Improving preterm ophthalmologic care in the era of accountable care organizations.

OBJECTIVES To understand retinopathy of prematurity (ROP) follow-up care for preterm very low-birth-weight infants (VLBW; <1500 g) in the context of the chronic care model and identify opportunities for improvement under accountable care organizations. METHODS We conducted focus groups and interviews with parents (N = 47) of VLBW infants and interviews with neonatal intensive care unit and ophthalmologic providers (N = 28) at 6 sites in Massachusetts and South Carolina. Themes are reported according to consolidated criteria for reporting qualitative research guidelines. RESULTS Respondents

Do Medicare Accountable Care Organizations Reduce Disparities After Spinal Fracture?

BACKGROUND: National changes in health care disparities within the setting of trauma care have not been examined within Accountable Care Organizations (ACOs) or non-ACOs. We sought to examine the impact of ACOs on post-treatment outcomes (in-hospital mortality, 90-day complications, and readmissions), as well as surgical intervention among whites and nonwhites treated for spinal fractures. MATERIALS AND METHODS: We identified all beneficiaries treated for spinal fractures between 2009 and 2014 using national Medicare fee for service claims data. Claims were used to identify sociodemographic a

Medicare Accountable Care Organizations: Clinical Performance for Patients With Heart Failure.

OBJECTIVES: Objective: To understand factors that distinguish higher-performing accountable care organizations (ACOs) in the Medicare Shared Savings Program from those with average levels of clinical performance for patients with heart failure (HF), a common and complex condition. STUDY DESIGN: We used a 2-step design. First, we measured ACOs' clinical performance using Medicare data to calculate risk-standardized admission rates over time (2014-2017) for patients with HF. Next, we conducted 16 videoconference interviews with key informants in 2 ACOs that were average performers on this measu

Physician Participation in Medicare Accountable Care Organizations and Spillovers in Commercial Spending.

IMPORTANCE: The benefits of public payment policy may extend to private populations through "spillover" effects. If cost-saving efforts in Medicare also reduce costs among commercially insured patients, Medicare payment systems could be a versatile policy tool in future reform efforts. OBJECTIVES: To determine whether physicians who participated in a Medicare Accountable Care Organization (ACO) reduced spending among their commercial patients. DESIGN: This was a retrospective, longitudinal study which was conducted using Blue Cross Blue Shield of Michigan (BCBSM) claims data from 2010 to 201

Changes in healthcare delivery following spinal fracture in Medicare Accountable Care Organizations.

BACKGROUND CONTEXT: Accountable Care Organizations (ACOs) were designed to reduce healthcare costs while simultaneously improving quality. Given that the success of ACOs is predicated on controlling costs, concerns have been expressed that patients could be adversely affected through restricted access to surgery, including in the context of spine fracture care. PURPOSE: Evaluate the impact of Medicare ACO formation on the utilization of surgery and outcomes following spinal fractures. STUDY DESIGN: Retrospective review of Medicare claims (2009-2014). PATIENT SAMPLE: Patients treated for spi

Changes in the Use of Lumbar Arthrodesis Procedures Within Accountable Care Organizations.

STUDY DESIGN: A retrospective review of Medicare claims data (2009-2014). OBJECTIVE: The aim of this study was to evaluate changes in the use of lumbar fusion procedures following the formation of Accountable Care Organizations (ACOs). SUMMARY OF BACKGROUND DATA: Within surgical care afforded by ACOs, savings are thought to be realized by improved care coordination as well as reductions in the use of preference-sensitive procedures such as lumbar fusion. METHODS: We queried fee-for-service claims for patients enrolled in Medicare Part A and B, identifying patients who received lumbar spine

Characteristics of accountable care organizations offering methadone to patients with opioid use disorder.

Understanding whether organizations with Medicare and Medicaid accountable care organization (ACO) contracts offer methadone provides important context about how organizations invested in payment and delivery system reform address the needs of patients with substance use disorders. We used data from the 2021-2022 National Survey of Accountable Care Organizations to assess whether organizations with ACO contracts, which are held accountable for the cost and quality of care for an assigned patient population, offered methadone to patients with opioid use disorder (OUD), and the organizational an

Cancer spending and accountable care organizations: Evidence from the Physician Group Practice Demonstration.

BackgroundAlthough accountable care organizations (ACOs) are rapidly being deployed in Medicare, little is known about how the model might affect high-risk, high cost groups such as cancer patients. The Physician Group Practice Demonstration, which ran from 2005 to 2010 in 10 physician groups, provides the best current evidence on the likely effectiveness of accountable care organizations for Medicare beneficiaries. Changes in cancer treatment and spending under this program may be indicative of cancer treatment under ACO payment reform.MethodsUsing Medicare fee-for-service claims data, regres

Impact of accountable care organizations on acute cholecystitis outcomes in the Rio Grande Valley.

BACKGROUND: Accountable care organizations through the Affordable Care Act are to improve Medicare beneficiaries' health while reducing costs. We hypothesize that this model may shift care, disease burden, and costs to nonaffiliated hospital facilities in patients with acute cholecystitis. METHODS: A retrospective difference-in-differences analysis was performed to compare severity, postoperative complications, diagnostic modality, length of stay, and costs in patients with acute cholecystitis from a post-accountable care organization implementation period (January 2014 through December 2015)

Champion-Led Deprescribing for Persons with Dementia in Primary Care: A Qualitative Study in Accountable Care Organizations.

BACKGROUND: Clinical champions are a known strategy for implementing evidence-based practices; however, their application in de-implementing potentially inappropriate medications (PIMs) among persons with dementia is underexplored. We conducted a pragmatic cluster-randomized clinical trial of a champion-led deprescribing intervention in accountable care organization (ACO) primary care settings. OBJECTIVE: To (1) understand clinical champions' perspectives of their deprescribing projects and (2) identify key contextual factors that influenced champions' deprescribing PIMs for persons with deme

Health Care Spending And Use Among People Experiencing Unstable Housing In The Era Of Accountable Care Organizations.

Provider organizations are increasingly held accountable for health care spending in vulnerable populations. Longitudinal data on health care spending and use among people experiencing episodes of homelessness could inform the design of alternative payment models. We used Medicaid claims data to analyze spending and use among 402 people who were continuously enrolled in the Boston Health Care for the Homeless Program (BHCHP) from 2013 through 2015, compared to spending and use among 18,638 people who were continuously enrolled in Massachusetts Medicaid with no evidence of experiencing homeless

Impact of Medicare Accountable Care Organizations on surgical and postoperative expenditures among patients with gastrointestinal cancer.

BACKGROUND: The Medicare Shared Savings Program Accountable Care Organization is one of the largest Medicare Accountable Care Organization programs designed to promote value-based care. We sought to investigate whether participation in the Medicare Shared Savings Program Accountable Care Organization program was associated with lower surgical and postoperative care spending related to gastrointestinal cancer surgery. METHODS: Individuals who underwent gastrointestinal cancer surgery between 2016 and 2020 were identified from the Medicare database. Difference-in-differences analysis was used t

Availability of Medication for Opioid Use Disorder Among Accountable Care Organizations: Evidence From a National Survey.

OBJECTIVE: This report aimed to assess how accountable care organizations (ACOs) addressed ongoing opioid use disorder treatment needs over time. METHODS: Responses from the 2018 (N=308 organizations) and 2022 (N=276) National Survey of Accountable Care Organizations (response rate=55% in both years) were used to examine changes in availability of medication for opioid use disorder (MOUD) among ACOs with Medicare and Medicaid contracts. RESULTS: The percentage of respondents offering at least one MOUD grew from 39% in 2018 to 52% in 2022 (p<0.01). MOUDs were more likely to be available in 20

The Effect of Medicare Accountable Care Organizations on Early and Late Payments for Cardiovascular Disease Episodes.

BACKGROUND: Initial evaluations of the Pioneer and Shared Savings Programs have shown modest savings associated with care receipt in a Medicare accountable care organization (ACO). Whether these savings are affected by disease chronicity and the mechanisms through which they occur are unclear. In this context, we examined the association between Medicare ACO implementation and episode spending for 2 different cardiovascular conditions. METHODS AND RESULTS: We analyzed a 20% sample of national Medicare data, identifying fee-for-service beneficiaries aged ≥65 years admitted for acute myocardial

Medicare Accountable Care Organizations Are Not Associated With Reductions in the Use of Low-Value Coronary Revascularization.

BACKGROUND: Because specialty care accounts for half of Medicare expenditures, improving its value is critical to the success of Medicare accountable care organizations (ACOs) in curbing spending growth. However, whether ACOs have reduced low-value specialty care without compromising use of high-value services remains unknown. METHODS AND RESULTS: Using national Medicare data, we identified 2 cohorts: beneficiaries for whom the value of coronary revascularization is lower (those with ischemic heart disease without angina, congestive heart failure, or recent admission for acute myocardial infa

Linking Practice Adoption of Patient Engagement Strategies and Relational Coordination to Patient-Reported Outcomes in Accountable Care Organizations.

UNLABELLED: Policy Points Accountable care organizations (ACOs) have incentives to promote the adoption of patient engagement strategies such as shared decision making and self-management support programs to improve patient outcomes and contain health care costs. High adoption of patient engagement strategies among ACO-affiliated practices did not improve patient-reported outcomes (PROs) of physical, emotional, and social function among adult patients with diabetes and/or cardiovascular disease over a one-year time frame, likely because implementing these strategies requires extensive clinicia

Accountable Care Organization Participation and Cardiovascular Care Quality.

IMPORTANCE: The Medicare Shared Savings Program (MSSP) was introduced in 2012 to improve care quality and lower costs to Medicare. Under this program, accountable care organizations (ACOs) assumed responsibility for costs and care quality for a group of Medicare beneficiaries. OBJECTIVE: To compare changes in quality measures for patients at outpatient cardiology practices before and after their participation in a Medicare Shared Savings Program ACO. DESIGN, SETTING, AND PARTICIPANTS: This pre-post cohort study comparing quality prior to and after ACO participation evaluated the MSSP at 83 A
