Defining the acute care surgery curriculum.

BACKGROUND: This study was designed to define the gaps in essential and desirable (E/D) case volumes that may prompt reevaluation of the acute care surgery (ACS) curriculum or restructuring of the training provided. METHODS: A review of the first 2 years of ACS case log entry (July 2011 to June 2013) was performed. Individual trainee logs were evaluated to determine how often they performed each case on the E/D list. Trainees described cases using current procedural terminology codes, which had been previously mapped to the E/D list. RESULTS: There were 29 trainees from 15 programs (Year 1)

Acute care surgery after renal transplantation.

Emergent operation after renal transplantation (RT) has traditionally been associated with substantial morbidity and mortality. We reviewed 2340 adult patients who underwent RT at our tertiary care center and identified 55 patients who required acute care surgical consultation within 1 year of transplantation. Of these, 43 were treated operatively and 12 nonoperatively. Primary diagnoses were intestinal problems in 29 patients (53%), including diverticulitis, ischemia, perforation, obstruction, and bleeding; cholecystitis in 10 (18%); fluid collections in six (11%), appendicitis and hernias in

Palliative care in acute care surgery: research challenges and opportunities.

Palliative care includes effective communication, relief of suffering and symptom management with an underlying goal of improving the quality of life for patients with serious illness and their families. Best practice palliative care is delivered in parallel with life-sustaining or life-prolonging care. Palliative care affirms life and regards death as a normal process, intends neither to hasten death nor to postpone death and includes but is not limited to end-of-life care. Palliative care encompasses both primary palliative care (which can and should be incorporated into the practice of acut

Value in acute care surgery, part 4: The economic value of an acute care surgery service to a hospital system.

The Healthcare Economics Committee of the American Association for the Surgery of Trauma has published a series of three articles on the topic of value in acute care surgery (ACS). In this series, the key elements of value, cost and outcomes, and the impact of stakeholder perspective on what constitutes high-value care are discussed. The fourth article in this series continues the discussion by focusing on the unique economic value that an ACS service brings to a hospital system and its patients. Characterized by the immediate 24-hour availability of surgeons trained in trauma management, emer

The impact of an acute care surgery team on general surgery residency.

BACKGROUND: Acute care surgical teams (ACSTs) have limited data in residency. We sought to determine the impact of an ACST on the depth and breadth of general surgery resident training. METHODS: One year prior to and after implementation of an ACST, Accreditation Council for Graduate Medical Education case logs spanning multiple postgraduate year levels were compared for numbers, case types, and complexity. RESULTS: We identified 6,009 cases, including 2,783 after ACST implementation. ACSTs accounted for 752 cases (27%), with 39.2% performed laparoscopically. ACST cases included biliary (19.

Malnutrition in Acute Care Surgery Patients in Rwanda.

INTRODUCTION: Malnutrition is prevalent in hospitalized surgical patients and has been shown to significantly alter outcomes including length of hospital stay, complications, and mortality. Different tools for nutrition assessment were developed and are being used. The aim of this study was to characterize the nutritional status of acute care surgery patients, determine risk factors for malnutrition, and describe outcomes in patients with malnutrition. METHODS: This was a prospective, descriptive study of malnutrition in acute care surgery patients at CHUK. Over a 6-month time period, we coll

Advances in laparoscopy for acute care surgery and trauma.

The greatest advantages of laparoscopy when compared to open surgery include the faster recovery times, shorter hospital stays, decreased postoperative pain, earlier return to work and resumption of normal daily activity as well as cosmetic benefits. Laparoscopy today is considered the gold standard of care in the treatment of cholecystitis and appendicitis worldwide. Laparoscopy has even been adopted in colorectal surgery with good results. The technological improvements in this surgical field along with the development of modern techniques and the acquisition of specific laparoscopic skills

Concerning Trends in Acute Care Surgeries Among General Surgery Trainees.

BACKGROUND: Surveys of residents, program directors, and fellowship leaders have raised concerns about general surgery graduates' readiness for independent practice. Evolving practice patterns, particularly the rise of laparoscopic surgery, advanced endoscopy, and interventional radiology, have altered residents' exposure to traditional open procedures. However, the impact of these changes on operative experience in acute care surgery (ACS) has not been well quantified. OBJECTIVE: This study assessed temporal trends in the operative experience of general surgery trainees in the acute care sur

Important role of acute care surgery during pandemic time.

Pandemic impacts acute care surgery for diseases, such as gallbladder disease and acute appendicitis. At the early stage of coronavirus disease 2019 (COVID-19) pandemic, the case number of patients needing surgery decreased in hospitals from different countries. This decline was associated with the stay-home order and fear of getting COVID-19 infection. However, recent reports show that the case number for acute surgery returns to the normal level, which is comparable to that before the beginning of the pandemic. In addition, a variety of diseases show more severe than the cases before the pan

Complications of bariatric surgery: the acute care surgeon's experience.

BACKGROUND: Complications of bariatric surgeries are common, can occur throughout the patient's lifetime, and can be life-threatening. We examined bariatric surgical complications presenting to our acute care surgery service. METHODS: Records were reviewed from January 2007 to June 2013 for patients presenting with a complication after bariatric surgery. RESULTS: Laparoscopic Roux-en-Y gastric bypass was the most common index operation (n = 20), followed by open Roux-en-Y gastric bypass (n = 6), laparoscopic gastric band (n = 4), and vertical banded gastroplasty (n = 3). Intestinal endometriosis: A rare cause of acute care surgery.

BACKGROUND: Intestinal endometriosis is a rare condition that can cause gastrointestinal symptoms such as abdominal pain, constipation, and diarrhea. It occurs in approximately 5%-15% of women with endometriosis. Although it rarely leads to obstruction or perforation, there is no clear consensus on its optimal management. Hormonal therapy is considered the first-line treatment for endometriosis; however, the best approach for intestinal involvement remains controversial. This study aims to contribute to the literature by evaluating patients with intestinal endometriosis who underwent surgery f

An acute care surgery model improves timeliness of care and reduces hospital stay for patients with acute cholecystitis.

In October 2009, an acute care surgery (ACS) model was implemented to facilitate urgent surgical consults. This study examines the impact of ACS on the timeliness of care and length of hospitalization for patients with acute cholecystitis. A retrospective cohort study was performed of patients presenting to the emergency department (ED) with acute cholecystitis who underwent early cholecystectomy. Patients with choledocholithiasis, pancreatitis, biliary colic, or cholelithiasis without cholecystitis were excluded. There were two study cohorts: ACS (October 2009 to July 2010) and pre-ACS (Octob

Time and cost analysis of gallbladder surgery under the acute care surgery model.

BACKGROUND: The acute care surgery (ACS) model has been shown to improve work flow efficiency and to reduce hospital stay. We hypothesized that, in patients with gallbladder (GB) disease who were admitted through our emergency department (ED) and then underwent surgery, the ACS model shortened the time to surgery, decreased the length of hospital stay, and reduced hospital costs. METHODS: We retrospectively queried our GB surgery practice records for 2008 (before the establishment of the ACS model at our institution in 2009). We then performed time and cost comparison with our prospectively m

The educational foundation for acute care surgery: Evolution of the specialty.

Acute care surgery (ACS) was conceived in the early 2000s in response to a growing crisis in access to emergency general surgical care. The ACS fellowship was created to train general surgeons in this subspecialty. The purpose of this review is to discuss the origin of ACS and its evolution over the last 20 years and to survey the subspecialty's current status and future directions. Since the first programs were certified in 2008, the fellowship curriculum has undergone many changes. There is now more emphasis on emergency general surgery and a movement toward competency-based evaluations. Add

Fast track pathway provides safe, value based care on busy acute care surgery service.

BACKGROUND: Fast track (FT) pathways have been adopted across a multitude of elective surgeries but have been slow to be adopted into the acute care surgery realm. We hypothesized that an FT pathway for acute cholecystitis patients would decrease patient length of stay and resource utilization. METHODS: All patients at two hospitals, one with an FT pathway and one with a traditional pathway, who underwent an urgent laparoscopic cholecystectomy for acute cholecystitis between May 1, 2019, and October 31, 2019, were queried using CPT codes. Improving Acute Care Surgery with Artificial Intelligence: A Practical Review.

Artificial intelligence (AI) and machine learning are poised to transform trauma care across the entire continuum, from prehospital triage to postoperative critical care. Trauma systems are uniquely suited for AI integration due to the time-sensitive, high-volume, and data-rich nature of care delivery. In this narrative review, we describe current and emerging AI applications across the trauma care spectrum, including triage, acute resuscitation, operative decision-making, intensive care, and detection of complications. We also examine AI's potential in nontraditional care environments, includ
