Association of Maternal Risk Factors With First-Trimester Missed Abortion: A Cross-Sectional Study.

Background First-trimester‍‌‍‍‌‍‌‍‍‌ missed abortion, that is, the death of the embryo or fetus without expulsion, is still the leading cause of pregnancy loss in the early stages, both worldwide and in India. The incidence of this event is a cry of demographic, metabolic, obstetric, and lifestyle-related maternal factors, with the respective patterns showing considerable differences from one region to another. Localized evidence is very important for the early warning of at-risk pregnancies. This research describes the distribution of maternal risk factors for the first-trimester missed

Sleep patterns of shiftworkers.

Work hours affect sleep because the sleep behavior of people on nonworkdays differs from that on workdays. For most workers, workday sleep is shorter than nonworkday sleep. On nonworkdays most workers sleep at night. Work schedule interacts with the effects of work hours on workday sleep. On workdays, people on the nightshift sleep the least, people on the afternoon/evening shift sleep the most, and people on the dayshift sleep less than those on the afternoon/evening shift but more than those on the nightshift. Thus, the infradian sleep strategy elected by a shiftworker is ordinarily related

Chest pain syndromes in pregnancy.

Chest pain syndromes in pregnancy include numerous catastrophic cardiovascular events. Acute myocardial infarction, aortic dissection, pulmonary embolism, and amniotic fluid embolism are the most important causes of nonobstetric mortality and morbidity in pregnancy. Each of these could result in poor maternal and fetal outcomes if not diagnosed and treated in a timely fashion. However, their diagnosis and management is limited by fetal risks of diagnostic procedures, dangers of pharmacotherapy and interventions that have neither been widely studied nor validated. This article reviews the curre

Artificial intelligence for personalized multiple micronutrient supplementation in maternal health.

Maternal undernutrition and micronutrient deficiencies remain pervasive, contributing to adverse pregnancy outcomes and long-term health risks for mothers and offspring. Multiple micronutrient supplementation (MMS) during pregnancy has demonstrated benefits, including reduced risks of low birth weight, small-for-gestational-age births, and neonatal mortality, when compared with standard iron-folic acid supplementation. Current MMS strategies, however, often follow a standard MMS, overlooking variations in nutritional status, health profiles, and context. Advances in artificial intelligence (AI

Opening Pandora's box: abnormal genetic carrier screening and need for lifetime follow-up.

OBJECTIVE: Expanded carrier screening (ECS) is rising in popularity because of its application in a diverse population, its decreasing cost, and efficiency.1 However, it has traditionally been used to assess fetal risk. The next generation sequencing ECS panel offered at our academic medical center consists of 283 genes associated with hereditary disorders. Of those, 20 (7.1%) are autosomal recessive conditions, notable for variable expression of the clinical phenotype in heterozygous carriers, which may increase maternal risk for malignancy, bleeding, cardiovascular, or rheumatologic disease.

The use of folic acid for the prevention of neural tube defects and other congenital anomalies.

OBJECTIVE: To provide information regarding the use of folic acid for the prevention of neural tube defects (NTDs) and other congenital anomalies, in order that physicians, midwives, nurses, and other health-care workers can assist in the education of women in the preconception phase of their health care. OPTION: Folic acid supplementation is problematic, since 50% of pregnancies are unplanned and the health status of women may not be optimal. OUTCOMES: Folic acid supplementation has been proven to decrease or minimize specific birth defects. EVIDENCE: A systematic review of the literature,

A complex behavioural change intervention to reduce the risk of diabetes and prediabetes in the pre-conception period in Malaysia: study protocol for a randomised controlled trial.

BACKGROUND: Over the past two decades, the population of Malaysia has grown rapidly and the prevalence of diabetes mellitus in Malaysia has dramatically increased, along with the frequency of obesity, hyperlipidaemia and hypertension. Early-life influences play an important role in the development of non-communicable diseases. Indeed, maternal lifestyle and conditions such as gestational diabetes mellitus or obesity can affect the risk of diabetes in the next generation. Lifestyle changes can help to prevent the development of type 2 diabetes mellitus. This is a protocol for an unblinded, comm

Live virus vaccination near a pregnancy: flawed policies, tragic results.

Vaccination of women with live virus vaccines around conception has always been contraindicated by the Center for Disease Control and Prevention (CDC) and the vaccine manufacturer because of potential risks to the fetus. Nevertheless this dangerous practice occurs and is associated with maternal health problems and a very high incidence of early-onset autism in the children. Postpartum vaccination with live virus vaccines has been recommended by the CDC, and described as 'convenient' by the vaccine manufacturer. This 'routine practice' may lead to health and is also associated with many health

Impact of being underweight before pregnancy on preterm birth by race/ethnicity and insurance status in California: an analysis of birth records.

BACKGROUND: The US still has a high burden of preterm birth (PTB), with important disparities by race/ethnicity and poverty status. There is a large body of literature looking at the impact of pre-pregnancy obesity on PTB, but fewer studies have explored the association between underweight status on PTB, especially with a lens toward health disparities. Furthermore, little is known about how weight, specifically pre-pregnancy underweight status, and socio-economic-demographic factors such as race/ethnicity and insurance status, interact with each other to contribute to risks of PTB. OBJECTIVE

Pregnancy after bariatric surgery and adverse perinatal outcomes: A systematic review and meta-analysis.

BACKGROUND: Women who undergo bariatric surgery prior to pregnancy are less likely to experience comorbidities associated with obesity such as gestational diabetes and hypertension. However, bariatric surgery, particularly malabsorptive procedures, can make patients susceptible to deficiencies in nutrients that are essential for healthy fetal development. The objective of this systematic review and meta-analysis is to investigate the association between pregnancy after bariatric surgery and adverse perinatal outcomes. METHODS AND FINDINGS: Searches were conducted in Medline, Embase, PsycINFO,

Digital Health Interventions Providing Behavioral Assessment and Goal Prioritization Support: Scoping Review.

BACKGROUND: Affordability of health care systems depends on populations' engagement in preventive health behavior and appropriate self-management of long-term conditions. Digital health interventions (DHIs) could facilitate this by prompting and supporting individual health behavior change. Behavior change is often undermined by suboptimal prioritization of goals. Therefore, DHIs aiming to promote behavior change should help users identify behavior patterns that need changing and scaffold goal prioritization. OBJECTIVE: This scoping review explores the extent to which DHIs are supporting user

Association Between Serum Glycated Hemoglobin Levels at Early Gestation and the Risk of Subsequent Pregnancy Loss in Pregnant Women Without Diabetes Mellitus: Prospective Cohort Study.

BACKGROUND: As a severe morbidity during pregnancy, the etiology of spontaneous pregnancy loss (SPL) remains largely unknown. Serum glycated hemoglobin (HbA1c) level is an established predictor of SPL risk among women with diabetes, but little is known about whether such an association exists among pregnant women without diabetes when glycemic levels are within the normal range. OBJECTIVE: This study aimed to quantify the association between maternal HbA1c levels in early pregnancy and subsequent SPL risk in a cohort of pregnant women without diabetes. METHODS: This prospective cohort study

Hookworm Infection among Pregnant Women at First Antenatal Visit in Lira, Uganda: A Cross-Sectional Study.

BACKGROUND: Hookworm infection in expectant mothers has adverse health effects on both the mothers and their unborn babies. Foetal effects are known to include intrauterine growth retardation and physical and mental growth retardation, while the mothers may develop anemia which could potentially result in death. Unfortunately, little is known about factors that may predispose a pregnant woman to infection by hookworm. In this study, we strived to determine not only the prevalence of hookworm infection among pregnant women attending their first antenatal visit during the current pregnancy in a

Diagnosis and Management of Gestational Diabetes Mellitus: An Overview of National and International Guidelines.

IMPORTANCE: Gestational diabetes mellitus (GDM) represents one of the most frequent complications of pregnancy and is associated with increased maternal and neonatal morbidity. Its incidence is rising, mostly due to an increase in maternal age and maternal obesity rate. OBJECTIVE: The aim of this study was to review and compare the recommendations of the most recently published guidelines on the diagnosis and management of this condition. EVIDENCE ACQUISITION: A descriptive review of guidelines from the National Institute for Health and Care Excellence (NICE), the International Federation of

The Effectiveness of Bariatric Surgery on Treating Infertility in Women-A Systematic Review and Meta-Analysis.

Background/Objectives: Obesity is a growing global health concern, which increases the risk of various diseases and has seen a rising prevalence over time. The global prevalence of obesity among adults has doubled over time. Obesity significantly impacts health by increasing the risk of a range of severe medical conditions. Cardiovascular diseases, such as heart attacks and strokes, are more prevalent in individuals with obesity due to factors like high blood pressure and abnormal cholesterol levels. This systematic review and meta-analysis sought to establish the effectiveness of bariatric su
