Comparison of tonic spinal cord stimulation, high-frequency and burst stimulation in patients with complex regional pain syndrome: a double-blind, randomised placebo controlled trial.

BACKGROUND: Complex Regional Pain Syndrome (CRPS) is a disabling disease that is sometimes difficult to treat. Although spinal cord stimulation (SCS) can reduce pain in most patients with CRPS, some do not achieve the desired reduction in pain. Moreover, the pain reduction can diminish over time even after an initially successful period of SCS. Pain reduction can be regained by increasing the SCS frequency, but this has not been investigated in a prospective trial. This study compares pain reduction using five SCS frequencies (standard 40 Hz, 500 Hz, 1200 Hz, burst and placebo stimulation) in

Clinically Important Reductions in Physical Function and Quality of Life in Adults with Tumor Prostheses in the Hip and Knee: A Cross-sectional Study.

BACKGROUND: Patients with a bone sarcoma who undergo limb-sparing surgery and reconstruction with a tumor prosthesis in the lower extremity have been shown to have reduced self-reported physical function and quality of life (QoL). To provide patients facing these operations with better expectations of future physical function and to better evaluate and improve upon postoperative interventions, data from objectively measured physical function have been suggested. QUESTIONS/PURPOSES: We sought to explore different aspects of physical function, using the International Classification of Functioni

[Stabilometric analysis in patients affected by maculopathy].

OBJECTIVES: The walk is strictly influenced by vision, that's essential for the position of the body in the space during the walk cycle and the postural control. The quality of life related to the vision is widely highlighted in literature: for example Brown et al. in 2005 underlined the comparison from DMLE and chronic ills, like HIV, angina, dialysis, advanced prostate tumor, stroke, and heart attack. Peripheral low vision appeared the most crippling aspect to the walk, but in recent years a lot of studies stressed the walk difficulty in patients with central low vision. MATERIALS AND METHO

Displaced midshaft fractures of the clavicle: non-operative treatment versus plate fixation (Sleutel-TRIAL). A multicentre randomised controlled trial.

BACKGROUND: The traditional view that the vast majority of midshaft clavicular fractures heal with good functional outcomes following non-operative treatment may be no longer valid for all midshaft clavicular fractures. Recent studies have presented a relatively high incidence of non-union and identified speciic limitations of the shoulder function in subgroups of patients with these injuries. AIM: A prospective, multicentre randomised controlled trial (RCT) will be conducted in 21 hospitals in the Netherlands, comparing fracture consolidation and shoulder function after either non-operative

The effects of surface anaesthesia on the autonomic dysreflexia response during functional electrical stimulation.

Recently, increases in blood pressure (BP) and concomitant bradycardia, suggestive of autonomic dysreflexia (AD), have been documented during functional electrical stimulation (FES) in individuals with a high spinal cord injury (SCI). If uncontrolled, this response could preclude the safe use of FES among such individuals. FES induced pain is partly related to stimulation of skin nociceptors. Therefore, measures to reduce skin sensitivity may reduce the risk of AD during FES. The purpose of this study was to determine if topical anaesthetic applied over the site of electrical stimulation could

Relationship Between Force-Velocity Characteristics and Sprint Performance in Elite Sprinters: A Pilot Study.

BACKGROUND: Sprinting is a type of running that consists of producing a short and intense effort in order to perform maximal speed in a short period of time. Sprinting is widely investigated because of the multiple and complex mechanisms involved. Sprint studies are now focusing on how to improve performance by focusing their analyses on the mechanic variables. The hamstrings are part of the most important muscle groups during sprinting because of their role of stabilization and propulsion, but they must be able to produce the maximum strength during a short moment, this ability is characteriz

The Negative Impacts of Sarcopenia on Primary Total Knee Arthroplasty under the Enhanced Recovery after Surgery Protocol.

OBJECTIVE: Sarcopenia, as an emerging public health concern, has been associated with postoperative adverse outcomes in various surgical procedures. However, the evidence regarding the impacts of sarcopenia on total knee arthroplasty (TKA) remained limited. This study aimed to assess the impacts of sarcopenia on primary TKA based on the enhanced recovery after surgery (ERAS) protocol. METHODS: This retrospective study included 291 patients who received unilateral TKA from October 2017 to May 2018 in our institution. Sarcopenia was diagnosed based on the algorithm of Asian Working Group for Sa

Effect of high-flow nasal therapy during exercise training in COPD patients with chronic respiratory failure: study protocol for a randomised controlled trial.

BACKGROUND: The benefit of pulmonary rehabilitation (PR) in symptomatic chronic obstructive pulmonary disease (COPD) is well known. However, advanced patients with chronic respiratory failure (CRF), a category excluded from most studies, are frequently unable to sustain a work-load sufficiently high to obtain the full benefit of PR on exercise tolerance. Recent studies involving heated and humidified high flow oxygen therapy (HFOT) showed positive effects on breathing pattern and ventilatory efficiency during effort. We thus plan to compare, in COPD patients with CRF undergoing a high-intensit

Operative treatment of chronic irreparable Achilles tendon ruptures with large flexor hallucis longus tendon transfers.

BACKGROUND: Transfer of the flexor hallucis longus (FHL) tendon aims to restore function and relieve pain in chronic Achilles tendon (AT) disease. The goal of the present study was to investigate the clinical and radiographic outcomes of FHL transfer to the AT and to compare the transtendinous technique to the transosseous technique. We hypothesized that the type of technique would have a notable impact on outcome. METHODS: Forty patients (42 ankles) were retrospectively reviewed and divided into group 1 (transtendinous technique, 22 patients/24 ankles) and group 2 (transosseous technique, 18

Equinus foot classification in cerebral palsy: an agreement study between clinical and gait analysis assessment.

BACKGROUND: Excessive ankle plantar flexion (equinus foot) is a common problem in cerebral palsy (CP) and several treatment options can be considered depending on the equinus type. Few attempts have been made to classify different forms of equinus foot for specific treatment. AIM: This study is aimed at defining equinus foot types in CP patients according to the Ferrari classification, integrating clinical and instrumental assessments. The hypothesis is that clinical differentiation of equinus foot can be evidenced by recurrent anomalies identifiable through gait analysis (GA), which can make

Identifying feigned isokinetic trunk extension effort in normal subjects: an efficiency study of the DEC.

STUDY DESIGN: A test-retest comparison of maximal and feigned trunk extension effort in normal subjects was performed. OBJECTIVE: To test the robustness of an index termed DEC for differentiating maximal from feigned effort in normal subjects. SUMMARY OF BACKGROUND DATA: Previous studies have indicated that the DEC, which is the difference between the eccentric to concentric strength ratio at two test velocities, is a powerful identifier of feigned effort. This has been found for various muscle groups including the trunk extensors. However, whether this efficiency is affected by exposure to

Measuring postural control during mini-squat posture in men with early knee osteoarthritis.

Studies have suggested a compromised postural control in individuals with knee osteoarthritis (OA) evidenced by larger and faster displacement of center of pressure (COP). However, quantification of postural control in the mini-squat posture performed by patients with early knee OA and its relation to muscle strength and self-reported symptoms have not been investigated. The main aim of this cross-sectional, observational, controlled study was to determine whether postural control in the mini-squat posture differs between individuals with early knee OA and a control group (CG) and verify the r
