Interprofessional advanced access - a quality improvement protocol for expanding access to primary care services.

BACKGROUND: The Advanced Access (AA) Model has shown considerable success in improving timely access for patients in primary care settings. As a result, a majority of family physicians have implemented AA in their organizations over the last decade. However, despite its widespread use, few professionals other than physicians and nurse practitioners have implemented the model. Among those who have integrated it to their practice, a wide variation in the level of implementation is observed, suggesting a need to support primary care teams in continuous improvement with AA implementation. This qua

Access to Primary Care Clinics for Patients With Chronic Pain Receiving Opioids.

IMPORTANCE: Opioid-prescribing policies and guidelines aimed at reducing inappropriate opioid prescribing may lead physicians to stop prescribing opioids. Patients may thus encounter difficulties finding primary care practitioners willing to care for them if they take opioids. OBJECTIVES: To assess practitioner willingness to accept and continue prescribing opioids to new patients with pain and whether this willingness differs across payer types. DESIGN, SETTING, AND PARTICIPANTS: This survey study used a simulated patient call audit method. A brief telephone survey was administered to all c

Access to primary care in adults in a provincial correctional facility in Ontario.

BACKGROUND: Little is known about access to primary care either prior to or following incarceration in Canada. International data demonstrate that the health of people in prisons and jails is poor, and access to primary care in the community may be inadequate for incarcerated persons. We aimed to describe the primary care experience of adults in custody in a provincial correctional facility in Ontario in the 12 months prior to admission. METHODS: We conducted a written survey, and invited all persons in the institution to participate, excluding those in segregation. RESULTS: One hundred and

Repositioning Primary Health Care in Sierra Leone: From Geographic Access to Effective Coverage.

Primary health care (PHC) is central to Sierra Leone's efforts to achieve universal health coverage. Despite substantial expansion of the peripheral health unit network, improvements in geographic access have not consistently translated into better health outcomes. This viewpoint argues that Sierra Leone must shift its focus from physical access to effective coverage, defined as the proportion of people who receive care of sufficient quality to produce meaningful health gains. Key challenges include persistent out-of-pocket expenditure, shortages of salaried health workers, weak service readin

Young adults' access to mental health care: integrating the Primary Care Behavioral Health model into routine care in Sweden.

BACKGROUND: Mental health issues among young adults are increasing. Swedish data indicate a three- to fourfold increase in symptoms of anxiety and distress over the past two decades. Access to mental health services in primary care is often hindered by long wait times and resource limitations. The Primary Care Behavioral Health (PCBH) model, characterized by brief, team-based interventions delivered by behavioural health consultants (BHCs), offers a promising approach to improve access. However, limited evidence exists on its impact on young adult populations. METHODS: A pragmatic stepped-wed

Prospective Cohort of Referrals to a Cancer of Unknown Primary Clinic, including Direct Access from Primary Care.

AIMS: The UK National Health Service has well-developed site-specific referral pathways for patients with suspected cancer, but historically there has been inequality of access for patients with suspected Metastatic malignant disease of Unknown primary Origin (MUO). The Brighton cancer of unknown primary (CUP) clinic covers a population of about 650 000. As well as 'in-house' referrals, direct general practitioner referrals are also accepted (since 2015), aiming to shorten the diagnostic pathway and improve patient support. We present data from the first 3 years of activity. MATERIALS AND MET
