[Clinical examination for patellofemoral problems].

Patellofemoral pain and instability account for 25% of all sports-related knee problems. However, definitive analysis seems to be difficult. Descriptive findings like "anterior knee pain", "patellofemoral pain syndrome", or "chondromalacia" are used instead of diagnosis. One reason may be the complex pathomechanics of the patellofemoral joint which is suspended between the pelvis, the femur, and the tibia and subsequently is influenced by the whole lower extremity from the hip down to the foot. On the other hand, different signs and functional tests are in use with at times lack of quantificat

Tibial tubercle transfer on a medial periosteal pedicle--a report of a new technique.

Tibial tubercle transfer is still probably the most widely used procedure of the numerous operative procedures described to realign the patella and extensor mechanism and to prevent a recurrent dislocation. Although this procedure most likely disturbs the blood supply to the tibial tubercle and thus may lead to a delayed union. Tibial tubercle transfer is also considered to play a role in the incidence of a tibial tubercle delayed union. Furthermore, a fracture of the tibial metaphysis has been reported to occur in some cases. We therefore devised a new procedure in which the periosteum of the

The relationship of the femoral physis and the medial patellofemoral ligament in children: a cadaveric study.

BACKGROUND: Patellar dislocations are common in skeletally immature athletes, and the medial patellofemoral ligament (MPFL) is an important primary restraint to lateral patellar translation. The relationship between the MPFL femoral origin footprint and femoral physis is unclear. The purpose of this study was to evaluate the MPFL femoral origin footprint and its relationship to the femoral physis in skeletally immature anatomic specimens. METHODS: Six skeletally immature cadaver knee specimens were examined through gross dissection (group A: 1, 11, and 11 mo; and group B: 8, 10, and 11 y). Me

Prospective analysis of uncomplicated bone bruises in the pediatric knee.

To determine the incidence of uncomplicated knee bone contusions in pediatric patients. MRI studies were obtained using either high-field (1.5 T) or mid-field strength magnets (0.2-0.3 T), identifying 48 pediatric patients suitable for study. Contusion location, size, and any ligamentous or meniscal injuries were recorded. Exclusionary criteria did not include plain film findings, the interval between injury to imaging, or history of patellar dislocation [Fulkerson (2002) 30:447-456]. Uncomplicated bone bruises were those occurring in the absence of other internal derangements of the knee, suc

Correlation Between Hyperlipidemia-Related Diseases and Thorax/Thigh Circumference Ratio Along with Body Condition Score in Dogs Focusing on Molecular Mechanism: A Pilot Study and Literature Review.

There are some limitations to using the body condition score (BCS) for client education to prevent obesity, hyperlipidemia-related diseases, and orthopedic diseases in dogs because it is hard to quantify in detail. Especially in small dogs, patellar luxation is a common orthopedic disease that is related to obesity and the hind leg muscle. In this pilot study, the author evaluated the thorax/thigh circumference ratio as a prognostic evaluation index, along with the BCS, for assessing patellar dislocation and other hyperlipidemia-related diseases and states such as hypertriglyceridemia and obes

Clinical heterogeneity of polish patients with KAT6B-related disorder.

BACKGROUND: Say-Barber-Biesecker-Young-Simpson (SBBYSS) variant of Ohdo syndrome is a rare, autosomal dominant and clinically heterogenous disorder, caused by pathogenic variants in the KAT6B gene located on chromosome 10q22.2. KAT6B encodes a highly conserved histone acetyltransferase belonging to the MYST family. Currently, diseases caused by pathogenic variants in KAT6B (KAT6B-related disorders) comprise two allelic entities: SBBYSS variant of Ohdo syndrome and genitopatellar syndrome (GPS). Increase in the number of cases with overlapping GPS/SBBYSS phenotype which makes it necessary to re

Reconstruction of the Medial Patellofemoral Ligament Using a Synthetic Graft With Arthroscopic Control of Patellofemoral Congruence.

PURPOSE: To compare the results of reconstruction of the medial patellofemoral ligament (MPFL) using a synthetic graft (Poly-Tape) between knee joints in which the patella was reduced to the strict center and those in which it was slightly lateral to the center of the trochlea to determine whether patellar position within this range affects the results. METHODS: Forty-six knee joints in 46 patients were examined retrospectively with a minimum follow-up of 2 years. The position of the patellar central ridge in the trochlea on arthroscopy immediately after reconstruction of the MPFL was measure

Dynamic arthroscopy in retropatellar cartilage defect: identifying trochlear ridge impingement and guiding towards mini-trochleoplasty.

Retropatellar (RP) chondral defects are commonly associated with trauma, sports-related injuries, or patellar dislocations. Effective management typically requires both cartilage repair and correction of underlying predisposing biomechanical factors. However, conventional imaging modalities may fail to detect subtle abnormalities. We describe a young female with persistent anterior knee pain in the absence of trauma or instability. Imaging studies, including radiographs and magnetic resonance imaging (MRI), confirmed an RP chondral defect, with no evidence of patella alta, malalignment or signi

The Rotaflex total knee replacement--a 5 year review.

We report the results of a retrospective analysis of 43 patients who received 56 Rotaflex total knee arthroplasties, with a mean follow-up of 55.7 months. The British Orthopaedic Association (BOA) knee assessment protocol was used in evaluating the clinical results. Two patients could not receive post-operative scores. In the remaining 54 knees, the mean pre-operative score was 25.6, improving to 30.8 post-operatively. Ten knees showed a decrease in knee score, two were unchanged and 42 improved. The greatest improvements were in pain relief and maximum flexion. Wound infection and dehiscences

Hip injuries in children and adolescents.

Hip injuries in children present a wide spectrum of problems. Frequently because of the severity of the trauma involved, other injuries may take precedence and may require modification of the usual approaches to treatment. However, certain precepts are essential to the successful treatment of these injuries and should be kept in mind regardless of the milieu in which they are found. In the child with a hip dislocation, the potential presence of an acetabular, femoral head, femoral shaft, patellar or tibial plateau fracture must always be considered. Specialized x-ray views are necessary for th

Measurement of TT-TG can change with sequential MRIs due to variations in tibiofemoral rotation in patellofemoral instability patients.

PURPOSE: There are various anatomic risk factors for patellofemoral instability (PFI) that help guide surgical treatment, including the tibial tubercle to trochlear groove (TT-TG) distance. However, no study has analysed the temporal changes in TT-TG prior to surgical intervention. This study sought to understand the variations in TT-TG over time for pediatric patients suffering from PFI prior to surgical intervention. The authors hypothesised that the TT-TG would substantially change between time points. METHODS: Patients undergoing medial patellofemoral ligament (MPFL) reconstruction betwee

Medialization of trochlear groove was correlated with extended lateral trochlear in trochlear dysplasia: a transverse CT analysis.

PURPOSE: To investigate the difference of trochlear width between normal and dysplastic trochlear and to analyze whether the medialization of trochlear groove was correlated with abnormal width of trochlear facets in trochlear dysplasia. METHODS: This study involved CT scans of fifty knees with trochlear dysplasia (TD group) and fifty knees without obvious trochlear dysplasia (Normal group). The linear distance from the medial femoral epicondyle to the various reference points was measured on axial CT images which included the medial edge of medial trochlear facet (dMTE), trochlear groove (dT

Revision lateralization osteotomy of the tibial tubercle has excellent outcomes in patients suffering from previous medial tibial tubercle overcorrection.

INTRODUCTION: Tibial tubercle osteotomy (TTO) is a common procedure used to treat patients with patellofemoral instability (PFI) and osteoarthritis (PFOA). Medial patellar maltracking due to previous excessive medialization of the tibial tubercle has rarely been reported. Therefore, the goal of this study was to assess patient-reported outcome measures (PROMs) after revision osteotomy with lateralization of the tibial tubercle (RL-TTO) to correct medial patellofemoral maltracking. MATERIALS AND METHODS: Between 2017 and 2021, a series of 11 patients (male/female 1/10; age 35.8 ± 10.5 years) w

Modern Treatment of Recurrent Patellofemoral Instability - Combined Medial Patellofemoral Ligament Reconstruction and Tibial Tubercle Transfer.

PURPOSE OF THE STUDY The treatment of recurrent patellofemoral instability has undergone a paradigm change during the last 15 years. Instead of non-anatomical operations, the current concept favors the reconstruction of injured structures and the normalization of the biomechanical environment. Our aim was to briefly summarize the etiology, diagnostic regimen, and therapeutic decision making of this varied patient group and to review our own patients who underwent combined medial patellofemoral ligament reconstrucion and tibial tubercle transfer. MATERIAL AND METHODS Between 2015-2017 we perfor

Combined transverse femoral ostectomy and tibial tuberosity distalisation for correction of medial patella luxation and patella alta in dogs.

BACKGROUND: Patella alta is the proximal displacement of the patella within the femoral trochlea. Previous studies have identified an association between patella alta and patella luxation. Distalisation of the tibial tuberosity has been recommended to establish proximodistal alignment of the stifle extensor mechanism with the underlying femur in dogs affected by patella alta. However, a recent canine ex vivo study found stifle extensor mechanism load increased significantly following tibial tuberosity distalisation. Generation of excessive load within the stifle extensor mechanism was avoidabl
