Fractures of the acromion are rare. Conservative therapy in the majority of cases results in return to full function of the shoulder. Occasionally, however, a painful pseudoarthrosis will develop and surgical care is required. The American literature reports several cases of acromial fracture but has not specifically addressed the treatment of pseudoarthrosis with open reduction and internal fixation. We report such a case and review the American and European literature.

Introduction Acromion is essential for stabilizing the shoulder complex. Tumors of the acromion are rare. We report the largest series of acromion tumor and tumor-like lesion. Materials and Methods A retrospective review of the oncology and radiology database within our tertiary center for orthopaedic oncology was performed to identify all tumors of the acromion over the past 30 years and imaging was reviewed. Results We identified a total of 31 lesions arising in the acromion and chondrosarcoma was the commonest. Conclusion One needs to be aware of tumor and tumor-like lesions of acromion.

Avulsion fractures of the acromion [corrected] are described in 10 patients with severe tetanus neonatorum. Typically bilateral and invariably contemporaneous, they result from isometric contraction. Forced abduction of the arm against adductor resistance leads to avulsion of that segment of the acromium where the multipenate fibres of the deltoid muscle originate. To our knowledge this unique and ominous orthopaedic complication of severe neonatal tetanus has not been documented previously.

BACKGROUND: The acromion projects laterally in a curving orientation. When a fracture of the acromion occurs, despite being rare, matching the best-fit fixation plate is challenging due to few options and limited availability of fixation plate types. Alternative fixation methods can carry risk of nonunion complications.

The case of 6-year-old girl with an asymptomatic aneurysmal bone cyst of the acromion is reported. Such tumors are rarely located in the scapula and are especially rare in the acromion. The diagnosis was confirmed by biopsy and surgical resection of the lesion. Roentgenographic, CT and histologic features of the cyst are discussed.

BACKGROUND: Acromial fractures are rare in the traumatic setting; however, they have recently gained attention due to the increase in incidence as a result of a postoperative complication of reverse total shoulder arthroplasty. While historically these fractures were routinely treated nonoperatively, there is evidence that surgery can improve outcomes. The study aims to evaluate clinical outcomes following surgical treatment of acromion fractures and compare outcomes among patients with an intact rotator cuff against those with a deficient rotator cuff or reverse shoulder arthroplasty.

Fractures of the acromion are uncommon clinical entities, and stress fractures are even more rare, with few cases reported. Due to their rarity, stress fractures are often misdiagnosed. Here, we report a case of an elderly patient with an acromion stress fracture, which was overlooked, resulting in nonunion followed by the displacement of the distal portion of the acromion. We report the case of a patient with a long standing clinical history of subacromial impingement, in which plain radiographs and CT-scan revealed a well-circumscribed lucent lesion expanding the undersurface of the acromion. MRI showed the subacromial fat to be completely obliterated and the infraspinatus tendon deformed due to compression by a mass localised at the acromion.
