A Novel Closure Technique for Complex Abdominal Wounds.

INTRODUCTION: Abdominal wound closure is a challenge in patients undergoing colorectal surgery with a complex history of multiple abdominopelvic operations. Loss of domain of the abdominal fascia because of prior laparotomies precludes the use of simple, everyday abdominal wound closure techniques. Furthermore, ongoing intra-abdominal sepsis, with or without a concurrent entero- or colocutaneous fistula, increases the risk of postoperative morbidity and mortality in this patient population. We propose an abdominal wound closure technique for patients with multiple previous complex operations a

Safe techniques for abdominal access and wound closure in laparoscopic surgery.

Laparoscopic surgery requiring tissue morcellation was performed on 66 consecutive patients, using open laparoscopy for primary entry and the SECURE cone technique for accessing the port of mechanical morcellation. Thirty-six patients had laparoscopic myomectomy and 30 had supracervical laparoscopic hysterectomy. In all instances, abdominal entry was accomplished under visual control through a minilaparotomy incision, or using telescopic visualisation. The surgical wound was closed in two layers. The first layer consisted of the fascia at the primary site and the bulk of the abdominal wall at

Simple and safe technique for closure of midline abdominal wound dehiscence.

PURPOSE: The aim of this study was to assess an alternative technique for closure of burst abdomen. METHOD: After reduction of the viscera and toilet of abdominal cavity with debridement of any necrotic tissues in the skin or fascia, the skin was undermined to expose the aponeurosis all-around the defect edges for at least 5-6 cm. The greater omentum is spread in the floor of the wound and fixed to the inner side of the defect. A piece of polypropylene mesh designed according to the size and shape of the defect and sutured to its external edge continuously. Another larger mesh is placed to co

Advanced Techniques in the Use of Negative Pressure Wound Therapy for Closure of Complex Neonatal Abdominal Wounds.

BACKGROUND: Negative pressure wound therapy (NPWT) has been described for closure of complex neonatal abdominal wounds, but advanced techniques for stoma or fistula control, skin protection, and the use of high pressure therapy not as well established. CASES: We identified neonatal patients at our institution who received NPWT for a complex abdominal wound, defined as a wound associated with a stoma or fistula with partial or complete dehiscence of the abdominal fascia or skin. We then reviewed techniques for decreasing wound contamination and protecting the newborn's skin. One patient had an

Temporary closure of open abdominal wounds by the modified sandwich-vacuum pack technique.

BACKGROUND: A 5-year experience with the modified sandwich-vacuum pack technique, using an opened 3-litre urological irrigation bag and continuous high-pressure suction, for temporary abdominal wall closure is presented. METHODS: The records of all patients who underwent temporary abdominal wall closure using this method from January 1996 to December 2000 were examined. RESULTS: The modified sandwich-vacuum pack was used 139 times in 55 patients. Forty patients sustained penetrating trauma while 15 patients sustained blunt trauma. The mean Injury Severity Score was 19 (range 9-34). Intra-abd

Modified sandwich vacuum pack technique for temporary closure of abdominal wounds: an african perspective.

INTRODUCTION: South Africa has very high levels of accidental trauma as well as interpersonal violence. There are more admissions for trauma in South Africa than for any other disease; therefore it can be regarded as the Number 1 disease in the country. Complex abdominal injuries are common, requiring specific management techniques. The aim is to document our experience with the Modified Sandwich Vacuum Pack technique for temporary closure of abdominal wounds. METHODS: After providing a short historical overview, we will demonstrate the technique which we carefully adapted over the last decad

[Results of abdominal wall closure using the 4:1 suture/wound lenght technique].

BACKGROUND: Laparotomy is a daily procedure for the general surgeon and its main complication is the formation of hernias. OBJECTIVE: To determine if the suture length to wound length ratio 4:1 for wall closure decreases the incidence of hernia. MATERIAL AND METHODS: Data from patients (n = 86) in whom abdominal wall closure was performed from August 2017 to January 2018 were prospectively reviewed. Patients who could not undergo adequate follow-up, those managed with open abdomen, or those with use of non-absorbable suture materials were excluded. 2 groups were formed: in one, the suture le

Tehran's Tie: A simple trick to fix the trocar to the abdominal wall and close the entry site of first trocar in laparoscopic surgery.

INTRODUCTION: Trocar dislodgment can occur frequently during minimally invasive surgery. Closing the entry port at the fascial layer after the end of a long laparoscopic surgery is time-consuming. Here, I introduce Tehran's Tie, a simple trick to overcome these problems. MATERIALS AND SURGICAL TECHNIQUE: After the first trocar site has been created with the open technique and before the first port is inserted, the fascial wound is grasped with a loop of nylon thread, similar to a transverse matrix suture. Then, the trocar is inserted through the loop into the abdominal cavity. One thread is t

Non-traumatic causes and the management of the open abdomen.

The open abdomen is increasingly used for the treatment and prevention of abdominal compartment syndrome. The leading non-traumatic conditions that may cause abdominal compartment syndrome requiring surgical decompression include secondary peritonitis, ruptured abdominal aortic aneurysm and severe acute pancreatitis. Patients may also end up with the open abdomen when the laparotomy wound cannot be closed without tension because of excessive visceral swelling. Also, surgical complications such as laparotomy wound dehiscence, may require temporary abdominal closure techniques. In critically ill

Total management of the open abdomen.

The management of complex abdominal problems with the 'open abdomen' (OA) technique has become a routine procedure in surgery. The number of cases treated with an OA has increased dramatically because of the popularisation of damage control for life-threatening conditions, recognition and treatment of intra-abdominal hypertension and abdominal compartment syndrome and new evidence regarding the management of severe intra-abdominal sepsis. Although OA has saved numerous lives and has addressed many problems related to the primary pathology, this technique is also associated with serious complic

Damage Control Laparotomy and Management of the Open Abdomen.

Management of the open abdomen has been used for decades by general surgeons. Techniques have evolved over those decades to improve control of infection, fluid loss, and improve the ability to close the abdomen to avoid hernia formation. The authors explore the history, indications, and techniques of open abdomen management in multiple settings. The most important considerations in open abdomen management include the reason for leaving the abdomen open, prevention and mitigation of ongoing organ dysfunction, and eventual plans for abdominal closure.

Open abdomen techniques - a herniologist's view.

Open abdomen, laparostomy, or temporary abdominal closure, represent a surgical strategy in which the abdominal cavity is intentionally left unclosed after laparotomy. This approach is indicated primarily in critically ill patients in whom primary abdominal wall closure would result in deterioration of intra-abdominal conditions or prevent adequate control of the underlying pathology. This group mainly includes patients with generalized peritonitis, severe acute pancreatitis, major abdominal trauma, or significant visceral edema. A second group comprises patients undergoing decompressive lapar

Results of the Smead-Jones technique of closure of vertical midline incisions for emergency laparotomies--a prospective study of 403 patients.

The early and late results of the Smead-Jones (SJ) technique of closure of emergency vertical midline laparotomies was compared with other conventional methods of closure such as anatomical repair (AR), mass closure (MC) and single layer (SL) closure. Four Hundred and Three patients who underwent emergency laparotomy were studied prospectively. The results of SJ closure of laparotomy in them were compared with other techniques of abdominal closure. Ninety percent of patients with SJ were in wound class IV and at high risk of wound complications. It was seen that the overall infection rate for

[Surgical wound closure in advanced peritonitis].

To date, mortality in widespread peritonitis is still high (15-20%) and increased up to 70-80% in case of septic shock. Surgeons actively discuss wound closure technique in these patients considering intraoperative findings and severity of illness. The authors present scientific data and opinions of national and foreign surgeons regarding the methods of laparotomy closure. There are still no generally accepted criteria for choosing the method of laparotomy closure in secondary widespread peritonitis. Indications and clinical efficacy of each procedure require additional research.
