Revisitation of Double-Inlet Left Ventricle or Tricuspid Atresia With Transposed Great Arteries.

BACKGROUND: Patients with double-inlet left ventricle or tricuspid atresia with transposition of the great arteries are predisposed to systemic outflow tract obstruction necessitating systemic outflow relief operations (SORO). METHODS: Between January 2000 and February 2018, 30 patients with double-inlet left ventricle (n = 20) or tricuspid atresia (n = 10) with transposition of the great arteries underwent single-ventricle palliation. Arch obstruction was observed in 14 patients. Initial palliative procedures comprised pulmonary artery banding with (n = 4) or without (n = 14) arch repair, bi

The American Association for Thoracic Surgery (AATS) 2022 Expert Consensus Document: Management of infants and neonates with tetralogy of Fallot.

OBJECTIVE: Despite decades of experience, aspects of the management of tetralogy of Fallot with pulmonary stenosis (TOF) remain controversial. Practitioners must consider newer, evolving treatment strategies with limited data to guide decision making. Therefore, the TOF Clinical Practice Standards Committee was commissioned by the American Association for Thoracic Surgery to provide a framework on this topic, focused on timing and types of interventions, management of high-risk patients, technical considerations during interventions, and best practices for assessment of outcomes of the interve

Tetralogy of Fallot and abnormal coronary artery: use of a prosthetic conduit is outdated.

OBJECTIVES: Repair of tetralogy of Fallot (ToF) can be challenging in the presence of an abnormal coronary artery (CA) in 5-12% of cases. The aim of this study was to report our experience with ToF repair without the systematic use of a right ventricle-to-pulmonary artery (RV-PA) conduit. METHODS: We conducted a monocentric retrospective study from 2000 to 2016, including 943 patients with ToF who underwent biventricular repair, of whom 8% (n = 76) presented with an abnormal CA. Mean follow-up time was 50 months (1 month-18 years). RESULTS: The most frequent CA anomaly was the left descendin

Truncus arteriosus repair: outcomes, risk factors, reoperation and management.

OBJECTIVES: Truncus arteriosus (TA) continues to be associated with significant morbidity and mortality, but there have been clinically significant improvements with early repair. METHODS: Sixty patients underwent physiological correction of TA between November 1978 and January 2000. The average age was 76 days (range, 3 days--20 months). Associated cardiac anomalies were frequently encountered, the most common being severe truncal valve regurgitation (n=7), interrupted aortic arch (n=6), coronary artery anomalies (n=6), non-confluent pulmonary arteries (n=4), and total anomalous pulmonary ve

Septal dysfunction and Abnormal Systolic Stretch Are Associated with Right Ventricular Dysfunction in Repaired Tetralogy of Fallot.

AIMS: Right ventricular (RV) dysfunction is a major determinant of outcomes after Tetralogy of Fallot repair (rToF). Although right bundle branch block is present in most patients and electro-mechanical dyssynchrony (EMD) has been associated with RV dysfunction, only a subset of patients ultimately develops RV dysfunction. We hypothesized that septal dysfunction is a key feature associated with the development of EMD-induced RV dysfunction in rToF. METHODS: We retrospectively studied 126 pediatric rToF patients (median age 14.7 years) with contemporaneous ECG, echocardiography, and cardiac ma

Tetralogy of Fallot: what operation, at which age.

BACKGROUND: The optimal management of tetralogy of Fallot is still under debate, particularly with respect to surgical approach and the age of operation. In recent times a transatrial-transpulmonary approach and primary repair in younger patients is favoured. The purpose of the present study was to analyze the result of our current surgical management by assessing the perioperative and intermediate term follow up in order to define the optimal strategy and timing of operation for our institution. METHODS: One hundred and thirty two patients with tetralogy of Fallot who underwent definitive re

Percutaneous transluminal septal myocardial ablation for hypertrophic obstructive cardiomyopathy; The Chinese experience in 119 patients from a single center.

BACKGROUND: Percutaneous transluminal septal myocardial ablation (PTSMA) has been introduced as an alternative treatment for hypertrophic obstructive cardiomyopathy (HOCM). We report on the acute, short- and long-term results of our experiences in 119 patients from a single center in China. METHODS AND RESULTS: PTSMA was performed in 119 patients with symptomatic HOCM (mean age 35.4+/-14.8 years, male 80, female 39). All patients had echocardiography performed prior to the procedure, 2-week post-PTSMA, and 6-month post-PTSMA, and 65 patients had echocardiography repeated at 2-year follow-up.

Stent fracture, valve dysfunction, and right ventricular outflow tract reintervention after transcatheter pulmonary valve implantation: patient-related and procedural risk factors in the US Melody Valve Trial.

BACKGROUND: Among patients undergoing transcatheter pulmonary valve (TPV) replacement with the Melody valve, risk factors for Melody stent fracture (MSF) and right ventricular outflow tract (RVOT) reintervention have not been well defined. METHODS AND RESULTS: From January 2007 to January 2010, 150 patients (median age, 19 years) underwent TPV implantation in the Melody valve Investigational Device Exemption trial. Existing conduit stents from a prior catheterization were present in 37 patients (25%, fractured in 12); 1 or more new prestents were placed at the TPV implant catheterization in 5

PTFE monocusp valve reconstruction of the right ventricular outflow tract.

BACKGROUND: Transannular patching of right ventricular outflow tract obstructive (RVOTO) defects results in pulmonary insufficiency (PI). Biologic monocusp valves (MO) can prevent acute PI but are prone to early degeneration and progressive regurgitation. Polytetrafluoroethylene (PTFE, 0.1 mm) MO leaflets demonstrated favorable characteristics in animal studies, and the technique was applied to a variety of RVOTO anomalies. METHODS: From June 1990 through June 1999, 158 patients underwent either PTFE MO RVOT reconstruction (n = 115 patients; 120 implants) or nonvalved transannular repair (TA)

[Diagnostic value of transesophageal echocardiography in diseases of the heart valve system].

Due to high resolution transesophageal echocardiography has been proven to have additional diagnostic benefits in valvular heart disease. The quantification of aortic and mitral valve insufficiency is possible by the use of semiquantitative methods. In aortic valve insufficiency the relation of the regurgitant jet width to the left ventricular outflow tract and in mitral valve insufficiency the relation of the regurgitant jet area to the left atrium are the parameters with the most accurate sensitivity. The main advantage of the method in this point is the rapid detection of morphological path

Survival Differences in Women and Men After Septal Myectomy for Obstructive Hypertrophic Cardiomyopathy.

IMPORTANCE: Recent data indicate that women with hypertrophic cardiomyopathy (HCM) are older and more symptomatic at presentation and have worse clinical outcomes than men. However, to our knowledge, there are no large studies of the association of patient sex with outcomes after surgical myectomy. OBJECTIVE: To analyze preoperative characteristics and overall survival of women and men undergoing septal myectomy for obstructive HCM. DESIGN, SETTING, AND PARTICIPANTS: This retrospective, single-center study included the clinical data of adult patients who underwent septal myectomy from Januar

[Prenatal diagnosis of heart defects and associated chromosomal aberrations].

AIM: According to epidemiological studies on newborns, the association of congenital heart defects with chromosomal anomalies varies between 4 and 12%. Prenatally this rate is probably higher, due to antenatal death occurring in fetuses with chromosomal aberrations. The aim of the study was therefore to determine the rate and the distribution of chromosomal aberrations in prenatally detected heart defects. PATIENTS AND METHOD: Within a period of 7 years fetal echocardiography was performed on 2716 fetuses at high risk for CHD. The analysis of the fetal heart was achieved by the visualization

Surgical management of congenital heart defects associated with heterotaxy syndrome.

OBJECTIVE: Heterotaxy syndrome (HS) is generally associated with complex congenital cardiac defects and has a high morbidity and mortality despite recent surgical progress. Only few reports deal with an overall surgical population. METHODS: Between 1989 and 2008, 139 patients with HS entered a programme of surgical repair. Fifty-one patients were suitable for univentricular heart repair (UVR) and 88 for biventricular repair (BVR). Among those tracked for UVR, two were switched to BVR and 11 from BVR to UVR. Median age at first surgery was 4.4 months (range: 3 days to 43 years] of whom 34 were

Transcatheter pulmonary valve replacement using the melody valve for treatment of dysfunctional surgical bioprostheses: A multicenter study.

BACKGROUND: Stented bioprosthetic valves (BPVs) are commonly used for surgical pulmonary valve (PV) replacement in postoperative congenital heart disease, but develop structural failure in a time-related fashion. The Melody transcatheter PV (TPV) (Medtronic, Minneapolis, Minn) has been used to treat BPV dysfunction, but there have been few studies in this population. METHODS: We performed a retrospective, multicenter study to evaluate Melody valve function in patients who underwent TPV replacement (TPVR) into a dysfunctional pulmonary BPV. RESULTS: One hundred patients who underwent TPVR at

Disrupted blood-brain barrier maturation links congenital heart diseases to hippocampal injury: Insights from a translational neonatal mouse model.

BACKGROUND: Neurodevelopmental impairment is the most common non-cardiac morbidity in children with congenital heart diseases (CHDs), yet its pathogenic mechanisms remain poorly understood due to a lack of disease-specific animal models. Right ventricular outflow tract obstruction (RVOTO) is a prevalent CHD lesion. We investigated whether neonatal RVOTO disrupts postnatal brain maturation, focusing on the blood-brain barrier (BBB). METHODS: RVOTO was induced in postnatal day 1 (P1) C57BL/6 mouse pups via pulmonary artery banding (PAB); sham-operated littermates served as controls. Hemodynamic

Is a hybrid strategy a lower-risk alternative to stage 1 Norwood operation?

BACKGROUND: For neonates with critical left ventricular outflow tract obstruction (LVOTO), hybrid procedures are an alternative to the Norwood stage 1 procedure. Despite perceived advantages, however, outcomes are not well defined. Therefore, we compared outcomes after stage 1 hybrid and Norwood procedures. METHODS: In a critical LVOTO inception cohort (2005-2014; 20 institutions), a total of 564 neonates underwent stage 1 palliation with the Norwood operation with a modified Blalock-Taussig shunt (NW-BT; n = 232; 41%), Norwood operation with a right ventricle-to-pulmonary artery conduit (NW-

Surgical outcomes in the treatment of patients with tetralogy of Fallot and absent pulmonary valve.

OBJECTIVE: Tetralogy of Fallot and absent pulmonary valve (TOF/APV) is associated with significant pulmonary artery dilatation and airway compression. Treatment of infants presenting with respiratory symptoms early in life is associated with high mortality (20-60%). We aim to report our results and identify factors associated with survival and prolonged ventilation. METHODS: We performed a retrospective review of 62 consecutive patients following repair of TOF/APV (1982-2006). Median age at repair was 1.4 years (1 day-35 years). Twenty patients required preoperative intubation. RESULTS: Sixt

Elective primary repair of acyanotic tetralogy of Fallot in early infancy: overall outcome and impact on the pulmonary valve.

OBJECTIVES: We sought to determine if early primary repair of acyanotic tetralogy of Fallot (ToF) can be performed safely with low requirement for transannular patching (TAP) and thereafter allow normal right ventricular outflow tract (RVOT) growth. BACKGROUND: Early primary repair of ToF normalizes intracardiac flow patterns, which may allow subsequent normal RVOT growth. Traditionally repair is deferred until symptoms occur or children are deemed of adequate size for operative risk to be acceptable because of a perceived increased requirement for TAP in small infants. METHODS: Between July

BACKGROUND: Percutaneous pulmonary valve implantation (PPVI) is a safe, less invasive alternative to surgical valve replacement for the congenital heart disease patient with right ventricular (RV) outflow tract dysfunction. The aim of this study was to determine whether reverse RV remodeling after PPVI was persistent in the longer term and whether timing of PPVI influenced outcomes. METHODS AND RESULTS: Consecutive patients from the pediatric and adult congenital heart disease programs were enrolled.
