The adequate cotreatment of chronic pain and addiction disorders is a complex and challenging problem for health care professionals. There is great potential for cannabinoids in the treatment of pain; however, the increasing prevalence of recreational cannabis use has led to a considerable increase in the number of people seeking treatment for cannabis use disorders. Evidence that cannabis abuse liability is higher than previously thought suggests that individuals with a history of substance abuse may be at an increased risk after taking cannabinoids, even for medicinal purposes. Smoked cannab

Most overdose deaths in the United States involve opioids. Identification and management of opioid use disorder by primary care physicians is a critical need in health care. Substance use disorders share neurobiological dysregulation of the central motivation and reward pathway (powered by dopamine) that manifests as a cycle of addiction driven by impulse and compulsion. It is important that concern for opioid use disorder does not disrupt appropriate treatment of pain and that analgesic narcotic use is adequately monitored, especially in patients at risk of opioid use disorder. Most patients

Practice guidelines consistently encourage short-term use of benzodiazepines for the management of common medical conditions. However, these medications are often prescribed long-term for unclear or variable indications. These prescribing patterns may be attributed to perceived low risk and low rate of benzodiazepine use disorders (0.2% of US adults). Compared with other addictive substances, benzodiazepines may have less overall risk and fewer adverse outcomes. Benzodiazepines have limited accessibility compared with alcohol and tobacco. When used alone, benzodiazepines have less risk of leth

The development of research, clinical practice and prevention in addiction medicine is, compared to other disciplines, more influenced by socio-political factors. Research on psychedelics has thus long been hampered by prohibitive policy but has recently been revived thanks to the relaxation of these political positions. The extensive prescription of baclofen for alcohol addiction is mainly the consequence of popular support and has so far not been clearly supported by specific research. The aim of the new Gambling Act was to harmonize the regulatory provisions for lotteries and betting on the

Primary care physicians play an integral role in the identification and management of alcohol use disorder, which has implications for the safety and physical and mental health of patients, their families, and the public. Screening to identify risky drinking behavior is recommended by the US Preventive Services Task Force but is not always performed consistently or correctly in primary care. When alcohol use disorder is identified, collaboration with patients is essential to determine an appropriate treatment approach. Abstinence may not always be the answer. Approximately one-half of patients

The number one cause of preventable disease, disability, and death in the United States is tobacco use. According to data from the National Health Interview Survey, 18.7% of US adults (46 million people) currently use a tobacco product. Smoking causes lung, laryngeal, hepatocellular, and colorectal cancers and possibly breast cancer. Nicotine is the highly addictive component of tobacco that releases dopamine when it binds to alpha-4 beta-2 nicotinic acetylcholine receptors in the brain. This produces reward sensations that become associated with specific behaviors and relieves stress and nega

Opioid addiction constitutes a major public health issue. Its management relies on opioid agonist therapy (OAT), which replaces the illicit consumption to stabilize the dependence. In 2023, 15,707 people with an average age of 49 were receiving OAT in Switzerland. A general practitioner can initiate OAT in most cases. A laboratory assessment and cantonal authorization are mandatory for OAT initiation. Methadone, the main treatment, requires gradual titration (20-30 mg initially and then 60-120 mg for maintenance) with daily monitoring. Adverse effects include constipation, hypogonadism, sleep

The results of the study of 158 patients with alcohol dependence, comorbid in 85 cases with slow progressive schizophrenia and in 75 cases with affective disorders (cyclothymia), are viewed in the aspect of a theory of comorbidity. The psychopathological peculiarities of each nosologic form are singled out. The author stresses that establishing of the comorbid pathology is important for the right therapeutic strategy. The effectiveness of using the combinations of neuroleptics, either typical or atypical, and antidepressants (tricyclic, SSRIs etc) is revealed.

BACKGROUND: For many years, clinical protocols for treatment of drug abuse patients and treatment standards in Russian Federation were not grounded on the principles of evidence-based medicine [1]. Recommendations for use of certain drugs were not accompanied by any indication of the level of credibility of the evidence supporting it. The appearance in 2014 of such indications in clinical recommendations can be considered a significant step forward for the science of addiction medicine [2].
OBJECTIVE: To compare Russian evidence and practice in addiction medicines with international standards

Addiction and the consequences including medical, social, and legal are seen almost daily in a primary care office. To effectively deal with these problems it is imperative that the physician view addiction as a disease. This article explores the disease concept and the basis for the view as well as providing a guide to diagnosis and assessment. Useful screening tools are also provided as well as a discussion of biochemical markers currently available. A guideline for delivering the diagnosis is also discussed. An overview of the stages of treatment is provided as well, with particular emphasi

Recent progress has been made in our understanding of nonsubstance or "behavioral" addictions, although these conditions and their most appropriate classification remain debated and the knowledge basis for understanding the pathophysiology of and treatments for these conditions includes important gaps. Recent developments include the classification of gambling disorder as a "Substance-Related and Addictive Disorder" in the 5th edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) and proposed diagnostic criteria for Internet Gaming Disorder in Section 3 of DSM-5. Increasingly, patients with unhealthy alcohol and other drug use are being seen in primary care and other non-specialty addiction settings. Primary care providers are well positioned to screen, assess, and treat patients with alcohol and other drug use because this use, and substance use disorders, may contribute to a host of medical and mental health harms. We sought to identify and examine important recent advances in addiction medicine in the medical literature that have implications for the care of patients in primary care or other generalist settings. To accomplish this aim, we selected a

INTRODUCTION: A minority of addiction patients receive appropriate treatment directly from trained professionals in addiction medicine. Most countries have not recognized addiction medicine (AM) as a specialty in its own right or within psychiatry/other specialties. Therefore, the effectiveness and organization of AM training around the world need to be improved. Unfortunately, standard instruments are rarely used in most studies to assess training needs. This study aimed to determine international competencies in AM among professionals in as many countries as possible using a standard instrum

BACKGROUND: Drug addiction, or substance abuse disorder, has a long history. The use of a drug that causes dependence has been used in various religious and tribal initiation ceremonies. Currently, recreational use or peer modelling leads an individual to substance abuse. The magnitude of the problem of addiction increases multifold in the presence of stressors like broken families, joblessness, occupational stress, etc.
OBJECTIVES: The de-addiction process is used to relieve an individual's craving for the drug. It is a painful process, as several effects of withdrawal compel a person to ret

Alcohol addiction or alcoholism is the most severe form of problem drinking. A variety of treatment methods for alcoholism are currently available that combine medications, behavioral treatment and peer support. The drugs that are currently approved by the U.S. Food and Drug Administration (FDA) for treatment of alcohol dependence are disulfiram, naltrexone and acamprosate. For many patients, however, these treatments are not effective. Evidence from a number of studies suggests that various factors, both psychosocial and economic, as well as genetic variation, are significant contributors to

Traditional Chinese medicine (TCM) includes Chinese medicine and acupuncture. Chinese medicine consists of natural products including plants, animals and minerals. TCM has been practiced in China for more than 2000 years, and for the past 200 years has been used in treatment of drug addiction. Ten Chinese medicines for the treatment of opiate addiction have been approved by the Chinese State Food and Drug Administration (SFDA), and at least 6 are in clinical trials. The general therapeutic principle of Chinese medicine developed was based on its unique theory of " reinforcing healthy Qi and re

PROBLEM: Substance use is highly prevalent in the United States, but little time in the curriculum is devoted to training internal medicine residents in addiction medicine.
APPROACH: In 2014, the authors developed and launched the Addiction Recovery Clinic (ARC) to address this educational gap while also providing outpatient clinical services to patients with substance use disorders. The ARC is embedded within the residency primary care practice and is staffed by three to four internal medicine residents, two board-certified addiction medicine specialists, one chief resident, and one psycholo
