Quantitative analysis of aconitine in body fluids in a case of aconitine poisoning.

Aconitine belongs to the Aconitum alkaloids and is a natural toxic substance. Aconitine has been used as a traditional medicine in East Asian culture. Today, aconitine is still in use with or without a prescription, in the Republic of Korea. Here we present a case report of accidental death due to acute aconitine poisoning. An 81-year-old woman ingested liquid that had been heat extracted from the root of the Aconitum plant; she presented to the emergency room 1 h after ingestion. Her electrocardiogram showed irregular ventricular arrhythmias including ventricular tachycardia; she progressed t

An enzyme-linked immunosorbent assay for aconitine-type alkaloids using an anti-aconitine monoclonal antibody.

3-Succinylaconitine was conjugated with bovine serum albumin (BSA) for use as an immunogen for the preparation of a monoclonal antibody (MAb) against aconitine (Aco). Splenocytes from mice immunized with the Aco-BSA conjugate were fused with an aminopterin-sensitive mouse myeloma cell line, P3-X63-Ag8-653, and a hybridoma secreting a MAb against Aco was successfully obtained. The MAb cross-reacted with mesaconitine, hypaconitine and jesaconitine, which are Aco-type alkaloids, but not with any other compounds examined. The full measurement range of an enzyme-linked immunosorbent assay (ELISA) d

Reversible intraventricular conduction defect in aconitine poisoning.

Chinese medicine plays an important role in providing medical care for people in countries with large Chinese communities, including Hong Kong. The aconite herb is one of the commonly-prescribed ingredients for various clinical problems. However, due to its narrow therapeutic index, toxicities are not uncommonly encountered, including life-threatening cardiac arrhythmias like ventricular arrhythmias. We report a 57-year-old woman with reversible intraventricular conduction defect attributed to the use of processed Fuzi.

[Aconitine ameliorates cardiomyocyte hypertrophy induced by angiotensin Ⅱ].

This paper was aimed to investigate the inhibitory effect of aconitine(AC) on angiotensin Ⅱ(Ang Ⅱ)-induced H9 c2 cell hypertrophy and explore its mechanism of action. The model of hypertrophy was induced by Ang Ⅱ(1×10-6 mol·L-1),and cardiomyocytes were incubated with different concentrations of AC. Western blot was used to quantify the protein expression levels of atrial natriuretic peptide(ANP),brain natriuretic peptide(BNP),β-myosin heavy chain(β-MHC),and α-smooth muscle actin(α-SMA). Real-time quantitative PCR(qRT-PCR) was used to quantify the mRNA expression levels of cardiac hypertrophic

Aconitine: A potential novel treatment for systemic lupus erythematosus.

BACKGROUND: Aconitum plants have been widely used in China for thousands of years. Recent evidences indicate that aconitine, the main active ingredient of Aconitum, has immunomodulatory properties that might be useful for treating autoimmune diseases, such as rheumatoid arthritis. In this study, we conducted a pilot study to explore the effect and mechanisms of aconitine on the treatment of systemic lupus erythematosus. METHODS: A pristane-induced murine model was used. The pristane-induced mice were treated with aconitine (25, 75 μg kg-1 d-1, po) for 9 weeks. Every three weeks, proteinuria w

Aconitine accidental poisoning: two case reports and literature review.

INTRODUCTION: Chinese Herbal Medicine (CHM) is widely integrated into clinical practice across Asia and other regions for disease treatment and health maintenance. Aconitine, a potent diterpenoid alkaloid derived from plants of the Aconitum genus (e.g., Aconitum carmichaelii and Aconitum kusnezoffii), exhibits significant pharmacological activity but also considerable toxicity. While properly processed Aconitum herbs are prescribed for their analgesic and anti-rheumatic properties, inadequate detoxification during processing or improper clinical use can lead to accidental aconitine poisoning.

Magnesium Sulfate Combination Therapy for Aconitine-Induced Electrical Storm.

We present the case of a 74-year-old man who developed 28 episodes of electrical storm secondary to aconitine poisoning from homemade herbal wine consumption, followed by complete recovery through 28 electrical cardioversions, hemopurification, and continuous magnesium sulfate infusion after 4 days. This case suggests that magnesium sulfate, by antagonizing calcium overload and stabilizing membrane potential, forms a synergistic effect with electrical resuscitation and hemopurification, and is the key to reversing the electrical storms caused by aconitine poisoning.

Fatal aconitine intoxication or thyroid storm? A case report.

The case of a female in the latter half of her teens found dead in her father's apartment is reported. A glass containing liquid and plant remnants was found at the death scene. There were no indications of any intervention or the application of force by a third party. Autopsy showed unremarkable findings. Toxicological investigations revealed lethal doses of aconitine, a highly poisonous alkaloid and the major active compound of Aconitum napellus, in all specimens. Plant remnants were identified as A. napellus leaves by a botanist and toxicological examination of the liquid in the glass on si

[Aconitine poisoning due to confusion of aconite leaves with lovage].

Due to a mix-up an older couple (69 years and 71 years) ate a meal with herbs from their garden that contained leaves of monkshood (Aconitum napellus). The monkshood plants grew close to the herbs in their garden. Both patients developed the typical symptoms of aconitine poisoning with paresthesia, hypotension and bradycardia. Over the course of time both developed cardiac arrhythmia with ventricular extrasystoles and required monitoring on the intensive care unit (ICU). The husband was more severely affected and needed catecholamines for successful treatment of hypotension (70/40 mmHg) and br

Magnesium in Aconitine-Induced Electrical Storm-Mechanism Yes, Mandate No.

Aconitine poisoning produces lethal ventricular arrhythmias through persistent activation of cardiac sodium channels. We critically comment on a recent case report describing 28 biphasic shocks for aconitine-induced electrical storm, addressing three fundamental questions. First, we demonstrate that verapamil is contraindicated as it worsens hypotension and reflex sympathetic activation without antagonizing sodium channel toxicity. Second, we review the evidence for magnesium sulfate: while basic science and observational data support its mechanistic role as a calcium antagonist and sodium cha

Acute aconitine poisoning resulting from the ingestion of medicinal liquor.

Diester diterpenoid alkaloids (DDAs) are the main active ingredients of herbaceous perennial plants Aconitum. DDAs possess cardiotoxic and neurotoxic properties. Although most deaths caused by DDA poisoning are accidental, a few instances of suicide and homicide have been reported. Presented is a case of an acute aconitine (AC) poisoning following the ingestion of approximately 50 mL of homemade medicinal liquor. We described the clinical manifestations after poisoning and detailed postmortem changes, and detected the concentrations of AC and hypaconitine (HA) by liquid chromatography-tandem m

Design, Synthesis, Antifeedant Activity, and Mechanistic Research of Aconitine Derivatives.

For decades, natural product-derived insect repellents have been a major focus of research. Aconitine, a diterpene alkaloid isolated from Aconitum plants, exhibits not only significant antifeedant activity but also potent toxicological effects against certain insects. In this study, 48 novel aconitine derivatives were synthesized via esterification of the C3-hydroxyl group. Among them, compound A4 demonstrated optimum antifeedant activity with an AFC50 value of 4.91 µg/cm2, surpassing that of the parent compound aconitine (AFC50 = 11.65 µg/cm2) and the positive control Azadirachtin (AFC50 = 5.

Comparison of analgesic activities of aconitine in different mice pain models.

Aconitine (AC) is the primary bioactive and secondary metabolite alkaloidin of Aconitum species which is accounted for more than 60% of the total diester-diterpenoid alkaloids in Aconite. To evaluate the analgesic effects of AC, 4 different pain models including hot plate assay, acetic acid writhing assay, formalin and CFA induced pain models were adopted in this study. In hot plate experiment, AC treatment at concentration of 0.3 mg/kg and 0.9 mg/kg improved the pain thresholds of mice similar to the positive drug aspirin at the concentration of 200 mg/kg (17.12% and 20.27% VS 19.21%).
