Life-threatening spontaneous splenic rupture in congenital afibrinogenemia: Two case reports and systematic literature review.

BACKGROUND: Spontaneous splenic rupture is a rare life-threatening finding with a challenging diagnosis which is largely ignored in the literature. Hematological disorders such as afibrinogenemia are reported to cause bleeding disorders mostly cerebral hemorrhage. Despite being a life-threatening condition, data about spontaneous splenic rupture in patients with Afibrinogenemia remain scarce. Such fatal incidents should warrant diligent efforts to establish and adopt prevention measures in this particularly vulnerable population. We report two new cases of spontaneous splenic rupture in two si

Cost-effectiveness analysis of toripalimab for metastatic or recurrent triple-negative breast cancer.

BACKGROUND: Toliparibizumab in combination with nab-paclitaxel (T+N) has excellent efficacy inmetastatic or recurrent triple-negative breast cancer (TNBC), but the optimal choice of sequence of therapy is unclear given the trade-offs between quality of life and cost. Cost-effectiveness analyses can quantify these tradeoffs, leading to more informed decisions. Our objective was to assess the societal cost-effectiveness of the T+N regimen for metastatic or recurrent TNBC. METHODS: Clinical data were extracted from a multicenter, randomized, double-blind trial, TORCHLIGHT (NCT04085276). Patients

Legionella pneumophila CRISPR-Cas Suggests Recurrent Encounters with One or More Phages in the Family Microviridae.

Legionella pneumophila is a ubiquitous freshwater pathogen and the causative agent of Legionnaires' disease. L. pneumophila growth within protists provides a refuge from desiccation, disinfection, and other remediation strategies. One outstanding question has been whether this protection extends to phages. L. pneumophila isolates are remarkably devoid of prophages and to date no Legionella phages have been identified. Nevertheless, many L. pneumophila isolates maintain active CRISPR-Cas defenses. So far, the only known target of these systems is an episomal element that we previously named Leg

The Utility of Resolving Asthma Molecular Signatures Using Tissue-Specific Transcriptome Data.

An integrative analysis focused on multi-tissue transcriptomics has not been done for asthma. Tissue-specific DEGs remain undetected in many multi-tissue analyses, which influences identification of disease-relevant pathways and potential drug candidates. Transcriptome data from 609 cases and 196 controls, generated using airway epithelium, bronchial, nasal, airway macrophages, distal lung fibroblasts, proximal lung fibroblasts, CD4+ lymphocytes, CD8+ lymphocytes from whole blood and induced sputum samples, were retrieved from Gene Expression Omnibus (GEO). Differentially regulated asthma-rele

Psychological Therapy for People with Tinnitus: A Scoping Review of Treatment Components.

BACKGROUND: Tinnitus is associated with depression and anxiety disorders, severely and adversely affecting the quality of life and functional health status for some people. With the dearth of clinical psychologists embedded in audiology services and the cessation of training for hearing therapists in the UK, it is left to audiologists to meet the psychological needs of many patients with tinnitus. However, there is no universally standardized training or manualized intervention specifically for audiologists across the whole UK public healthcare system and similar systems elsewhere across the w

Toward A Whole-Brain Interface Enabled by High-Resolution Surface Electrophysiology.

The goal of building a whole-brain interface-capable of generating a real-time digital representation of the state of the entire central nervous system-has long been framed as a hardware challenge of recording simultaneously from ever-larger numbers of microelectrodes. Advances in high-resolution neural interfaces now enable electrophysiologic interaction with the human nervous system at spatial and temporal resolutions previously unattainable without disrupting tissue architecture or physiology. These capabilities create new opportunities for mechanistic neuroscience, neuroprosthetics, and pr

Diversity and geographical distribution of potential carbon monoxide oxidizers using molybdenum-containing enzymes in the ocean.

Carbon monoxide (CO) evolves from photochemical or thermal reactions in the ocean. It is estimated that 90% of oceanic CO is consumed by CO-oxidizing prokaryotes, which convert CO to carbon dioxide using molybdenum-containing CO dehydrogenase (Mo-CODH). Investigation of form I cox, which encodes Mo-CODH, has revealed that oceanic prokaryotes with form I cox (potential cox-containing CO oxidizers: pcox-CO oxidizers) belong to eight phyla and occupy 10%-20% of prokaryotes. However, previous studies may have overestimated their diversity due to the use of less stringent criteria, and their ecolog

Disentangling production and persistence of extracellular virions in grassland soils with SIP-viromics.

Viruses are abundant and ecologically important in soils, yet the persistence and production dynamics of extracellular virions remain poorly understood. We applied genome-resolved stable isotope probing viromics (SIP-viromics), combining H218O labeling with viral metagenomics, to track virion turnover in seasonally dry grassland soils following rewetting. We identified 354 viral populations (vOTUs) using individual-sample and combined virome assemblies. Only 22% of vOTUs exhibited significant 18O enrichment, indicating active replication and new virion production during the 1-week incubation;

AI Agents in Clinical Medicine: A Systematic Review.

BACKGROUND: AI agents built on large language models (LLMs) can plan tasks, use external tools, and coordinate with other agents. Unlike standard LLMs, agents can execute multi-step processes, access real-time clinical information, and integrate multiple data sources. There has been interest in using such agents for clinical and administrative tasks, however, there is limited knowledge on their performance and whether multi-agent systems function better than a single agent for healthcare tasks. PURPOSE: To evaluate the performance of AI agents in healthcare, compare AI agent systems vs. stand

Causal relationship between hypertension and cancer: a two‑sample bidirectional Mendelian randomization study.

BACKGROUND: Evidence from observational studies suggests that the association between hypertension and cancer is controversial, likely due to confounding factors and reverse causality. This study employs Mendelian randomization (MR) to investigate the causal relationship between hypertension and various cancers. METHODS: We conducted two‑sample bidirectional MR analyses using genetic variants associated with hypertension (484,598 participants) and 17 site-specific cancers from various genome-wide association studies (GWAS) including the GWAS Catalog, Integrative Epidemiology Unit OpenGWAS pro

Causal role of immune cells in generalized anxiety disorder: Mendelian randomization study.

BACKGROUND: Generalized anxiety disorder (GAD) is a prevalent emotional disorder that has received relatively little attention regarding its immunological basis. Recent years have seen the widespread use of high-density genetic markers such as SNPs or CNVs for genotyping, as well as the advancement of genome-wide association studies (GWAS) technologies, which have facilitated the understanding of immunological mechanisms underlying several major psychiatric disorders. Despite these advancements, the immunological basis of GAD remains poorly understood. In light of this, we aimed to explore the

A Mendelian randomization study investigating the causal relationships between inflammation and immunoglobulin A nephropathy.

BACKGROUND: Immunoglobulin A nephropathy (IgAN) is an autoimmune disease characterized by the production of galactose‑deficient IgA1 (Gd‑IgA1) and the deposition of immune complexes in the kidney. Exploring the landscape of immune dysregulation in IgAN is valuable for pathogenesis and disease treatment. We conducted Mendelian randomization (MR) to assess the causal correlations between inflammation and IgAN. METHODS: Based on available genetic datasets, we investigated potential causal links between inflammation and the risk of IgAN using two-sample MR. We used genome-wide association study (

Traditional oriental medicine for sensorineural hearing loss: Can ethnopharmacology contribute to potential drug discovery?

ETHNOPHARMACOLOGICAL RELEVANCE: In Traditional Oriental Medicine (TOM), the development of hearing pathologies is related to an inadequate nourishment of the ears by the kidney and other organs involved in regulation of bodily fluids and nutrients. Several herbal species have historically been prescribed for promoting the production of bodily fluids or as antiaging agents to treat deficiencies in hearing. AIM OF REVIEW: The prevalence of hearing loss has been increasing in the last decade and is projected to grow considerably in the coming years. Recently, several herbal-derived products pres

On the occasion of the centennial of the Nobel Prize in Physiology or Medicine, 1923: Nicolae C. Paulescu-between scientific creativity and political fanatism.

AIMS: Since the Nobel Prize in Physiology or Medicine was awarded in 1923 to FG Banting and JJR Macleod, many voices have been raised against this decision. The bitterest protest was that of the Romanian scientist Nicolae C. Paulescu. In 2002, The Romanian Academy of Sciences, the European Association for the Study of Diabetes (EASD) and the International Diabetes Federation (IDF) planned to hold a series of academic events the following year in Paris to acknowledge Paulescu's scientific merits in the discovery of the antidiabetic hormone. However, the initiative was cancelled in August 2003,
