Studies on vascular hyperacute xenograft rejection (HAR) are usually conducted in vitro on cultured endothelial cells (EC) exposed to human serum, in complex whole organ perfusion models using heparinized blood or in vivo models. Here we describe a new model allowing perfusion of pig vessels with human whole blood without anticoagulants. Segments of the porcine iliac artery were connected to circular polyvinyl chloride (PVC) tubing, whose inner surface was conjugated with immobilized heparin. The vessels were perfused with 7 to 8 ml of fresh, non-anticoagulated human blood by rocking of the tu

The adenovirus oncoprotein E1B-55K reshapes epigenetic histone modifications in primary human cells.

Oncogenic viruses can induce epigenetic changes in host cells, which may contribute to cell transformation and tumorigenesis by altering gene expression patterns. Human adenoviruses, while primarily known for their lytic infections, have also been implicated in oncogenic transformation in experimental settings, but the mechanisms underlying adenovirus oncoprotein-induced epigenetic dysregulation remain poorly defined. Building on our recent findings that the adenoviral oncoprotein E1B-55K coordinates cellular transformation through interactions with DNA-bound host transcription factors, we nex

GAG-Former: An LC-MS/MS data-processing workflow with a physics-informed dual-stream transformer backbone for co-elution deconvolution and simultaneous structural identification of glycosaminoglycan disaccharides in biological samples.

Glycosaminoglycan (GAG) disaccharide profiling by LC-MS/MS underpins heparin quality control and glycomics biomarker research. Existing workflows are constrained by co-elution of sulfation-positional isomers and C5 epimers, disconnection between structural identification and peak-area quantification, and matrix-driven transfer loss between standards and biological samples. This study develops GAG-Former, an LC-MS/MS data-processing workflow built on a physics-informed dual-stream Transformer backbone that encodes fragment spectra and chromatographic retention times in parallel. The sulfation b

Age-Associated Plasma Protein Signatures and Proteomic Age Estimation by Data-Independent Acquisition Mass Spectrometry in a Russian Cross-Sectional Cohort.

Chronological age can be estimated from molecular profiles; however, predictive accuracy alone does not establish a biologically validated ageing clock. We therefore evaluated whether plasma protein intensities measured by data-independent acquisition mass spectrometry (DIA-MS) support chronological-age estimation in a cross-sectional cohort from Moscow, Russia, while explicitly examining acquisition-order and MS-batch confounding. Plasma samples from 350 adult participants aged 19.8 to 99.1 years were analyzed as single-shot, single-injection DIA-MS acquisitions on a Q Exactive HFX platform.

Dissecting causal relationships between immune cells, blood metabolites, and aortic dissection: A mediation Mendelian randomization study.

BACKGROUND: There exists a robust correlation between the infiltration of immune cells and the pathogenesis of aortic dissection (AD). Moreover, blood metabolites serve as immunomodulatory agents within the organism, influencing the immune system's response and potentially playing a role in the development of AD. Nevertheless, the intricate genetic causal nexus between specific immune cells, blood metabolites, and AD remains partially elucidated. OBJECTIVES: This study aims to elucidate the causal relationships between specific immune cell types and the risk of developing AD, mediated by bloo

Mitochondrial damage and clearance in retinal pigment epithelial cells.

Age-related macular degeneration (AMD) is a devastating eye disease that causes permanent vision loss in the central part of the retina, known as the macula. Patients with such severe visual loss face a reduced quality of life and are at a 1.5 times greater risk of death compared to the general population. Currently, there is no cure for or effective treatment for dry AMD. There are several mechanisms thought to underlie the disease, for example, ageing-associated chronic oxidative stress, mitochondrial damage, harmful protein aggregation and inflammation. As a way of gaining a better understa

Hemolytic uremic syndrome: differential diagnosis with the onset of inflammatory bowel diseases.

BACKGROUND: Shiga-toxin Escherichia coli productor (STEC) provokes frequently an important intestinal damage that may be considered in differential diagnosis with the onset of Inflammatory Bowel Disease (IBD). The aim of this work is to review in the current literature about Hemolytic Uremic Syndrome (HUS) and IBD symptoms at the onset, comparing the clinical presentation and symptoms, as the timing of diagnosis and of the correct treatment of both these conditions is a fundamental prognostic factor. A focus is made about the association between typical or atypical HUS and IBD and a possible r

Updates on C3 Glomerulopathy in Kidney Transplantation: Pathogenesis and Treatment Options.

C3 glomerulopathy is a rare disease, characterized by an abnormal activation of the complement's alternative pathway that leads to the accumulation of the C3 component in the kidney. The disease recurs in more than half of kidney transplant recipients, with a significant impact on graft survival. Recurrence of the primary disease represents the second cause of graft loss after organ rejection. In C3 glomerulopathy, there are several risk factors which can promote a recurrence during transplantation, such as delayed graft function, infection and monoclonal gammopathy. All these events can trigg

Therapeutic Antibodies in Hematology: Advances in Malignant and Non-Malignant Disorders.

Therapeutic antibodies have revolutionized hematology, offering targeted and effective treatments for both malignant and non-malignant diseases. In hematologic malignancies, anti-CD20, anti-CD19, anti-CD38, and anti-B-cell maturation antigen (BCMA) antibodies have markedly improved survival outcomes, whereas antibody-drug conjugates and bispecific antibodies continue to expand therapeutic possibilities. Besides cancer, complement inhibitors such as eculizumab, ravulizumab, and the recently approved crovalimab have redefined paroxysmal nocturnal hemoglobinuria and atypical hemolytic uremic synd
