Acute Febrile Encephalopathy with Rigidity.

Febrile encephalopathy with rigidity can be the presenting feature of several life-threatening neurological emergencies such as neuroleptic malignant syndrome (NMS) and serotonin syndrome. Although the pathogenesis of these conditions is poorly understood, we review advances in our understanding of underlying genetic and other biological mechanisms. We also discuss the complex and expanding differential diagnoses which include other drug-induced hyperpyrexia or rigidity syndromes (eg malignant hyperthermia, parkinsonism-hyperpyrexia syndrome, malignant catatonia) as well as autoimmune syndrome

Therapeutic indicators of acute encephalopathy in patients with complex febrile seizures.

BACKGROUND: The aim of this study was to identify predictors of neurologic damage in children with febrile seizures or altered consciousness within 6 h of seizure onset. METHODS: The present study included 86 children (age range, 4-159 months old), who fulfilled the diagnostic criteria for complex febrile seizures on admission between October 2002 and November 2008. Using the Pediatric Cerebral Performance Category Scale (PCPC), the children were classified into two groups: poor outcome (PCPC = 2-6) and good outcome (PCPC = 1). Clinical profiles and laboratory findings were compared between t

Spectrum of Pediatric Acute Febrile Encephalopathy in Hilly Areas of Northwestern Himalayas.

BACKGROUND AND AIM: The etiological profile of acute febrile encephalopathy (AFE) varies across different geographic areas and in different seasons across the world. Limited literature in children suggests central nervous system (CNS) infections to be the most common cause of AFE in India and developing countries. Most of these studies have been done in the plains of India, and no study has been done in the Himalayan region of northwestern India, where most of the vectors causing AFE in other parts of India, like mosquitoes, are not found. Therefore, this leads to a lacuna in available data on

Familial recurrent febrile encephalopathy: an atypical presentation of acute necrotising encephalopathy type 1 with unusual neuroimaging findings.

Acute necrotising encephalopathy type 1 (ANE1) is a rare, autosomal dominant disorder caused by mutations in the RANBP2 gene, characterised by recurrent episodes of encephalopathy triggered by febrile illness. The hallmark neuroimaging feature typically involves bilateral symmetric thalamic lesions. Atypical findings, either clinical or neuroimaging-wise, can pose significant diagnostic challenges. We report one such case of an early adolescent boy with a strong family history of recurrent febrile encephalopathies, who presented with his first episode of febrile illness associated with neurolo

Pathogenic Variants in NUP214 Cause "Plugged" Nuclear Pore Channels and Acute Febrile Encephalopathy.

We report biallelic missense and frameshift pathogenic variants in the gene encoding human nucleoporin NUP214 causing acute febrile encephalopathy. Clinical symptoms include neurodevelopmental regression, seizures, myoclonic jerks, progressive microcephaly, and cerebellar atrophy. NUP214 and NUP88 protein levels were reduced in primary skin fibroblasts derived from affected individuals, while the total number and density of nuclear pore complexes remained normal. Nuclear transport assays exhibited defects in the classical protein import and mRNA export pathways in affected cells. Direct surfac

Febrile Hashimoto's encephalopathy associated with Graves' disease and acute pancytopenia: A case report.

RATIONALE: Hashimoto's encephalopathy (HE) is an autoimmune-mediated encephalopathy rarely seen in Graves' disease, with <20 cases reported previously, associated with elevated concentration of circulating serum anti-thyroid antibodies usually responsive to steroid therapy. PATIENT CONCERNS: We present a HE case (25-year-old male) with Graves' disease, complicated by fever and pancytopenia. The patient presented with fever, gait impairment, delirium, agitation and disorientation. DIAGNOSES: Thyroid-related antibodies were elevated and brain magnetic resonance imaging confirmed symmetrical wh

[Clinical picture and course of the neuroleptic malignant syndrome (acute febrile neuroleptic encephalopathy)].

On the basis of their observations and management of 23 cases of the neuroleptic malignant syndrome (MNS) the authors draw a conclusion that this syndrome is similar to extrapyramidal-psychotic side effects of neuroleptic therapy. Different variants of the course of this complication of neuroleptic therapy attended by hyperthermia are considered. The authors propose the term 'acute febrile neuroleptic therapy' which incorporates all hyperthermic variants of the course of this complication of neuroleptic therapy, varying from mild transitory one to the MNS. Criteria of the early diagnosis are p

Clinical Reasoning: A 2-Year-Old Girl With Acute Encephalopathy After Febrile Systemic Illness.

A 2-year-old girl presented with an acute encephalopathy with hemiparesis after febrile systemic illness. MRI showed multifocal symmetrical lesions in both thalami, and CSF showed a slightly elevated protein. The final diagnosis in this patient led to a revision of the diagnosis of neuroborreliosis in her mother, 15 years earlier. The bilateral, symmetric thalamic involvement at presentation was an important clue to suggest the final diagnosis in the mother. This case highlights common clinical pitfalls and important neuroimaging findings that led to the final diagnosis in the case of an acute

Risk Factors of Prehospital Emergency Care for Acute Encephalopathy in Children With Febrile Status Epilepticus.

BACKGROUND: Acute encephalopathy with biphasic seizures and late reduced diffusion (AESD) often develops in children with febrile status epilepticus (FSE) with neurological sequelae. No study has investigated the associations between prehospital emergency care and AESD onset. METHODS: We retrospectively collected the data of children with FSE (>30 min) treated in Tottori University Hospital. We evaluated the prehospital emergency care information, investigating its association with AESD development. RESULTS: We identified 11 patients with AESD and 44 with FSE. The time from onset to the arri
