Translation regulation promotes stress adaptation in the human fungal pathogen Candida glabrata.

Invasive candidiasis presents a significant healthcare challenge. The human opportunistic fungal pathogen Candida glabrata, a cause of mucosal and deep-seated infections, resists key antifungal drugs and rapidly proliferates within host macrophages, where it withstands high oxidative stress and amino acid starvation. Unlike C. albicans, C. glabrata lacks true hyphae and relies more on stress adaptation mechanisms than filamentation for virulence. This study explores the molecular mechanisms underlying stress adaptations in C. glabrata that contribute to its pathogenicity. Our findings revealed

Candidemia Following Severe COVID-19 in Hospitalised and Critical Ill Patients: A Systematic Review and Meta-Analysis.

RATIONALE: The epidemiology and clinical impact of COVID-19-associated candidemia (CAC) remained uncertain, leaving gaps in understanding its prevalence, risk factors and outcomes. METHODS: A systematic review and meta-analysis were conducted by searching PubMed, Embase and Scopus for reports of CAC prevalence, risk factors and clinical outcomes up to June 18, 2024. The generalised linear mixed model was employed to determine the prevalence and 95% confidence intervals (CIs). The risk factors and clinical outcomes were compared between patients with and without CAC using the inverse variance

SPaCE Swab: Point-of-Care Sensor for Simple and Rapid Detection of Acute Wound Infection.

Wound infection is commonly observed after surgery and trauma but is difficult to diagnose and poorly defined in terms of objective clinical parameters. The assumption that bacteria in a wound correlate with infection is false; all wounds contain microorganisms, but not all wounds are clinically infected. This makes it difficult for clinicians to determine true wound infection, especially in wounds with pathogenic biofilms. If an infection is not properly treated, pathogenic virulence factors, such as rhamnolipids from Pseudomonas aeruginosa, can modulate the host immune response and cause tis

Multi-omics characterization of the necrotrophic mycoparasite Saccharomycopsis schoenii.

Pathogenic yeasts and fungi are an increasing global healthcare burden, but discovery of novel antifungal agents is slow. The mycoparasitic yeast Saccharomycopsis schoenii was recently demonstrated to be able to kill the emerging multi-drug resistant yeast pathogen Candida auris. However, the molecular mechanisms involved in the predatory activity of S. schoenii have not been explored. To this end, we de novo sequenced, assembled and annotated a draft genome of S. schoenii. Using proteomics, we confirmed that Saccharomycopsis yeasts have reassigned the CTG codon and translate CTG into serine i

Adaptation of the tetracycline-repressible system for modulating the expression of essential genes in Cryptococcus neoformans.

The opportunistic human fungal pathogen Cryptococcus neoformans has an enormous impact on human health as the causative agent of cryptococcal meningitis, and there is a dire need to expand our current antifungal arsenal. Essential gene products often serve as ideal targets for antimicrobials, and identifying and characterizing essential genes in a pathogen of interest is critical for drug development. Unfortunately, characterization of essential genes in C. neoformans is limited due to its haploid nature and lack of genetic tools for generating effective conditional-expression mutants. To date

Update on therapeutic approaches for invasive fungal infections in adults.

Invasive fungal infections are increasingly encountered with the expansion of iatrogenic immunosuppression, including not only solid organ and hematopoietic stem cell transplant recipients but also patients with malignancies or autoimmune diseases receiving immunomodulatory therapies, such as Bruton Tyrosine Kinase (BTK) inhibitor. Their attributable mortality remains elevated, part of which is a contribution from globally emerging resistance in both molds and yeasts. Because antifungal susceptibility test results are often unavailable or delayed, empiric and tailored antifungal approaches inc

Ibrexafungerp: A narrative overview.

Ibrexafungerp (IBX) is a new antifungal drug that recently entered the antifungal landscape. It disrupts fungal cell wall synthesis by non-competitive inhibition of the β-(1,3)-D-glucan (BDG) synthase enzyme. It has demonstrated activity against a range of pathogens including Candida and Aspergillus spp., as well as retaining its activity against azole-resistant and echinocandin-resistant strains. It also exhibits anti-biofilm properties. Pharmacokinetic (PK) studies revealed favorable bioavailability, high protein binding, and extensive tissue distribution with a low potential for CYP-mediate

Effectiveness of ultraviolet-C vs aerosolized hydrogen peroxide in ICU terminal disinfection.

BACKGROUND: According to the Centers for Disease Control and Prevention, 10% of patients with healthcare-acquired infections (HAIs) died during hospitalization in 2015. Thus, a reduction in the prevalence of HAIs is critical. One strategy to achieve this is the adequate disinfection of patient rooms within hospitals. AIM: To compare the effectiveness of an ultraviolet-C (UV-C) room sanitizer with an aerosolized hydrogen peroxide (aHP) device for the elimination of selected healthcare-associated (HA) pathogens and other HA organisms in an intensive care unit (ICU) setting. METHODS: The disinf

The rise of WHO-priority pathogens in central line associated bloodstream infection: Challenging the AWaRe paradigm in critical care.

Central line-associated bloodstream infections (CLABSIs) pose a serious threat to critically ill patients, particularly in low- and middle-income countries facing rising antimicrobial resistance (AMR). Integrating WHO's Priority Pathogen Lists (PPL) and AWaRe antibiotic classification into CLABSI surveillance provides a clinically meaningful framework to guide antimicrobial stewardship and infection control. We conducted an observational study in ICUs from 2021 to 2024. Patients with central lines in situ for ≥ 2 calendar days were evaluated using CDC-NHSN definitions. Isolates underwent antim

Rapid broad-spectrum activity of molecular iodine against multidrug-resistant bacterial and fungal pathogens without residual staining.

Povidone iodine (PVP-I) is a widely used topical antiseptic with broad antimicrobial activity, but its clinical utility can be limited by skin irritation, slow drying, and persistent staining of skin, textiles, and surfaces. Here, we evaluated reformulated molecular iodine (I2) products as potential non-staining alternatives with retained or enhanced antimicrobial efficacy. Three I2 formulations (soak, nasal antiseptic, and hand ointment) were tested against 17 clinically relevant bacterial and fungal pathogens, including multidrug-resistant organisms and emerging nosocomial pathogens. Antimic

Streamlining fungal diagnostics: Evaluation of a Direct-to-PCR extraction-free workflow for candida detection.

Fungal infections are an escalating health threat, and delays from conventional nucleic acid extraction hinder timely diagnosis and treatment. This study evaluated the diagnostic performance of a novel extraction-free technology, Direct-to-PCR (D2P; Scienetix, Tyler, TX, USA), for the detection of clinically significant Candida species (C. albicans, C. glabrata, C. auris, C. parapsilosis, and C. tropicalis). D2P was compared against conventional silica column-based (Qiagen) and magnetic bead-based (KingFisher) extraction methods, using microbial reference isolates, residual clinical specimens,

Determinants of fluconazole resistance and echinocandin tolerance in C. parapsilosis isolates causing a large clonal candidemia outbreak among COVID-19 patients in a Brazilian ICU.

Patients presenting with severe COVID-19 are predisposed to acquire secondary fungal infections such as COVID-19-associated candidemia (CAC), which are associated with poor clinical outcomes despite antifungal treatment. The extreme burden imposed on clinical facilities during the COVID-19 pandemic has provided a permissive environment for the emergence of clonal outbreaks of multiple Candida species, including C. auris and C. parapsilosis. Here we report the largest clonal CAC outbreak to date caused by fluconazole resistant (FLZR) and echinocandin tolerant (ECT) C. parapsilosis. Sixty C. par

Exploring the potential of hydro alcoholic crude extract of beeswax as antibacterial antifungal antiviral antiinflammatory and antioxidant agent.

Apis mellifera beeswax is synthesized from honey sugars and secreted by specialized glands located on the ventral side of the abdomen of worker bees aged 12-18 days. This beeswax possesses various therapeutic properties beneficial to human health. Given the priority in pharmacological research to develop new drugs from natural sources with minimal side effects, the absence of studies on the biological activities of A. mellifera beeswax is notable. Therefore, this study aims to investigate the chemical composition, as well as the anti-inflammatory, antiviral, cytotoxic, antimicrobial, and antio

Analysis and comparison of cumulative antibiograms for the Charlotte Maxeke Johannesburg Academic Hospital adult intensive care and high-care units, 2013 and 2017.

BACKGROUND: Infection is a common complication for patients in intensive care units (ICUs), and increasing antimicrobial resistance (AMR) is a major concern. It is therefore crucial to monitor AMR patterns in order to support clinical decision-making and antimicrobial stewardship strategies. Clinical microbiologists should provide annual cumulative antibiogram reports, which can be used to guide initial empirical antimicrobial therapy for the management of infections. OBJECTIVES: To analyse the cumulative antibiograms for the Charlotte Maxeke Johannesburg Academic Hospital (CMJAH) combined ad

Engineering Penicillium expansum antifungal proteins unveils new clues about their mode of action.

Fungal antifungal proteins (AFPs) are promising biofungicides. PeAfpA and PeAfpB from Penicillium expansum show different activity profiles and potency, with PeAfpA being more active. Based on the PeAfpB solved structure, we had previously designed PeAfpB-PeAfpA chimeras that showed different properties. From these, we engineer here two additional variants, chPeAFPV6 and chPeAFPV7, that revealed novel aspects of the AFP structure, antifungal determinants and mechanism. chPeAFPV6, with a single E11K mutation in the loop L1 that is part of the γ-core motif, increased PeAfpB antifungal activity t
