The authors present a case of seemingly textbook digital osteomyelitis with draining ulceration. Per protocol, specimens were sent to pathology with the intent to verify and identify the offending organisms. Surprisingly, the textbook osteomyelitis returned with a pathological diagnosis of rare sweat gland tumor, malignant eccrine acropsiroma, prompting immediate systemic medical workup and treatment. This case emphasizes the importance of proper laboratory workup of all surgically removed specimens, despite having a "typical" presentation.

Tumors of the external auditory canal include osteomas, ceruminomas, sebaceous cysts, keloids, basal cell carcinomas, squamous cell carcinomas, and, in rare cases, eccrine acrospiromas. We describe an eccrine acrospiroma arising from the distal external auditory canal, which was associated with decreased hearing on the affected side. Surgical resection of the tumor resulted in improved hearing.

Malignant eccrine acrospiromas are rare. Clinically, they resemble other cutaneous lesions. A high index of suspicion must be maintained in cases of histologically benign eccrine acrospiromas for three reasons: (1) malignant transformation can occur, (2) the presence of both benign and malignant tissue can lead to a false-negative diagnosis if only the benign component is obtained in the biopsy specimen, and (3) benign-appearing tumors can recur locally or metastasize. Eccrine acrospiroma is a tumor originating from the sweat glands. The computed tomography findings of eccrine acrospiroma in the right inguinal area are presented. In our case, the tumor was seen as a well-defined and ovoid-shaped cystic mass with an internal enhancing solid portion and containing calcifications. Eccrine acrospiroma should be considered in the differential diagnosis of a subcutaneous cystic mass with an internal solid nodule.

Eccrine acrospiroma, better known as eccrine poroma, is a benign adnexal neoplasm of the skin. Its clinical aspect can masquerade as some other nodular and cystic lesions. The current dermoscopy literature offers very few case studies. Moreover, these very few examples present a totally different appearance pattern compared to the one we examined. Its homogeneous blue pattern suggested the better-known nodular malignant melanoma; in fact, this dermoscopic appearance was due to the Tyndall effect.

BACKGROUND: Malignant acrospiroma is a rare tumor of the eccrine sweat glands accounting for around 6% of all malignant eccrine tumors. Typically, it presents as large ulcerated nodules, and diagnosis can be challenging as it has great overlap with its benign counterpart.
CASE PRESENTATION: We herein report a case of acral malignant acrospiroma, initially treated with surgical excision and adjuvant radiotherapy. After metastatic disease was confirmed, subject received multiple lines of chemo- as well as targeted therapy. Genomic testing was also done using next generation sequencing.

A clinicopathologic study of 18 eccrine acrospiromas with atypical nuclear changes is presented. The changes included nuclear pleomorphism, prominent nucleoli, chromatin condensation, multinucleate giant cells and increased mitotic activity. Benign adnexal tumors of the skin, excluding pilomatrixomas, were identified in 28 patients between the ages of 4 and 20 years with a mean age of 15; 23 women and 5 men. Most lesions were described as papules and they occurred on the face, scalp and neck in 75% of cases. Trichoepithelioma and syringoma accounted for 56% of all neoplasms. Acrospiroma, also known as hidradenoma, is a rare cutaneous tumor that has several histological characteristics. As a consequence, a high index of suspicion is necessary for its diagnosis. Here we report a case that illustrates the importance of a good clinical-pathologic correlation in order to recognize this disease.

An 87-year-old woman presented with a rapidly growing tumour at the nasal side of the upper eyelid. The clinical diagnosis was basal cell carcinoma. During surgery, the impression of malignancy was accentuated by the finding that even the conjunctiva had been invaded. Histological examination revealed an eccrine acrospiroma with, indeed, many mitoses but, on the other hand, a completely benign aspect. Besides, there was no recurrence after 1 year.

Using a rabbit anti-human liver ferritin antibody, we examined the binding patterns of this reagent in normal skin and observed a unique binding pattern limited to the outermost layer of the eccrine duct. Examination of a variety of sweat gland neoplasms revealed 2 distinct patterns. One was the binding of this antibody to the outermost layer of cells in the epithelial cords of syringoma, producing a characteristic ring when seen in cross-section.
