The relationship of the femoral physis and the medial patellofemoral ligament in children: a cadaveric study.

BACKGROUND: Patellar dislocations are common in skeletally immature athletes, and the medial patellofemoral ligament (MPFL) is an important primary restraint to lateral patellar translation. The relationship between the MPFL femoral origin footprint and femoral physis is unclear. The purpose of this study was to evaluate the MPFL femoral origin footprint and its relationship to the femoral physis in skeletally immature anatomic specimens. METHODS: Six skeletally immature cadaver knee specimens were examined through gross dissection (group A: 1, 11, and 11 mo; and group B: 8, 10, and 11 y). Me

Dynamic arthroscopy in retropatellar cartilage defect: identifying trochlear ridge impingement and guiding towards mini-trochleoplasty.

Retropatellar (RP) chondral defects are commonly associated with trauma, sports-related injuries, or patellar dislocations. Effective management typically requires both cartilage repair and correction of underlying predisposing biomechanical factors. However, conventional imaging modalities may fail to detect subtle abnormalities. We describe a young female with persistent anterior knee pain in the absence of trauma or instability. Imaging studies, including radiographs and magnetic resonance imaging (MRI), confirmed an RP chondral defect, with no evidence of patella alta, malalignment or signi

The Rotaflex total knee replacement--a 5 year review.

We report the results of a retrospective analysis of 43 patients who received 56 Rotaflex total knee arthroplasties, with a mean follow-up of 55.7 months. The British Orthopaedic Association (BOA) knee assessment protocol was used in evaluating the clinical results. Two patients could not receive post-operative scores. In the remaining 54 knees, the mean pre-operative score was 25.6, improving to 30.8 post-operatively. Ten knees showed a decrease in knee score, two were unchanged and 42 improved. The greatest improvements were in pain relief and maximum flexion. Wound infection and dehiscences

Hip injuries in children and adolescents.

Hip injuries in children present a wide spectrum of problems. Frequently because of the severity of the trauma involved, other injuries may take precedence and may require modification of the usual approaches to treatment. However, certain precepts are essential to the successful treatment of these injuries and should be kept in mind regardless of the milieu in which they are found. In the child with a hip dislocation, the potential presence of an acetabular, femoral head, femoral shaft, patellar or tibial plateau fracture must always be considered. Specialized x-ray views are necessary for th

MR imaging in congenital lower limb deformities.

Treatment for children with congenital deformities of the lower extremities may vary, depending on the state of the unossified skeletal structures and surrounding soft tissues. The purpose of our study was to demonstrate the spectrum of the osteochondral and extrasosseous abnormalities as depicted with MR imaging. We retrospectively reviewed MR examinations of 13 limbs of ten children (aged 1 month-9 years, mean 2.1 years) with longitudinal and transverse deformities of the lower extremities. The lesions imaged were fibular hemimelia (n = 5), tibial hemimelia (n = 5), and congenital constricti

Does landmark selection affect the reliability of tibial tubercle-trochlear groove measurements using MRI?

BACKGROUND: A lateralized tibial tubercle is one potential cause of patellar instability. The tibial tubercle-trochlear groove (TT-TG) distance using CT is a reliable measure and considered the gold standard. Using MRI for this purpose has increased, although the reliability of doing so is not well studied. QUESTIONS/PURPOSES: We sought to (1) determine variability in the insertion of the patellar tendon relative to the tibial tubercle and whether this affects the measurement on MRI of the traditional TT-TG distance versus the functional patellar tendon-trochlear groove (PT-TG) distance, (2)

[Anatomy and biomechanics of the patellofemoral joint: physiological conditions and changes after total knee arthroplasty].

The patellofemoral joint constitutes a complex anatomical and functional entity. The tensile force of the quadriceps femoris muscle is transmitted through the patella and patellar ligament onto the tibial tuberosity. This particular three-dimensional arrangement increases the torsional moment acting on the knee joint. Dynamic alignment of the patella is determined by trochlear geometry and is supported by active muscular and passive connective tissue stabilizers. In addition to the retinaculum of the patella, the medial patellofemoral ligament is attracting increasing clinical attention. Multi

The crossing-sign angle and trochlear dysplasia length: New radiographic parameters to measure the dysplastic trochlea.

INTRODUCTION: Trochlear dysplasia is one of the most relevant anatomic factors predisposing patients to patellofemoral instability and its recurrence. This study introduces two novel radiographic measurements: the crossing-sign angle and the trochlear dysplasia length. METHODS: A concordance study was conducted in patients aged 10-40 years with at least one episode of patellofemoral dislocation and radiographic evidence of trochlear dysplasia on a true lateral knee radiograph. Three observers independently measured the crossing-sign angle, the trochlear dysplasia length, and bump height, as w

Radiological assessment of patellar instability: comparative analysis of patients with and without instability and the impact of patellar stabilisation surgery.

BACKGROUND: Patellar instability (PI) is a clinical diagnosis often complicated by symptom overlap with other knee pathologies and spontaneous relocation of the patella. Anatomical variations are key risk factors, highlighting the role of radiological investigations in assessing underlying pathology. Inconsistent measurement parameters and limited comparative data reduce diagnostic reliability. This study aimed to compare radiological parameters between patients with and without PI, and to assess how these parameters change following patellar stabilisation surgery. METHODS: This case-control

Developmental Morphology in Childhood Patellar Instability: Age-dependent Differences on Magnetic Resonance Imaging.

BACKGROUND: Magnetic resonance imaging (MRI) has been used to define pathologic morphology of patellar instability primarily in adults, but few studies have evaluated skeletally immature patients. The purpose of this study was to delineate differences in morphologic parameters between normal children and children with patellar instability. METHODS: A retrospective review of knee MRIs, ages 8 to 18 years, using a "normal" cohort without evidence of patella instability (normal group, NG) and a cohort with unstable patella (unstable group, UG). Thirty-four measurements were made on patellar char

The difficult primary total knee arthroplasty: a review.

Primary total knee arthroplasty (TKA) is a reliable procedure with reproducible long-term results. Nevertheless, there are conditions related to the type of patient or local conditions of the knee that can make it a difficult procedure. The most common scenarios that make it difficult are discussed in this review. These include patients with many previous operations and incisions, and those with severe coronal deformities, genu recurvatum, a stiff knee, extra-articular deformities and those who have previously undergone osteotomy around the knee and those with chronic dislocation of the patell

[Patella dislocation: an algorithm for diagnostic and treatment considering the rotation].

BACKGROUND: Patella dislocation and re-dislocation are common diseases. Although patella dislocation is frequent, it always poses a challenge concerning diagnostics and therapy. Mixed forms of pathological disorder in the patellofemoral joint make the analysis of the malalignment and the choice of the correct treatment more difficult. MATERIAL AND METHODS: By reviewing the available literature of the last few years and taking into account considerations concerning the malalignment, an overview of the necessary diagnostics can be given. Radiographs and MRI are essential parts of the diagnostic

The prevalence of canine patellar luxation in three centres. Clinical features and radiographic evidence of limb deviation.

The medical records of 155 dogs with patellar luxation (PL) from three different centres were analysed. Each case was classified according to the nature of its luxation and any concurrent orthopaedic conditions plus the age at diagnosis were also noted. Measurements relating to angle of inclination (AOI) of the femoral neck and medio-lateral bowing of the femur and tibia at the stifle were also recorded. The femoral and tibial data were compared to dogs with another orthopaedic condition in a case-control assessment. Labradors were most commonly affected (21%). Most luxations were medial (92%)
