Menorrhagia is a frequent gynecological problem that is clinically described as excessive blood loss of 80 mL per menstrual cycle. It has the potential to lower their quality of life and to induce anemia. Medical therapy has typically been the first line of treatment; however, it is frequently ineffectual. Hysterectomy, on the other hand, is clearly 100 percent effective in stopping bleeding, but it is more expensive and can cause serious problems. So, the endometrial ablation is preferred when the endometrial layer is destroyed or removed during the procedure. To "ablate" (remove) the endomet

BACKGROUND: Pharmacological management of arrhythmias is not curative, is sometimes difficult, and may be associated with significant morbidity.
OBJECTIVE: This article discusses the place of catheter ablation in the treatment of arrhythmias.
DISCUSSION: Supraventricular and typical atrial flutter can be cured with catheter ablation in approximately 95% of cases with very low complication rates. Catheter ablation can successfully restore and maintain sinus rhythm in patients with atrial fibrillation (AF) and should be considered in symptomatic patients not controlled on medical therapy befor

In addition to the basic techniques used in radiofrequency ablation of thyroid nodules, a variety of more advanced techniques have been developed. To address hyper-vascular nodules, vascular ablative techniques have been described. Protective hydro-dissection can be used around the carotid artery and the trachea to improve safety. Rescue hydro dissection can be used to reverse nerve injury. Additionally, several new ablation devices show promise at increasing efficacy and safety. These techniques and devices will be reviewed to provide an understanding of these advanced techniques and novel de

Ultrasound-guided ablation techniques have been increasingly introduced into routine treatment of thyroid lesions as a complement to existing surgical therapies and radioiodine treatment. In cystic or predominantly cystic lesions instillation therapy (ethanol/polidocanol ablation) has yielded good results. Novel thermal ablation techniques, including radiofrequency ablation (RFA), microwave ablation (MWA), laser ablation (LA) and high intensity focused ultrasound (HIFU), induce irreversible cellular effects by locally applying temperatures ≥ 60 °C to the targeted thyroid region. Lesions causin

BACKGROUND: The incidence of proarrhythmia induced by ablation for atrial fibrillation (AF) is not entirely known. We describe the incidence and management of atrial arrhythmias occurring after various techniques for the ablative therapy of AF.
METHODS: Ninety-four patients with paroxysmal AF underwent ostial pulmonary vein (PV) ablation (n=54) or circumferential ablation around the PV ostia (n=40).
RESULTS: Atrial tachycardia or flutter was detected during the first 6 months after AF ablation in 10 patients. Atrial arrhythmia was more common among patients who underwent circumferential abla

Transthoracic epicardial catheter ablation is a useful supplemental or even preferred strategy to eliminate cardiac arrhythmias in the electrophysiology laboratory. The indication for this technique has extended to a diverse range of cardiac arrhythmias, including scar-related ventricular tachycardia (VT), idiopathic VTs, accessory pathways, atrial tachycardias, inappropriate sinus tachycardia, and atrial fibrillation, as the epicardial substrates of these tachyarrhythmias have become increasingly recognized. When endocardial ablation and epicardial ablation through the cardiac veins are unsuc

Catheter ablation is an experimental procedure that may prove useful for patients with drug-refractory supraventricular tachycardia. Current techniques involve the use of high-energy direct-current shocks, delivered via electrode catheters. These techniques have been used most commonly for ablating the atrioventricular (AV) junction in patients with supraventricular tachycardia. Such ablation may be of value in a subset of patients with the Wolff-Parkinson-White syndrome. In addition, limited data suggest that direct catheter ablation of posteroseptal accessory pathways may prove safe and effe

Atrial fibrillation remains the most common arrhythmia worldwide, with pulmonary vein isolation (PVI) being an essential component in the treatment of this arrhythmia. In view of the close proximity of the oesophagus with the posterior wall of the left atrium, oesophageal injury prevention has become a major concern during PVI procedures. Oesophageal changes varying from erythema to fistulas have been reported, with atrio-oesophageal fistulas being the most feared as they are associated with major morbidity and mortality. This review article provides a detailed description of the risk factors

BACKGROUND: Heavy menstrual bleeding (HMB) is a significant health problem in premenopausal women; it can reduce their quality of life and cause anaemia. First-line therapy has traditionally been medical therapy but this is frequently ineffective. On the other hand, hysterectomy is obviously 100% effective in stopping bleeding but is more costly and can cause severe complications. Endometrial ablation is less invasive and preserves the uterus, although long-term studies have found that the costs of ablative surgery approach the cost of hysterectomy due to the requirement for repeat procedures.

Chronic low back pain represents a leading cause of global disability, but can often be complex in etiology, with multiple pain generators contributing to patient discomfort. Anterior column low back pain involves the vertebral bodies and discs, with the vertebral body endplates now known to be a significant pain generator. Vertebrogenic low back pain secondary to irritation of the basivertebral nerve presents as axial discomfort, worse with forward flexion, and with concomitant Type I or II Modic changes on MRI. Minimally invasive basivertebral nerve radiofrequency ablation has emerged as an

BACKGROUND: Heavy menstrual bleeding (HMB) is a significant health problem in premenopausal women; it can reduce their quality of life and can cause social disruption and physical problems such as iron deficiency anaemia. First-line treatment has traditionally consisted of medical therapy (hormonal and non-hormonal), but this is not always successful in reducing menstrual bleeding to acceptable levels. Hysterectomy is a definitive treatment, but it is more costly and carries some risk. Endometrial ablation may be an alternative to hysterectomy that preserves the uterus. Many techniques have be

BACKGROUND: About 15,000 persons receive the diagnosis of kidney cancer in Germany every year. Surgical resection is the standard treatment for locally confined tumors, but minimally invasive thermoablative techniques are increasingly being used as well.
METHODS: This article is based on publications retrieved by a selective literature search in PubMed regarding the thermoablative techniques now used in clinical practice, with particular attention to radiofrequency ablation (RFA) and cryoablation (CA).
RESULTS: RFA and CA are suitable for patients who cannot undergo surgery because of comorb

Innovations in radiofrequency (RF) ablation and nonablative techniques have led to significant advances in addressing complex arrhythmogenic substrates for a variety of cardiac arrhythmias. Anatomical challenges, deep substrate, and mid-myocardial locations may pose difficulties and decrease success rates using routine methods. In this review, we provide an update on novel RF technology and techniques including (a) high-power, low-duration ablation, (b) ablation facilitated by low-ionic irrigant, and (c) bipolar ablation. In addition, we review emerging technologies including electroporation,

When available, state of the art noninvasive localization studies should be utilized routinely in previously unexplored patients for localizing parathyroid pathology, even when exceptional surgical experience exists. These studies can both minimize the 3 to 20% incidence of missed pathology and promote an approach of limited neck exploration with consequent lowering of morbidity, complications, and costs. Choice of imaging modalities for localizing these small masses is largely dependent on the level of state of the art of available equipment, the interest and experience of the performing phys

Pulsed field ablation (PFA) is a novel method of cardiac ablation in which electrical fields create microscopic pores in cardiomyocyte cell membranes, resulting in cell death through a process of electroporation. Unlike thermal technologies, PFA has preferential cardiomyocyte selectivity, thereby reducing the risk of collateral damage to surrounding structures. The PulseSelect PFA system (Medtronic Inc., Minneapolis, MN, USA) is a 9-electrode circular ablation catheter with a central guidewire used for atrial fibrillation ablation. Currently, there is no consensus on workflow or best practice
