Blood, urine, and tissue specimens were received from 377 Federal Aviation Administration (FAA) aviation fatalities during fiscal year 1989. Carbon monoxide at less than 10% saturation was found in 94% of the cases, and cyanide at less than 0.5 mg/L was found in 96% of the cases. Ethanol at greater than 10 mg/dL was found in 14.8% of the cases, but only 4.5% were determined to be due to ethanol ingestion from toxicological findings. Excluding nicotine and ethanol, 12.6% of the cases were positive for one or more drugs. Acetaminophen and salicylate were the most frequently found drugs. Cannabin

INTRODUCTION: Spatial disorientation (SD) is a well-known risk factor across aviation operations but is a particular risk within general aviation (GA). While previous work has examined historical rates of SD related accidents in GA, modern analyses of SD accidents that account for overall changes to the GA environment are limited.
METHODS: The current analysis investigates recent trends from 2003 to 2021 in SD GA fatal accidents and compares these trends to historical analyses, while incorporating context about changes to the overall GA landscape.
RESULTS: While fatal SD GA accidents rates a

INTRODUCTION: General aviation (GA), mainly comprised of light (≤12,500 lb) aircraft, maintains an inferior safety record compared with air carriers. To improve safety, aeronautical decision-making (ADM) practices have been advocated to GA pilots since 1991. Herein, we determined the extent to which GA pilots disregard such practices.METHODS: Fatal accidents (1991-2019) involving private pilots (PPLs) in single-engine airplanes were identified (N = 1481) from the National Transportation Safety Board AccessR database. Of these, deficient go/no-go and in-flight ADM-related mishaps were scored us

BACKGROUND: In aviation accident investigations, control pattern injuries (CPIs) have been useful for accident reconstruction and for determining who was in control of the aircraft at the time of impact. Recently, CPIs have been considered nonspecific. This study compares CPIs suffered by pilots and passengers in fatal fixed-wing general aviation (GA) accidents.
METHODS: Accident data were collected from the Federal Aviation Administration's Civil Aerospace Medical Institute (FAA-CAMI) and National Transportation Safety Board (NTSB) accident reports for the year 2010. Pilot and passenger data

INTRODUCTION: It is important in aviation accident investigations to determine if a fire occurred during flight or after the crash and to establish the source(s) of the toxic gases.
METHODS: Bio-specimens from aviation accident fatalities are submitted to CAMI for analyses. In blood, CO is analyzed as carboxyhemoglobin (COHb) and hydrogen cyanide as cyanide (CN-). Analytical data were stored in a database, and this database was searched for the period of 1990-2002 for the presence of COHb and CN in the submitted cases.
RESULTS: Out of 5945 cases, there were 223 (4%) cases wherein COHb was >

So far no national or regional studies have been published in Germany regarding the number of fatal aviation accidents and results of autopsy findings. Therefore, we evaluated all fatal aviation accidents occurring in Lower Saxony from 1979 to 1996. A total of 96 aviation accidents occurred in this period involving 73 aeroplanes. The crashes resulted in the death of 154 people ranging in age from 19 to 68 years. The greatest number of victims in a single crash of an aircraft was (n=7). Other types of fatal accidents were crashes of aircraft and helicopter while on the ground (n=5), hot-air bal

Toxicological findings in 1,345 fatal general aviation accidents from fiscal year 1968 through 1974 are summarized. Methods used in examination of specimens for alcohol, drugs, carbon monoxide, cyanide, and cholinesterase activity are described. Blood ethanol levels in excess of 0.050% were found in 117 of the 1,345 pilots (8.7%). Drugs were found in 16 cases (1.2%). These and other toxicological findings indicate that in more than 40% of the cases, information worthy of consideration in developing the human-factors history of an accident or the cause of death from survivable crashes was obtai

The problem of flight safety and aviation injury were considered, data on aviation accidents in Civil aviation were shown, the main causes of aviation accidents were presented: technical failure, difficult meteorological conditions, deficiencies in flight organization and «human factor». The conditional classification of aviation injury was presented. Detection of primary injuries is especially important for aeromedicine (head impact on dashboard, bone fracture at the ankle, «loss of vital space», laceration between 1st and 2nd fingers from the control wheel), that determine the pattern of pil

Prevalence of drugs in fatally injured obese pilots involved in aviation accidents has not been evaluated. Therefore, toxicological findings in such pilots (body mass index ≥30 kg/m(2) ) were examined in a data set derived from the Civil Aerospace Medical Institute's (CAMI's) Scientific Information System for 1990-2005. Aeromedical histories of these aviators were retrieved from the CAMI medical certification and toxicology databases, and the cause/factors in the related accidents from the National Transportation Safety Board's database. In 311 of the 889 pilots, carbon monoxide, cyanide, etha

INTRODUCTION: The number of aircraft accidents remains on a constant level since the late 1990s. Routine analysis in detail of the causative factors is not carried out in Germany. The analysis of flight mishaps has been demonstrated to be an important basis for flight safety. The Human Factors Analysis and Classification System (HFACS) model is best suitable for aircraft accident analysis. The aim of this study was to classify aircraft accidents in the General Aviation (GA) of Germany according to the HFACS model and to figure out the underlying causes.
MATERIAL AND METHODS: The analysis was

INTRODUCTION: Selective serotonin reuptake inhibitors (SSRIs) are popularly prescribed for treating depression. With a few exceptions, these psychotropic medications are not approved by aeromedical regulatory authorities for use by aviators. Since SSRIs have the potential for impairing performance and causing drug-drug interactions, the prevalence of SSRIs in pilot fatalities of civil aviation accidents was evaluated.
METHODS: Postmortem samples from pilots involved in fatal civil aircraft accidents are submitted to the Civil Aerospace Medical Institute (CAMI) for toxicological evaluation. Fi

INTRODUCTION: Approximately 54% of civilian pilots rely on ophthalmic lenses to correct defective vision in order to maintain a valid airman medical certificate. This report reviews aviation accidents and incidents where the use or misuse of ophthalmic devices was considered to have been a contributing factor in the mishap.
METHODS: The NTSB's Aviation Accident/Incident Database and the FAA's Incident Data System were queried for terms related to ophthalmic devices. All reports were reviewed to determine whether an ophthalmic device was a factor in the mishap and then stratified based on the

The National Institute on Drug Abuse (NIDA) and the Office of National Drug Control Policy (ONDCP) reported a 1.5-fold increase in the delta-9-tetrahydrocannabinol (THC) content of street cannabis seizures from 1997 to 2001 versus 2002 to 2006. This study was conducted to compare the changes, over those years, in blood and urine cannabinoid concentrations with the potency of THC reported in the cannabis plant. Cannabinoids were screened using radioimmunoassay (RIA) for blood and fluorescence polarization immunoassay (FPIA) for urine and confirmed using GC/MS. A total of 95 individuals were fou

INTRODUCTION: First-generation H1-receptor antagonists are popularly used for alleviating allergy and cold symptoms, but these antihistaminics cause drowsiness and sedation. Such side effects could impair performance and, thus, could be the cause or a factor in accidents. Therefore, the prevalence of these antagonists was evaluated in aviation accident pilot fatalities.
METHODS: The Civil Aerospace Medical Institute's (CAMI's) Toxicology Database was examined for the presence of the first-generation antihistamines in pilot fatalities of civil aircraft accidents that occurred during a 16-yr (1

Paroxetine is a selective serotonin reuptake inhibitor commonly prescribed for the treatment of depression, obsessive-compulsive disorder, panic disorder, social anxiety disorder and post-traumatic stress disorder. While the use of paroxetine is considered relatively safe, negative side effects, including nausea, drowsiness, insomnia and dizziness, can adversely affect a pilot's ability to safely operate an aircraft. The use of paroxetine may increase suicidal behavior and suicidal ideation. When relying on postmortem specimens for toxicological evaluation, a general understanding of drug dist

Sertraline (Zoloft) is a selective serotonin reuptake inhibitor that is a commonly prescribed drug for the treatment of depression, obsessive-compulsive disorder, panic disorder, social anxiety disorder, premenstrual dysphoric disorder and post-traumatic stress disorder. Although the use of sertraline is relatively safe, certain side effects may negatively affect a pilot's performance and become a factor in an aviation accident. The authors' laboratory investigated the distribution of sertraline and its primary metabolite, desmethylsertraline, in various postmortem tissues and fluids obtained

INTRODUCTION: Little research has been done on ultralight aviation accidents. A better understanding of accident factors allows for better development of preventive measures. This study analyzes ultralight accidents with respect to significant factors related to active and latent failures, as referred to in Reason's Swiss cheese model of human error.
METHODS: There were 66 National Transportation Safety Board (NTSB) ultralight accident reports that were examined from 1985-2004. Contextual information about active and latent failures was identified, compiled, and cross-referenced with pilot, a

Analyses of the accident experience of pilots who were monocular, did not meet vision standards, had color vision defects and no operational restrictions, or wore contact lenses have shown higher-than-expected accident experience in previous studies. However, no causal role had been assigned. In the present study of accidents during 1979, the pilots with aphakia and artificial lens implants and the total eye pathology population had significantly higher accident rates, but the monocular pilots did not. Again, no causal role had been ascribe. There are still unresolved questions about the consi

Low-altitude aviation accidents can become fatal when pilots have little time or altitude to recover after an abnormal event. Fatal outcomes, however, are rarely explained by altitude alone: they often depend on how human, aircraft, environmental, and operational factors combine within the accident sequence. This study asks whether the structure of these documented factor combinations helps explain injury severity in U.S. low-altitude aviation accidents. The data comprise 11,639 low-altitude accidents with completed Findings investigations, drawn from the National Transportation Safety Board (

Sometimes it is difficult to differentiate between injuries and drowning as a cause of death in aircraft-accident fatalities. There are certain anatomic and chemical changes that are compatible with drowning, but there is no single change that is diagnostic of drowning. Drowning is a diagnosis that can be made only after all other causes of death have been excluded. Only after a complete review of the circumstances surrounding the accident and the performance of a thorough autopsy can a correct diagnosis be made when there is a possibility of drowning.
