IMPORTANCE: Among adults with chronic illness, 30% to 50% of medications are not taken as prescribed. In the United States, it is estimated that medication nonadherence is associated with 125 000 deaths, 10% of hospitalizations, and $100 billion in health care services annually.
OBSERVATIONS: PubMed was searched from January 1, 2000, to September 6, 2018, for English-language randomized clinical trials of interventions to improve medication adherence. Trials of patients younger than 18 years, trials that used self-report as the primary adherence outcome, and trials with follow-up periods less

BACKGROUND: Over the last 20 years, there have been marked improvements in the availability of effective medications for rheumatic conditions such as gout, osteoporosis and rheumatoid arthritis (RA), which have led to a reduction in disease flares and the risk of re-fracture in osteoporosis, and the slowing of disease progression in RA. However, medication adherence remains suboptimal, as treatment regimens can be complex and difficult to continue long term. Many trials have been conducted to improve adherence to medication. Core domains, which are the outcomes of most relevance to patients an

BACKGROUND: Poor adherence to therapy is a significant healthcare issue, particularly in patients with chronic disease such as open-angle glaucoma. Treatment failure may necessitate unwarranted changes of medications, increased healthcare expenditure and risk to the patient if surgical intervention is required. Simplifying eye drop regimes, providing adequate information, teaching drop instillation technique and ongoing support according to the patient need may have a positive effect on improving adherence.
OBJECTIVES: To summarise the effects of interventions for improving adherence to ocula

BACKGROUND: Lipid lowering drugs are still widely underused, despite compelling evidence about their effectiveness in the treatment and prevention of cardiovascular disease. Poor patient adherence to a medication regimen is a major factor in the lack of success in treating hyperlipidaemia. In this updated review we focus on interventions which encourage patients at risk of heart disease or stroke to take lipid lowering medication regularly.
OBJECTIVES: To assess the effects of interventions aimed at improved adherence to lipid lowering drugs, focusing on measures of adherence and clinical out

This literature review explores the range and nature of medication adherence interventions tested with older adults. The unique needs of older adults require specifically designed and tailored interventions. Low medication adherence rates among some elderly contribute to inadequate pharmacological management of illnesses. Searches were conducted to identify randomized controlled trials of medication adherence; computerized databases, journal hand searches, and ancestry searches yielded 63 studies published between 1977 and 2005 where participants' mean age was > 60 years. Interventions were ca

BACKGROUND: Pharmacological treatments for tobacco dependence, such as nicotine replacement therapy (NRT), have been shown to be safe and effective interventions for smoking cessation. Higher levels of adherence to these medications increase the likelihood of sustained smoking cessation, but many smokers use them at a lower dose and for less time than is optimal. It is therefore important to determine the effectiveness of interventions designed specifically to increase medication adherence. Such interventions may include further educating individuals about the value of taking medications and p

Low adherence remains a persistent challenge in self-guided internet interventions, and universal strategies are needed to address it. In this study, we designed and evaluated a brief self-guided add-on module aimed at strengthening self-efficacy specific to adhering to internet interventions. The module included a testimonial video and two text-based exercises grounded in self-efficacy theory. It targeted self-efficacy specific to integrating and maintaining use of internet interventions. In Phase 1, university students (N = 217) were randomized to the module or a waitlist control. The aim wa

Morbidly adherent placentas are a spectrum of abnormalities ranging from placental invasion of the myometrium to invasion past the myometrium and muscular layers into adjacent structures. This entity is becoming more prevalent recently with increased number of cesarean deliveries. Given the high risk of morbidity and mortality, this was traditionally treated with pre-term planned cesarean hysterectomy. However, recently, uterine preservation techniques have been implemented for those women wishing to preserve future fertility or their uterus. Early identification is crucial as studies have sho

BACKGROUND: Bronchiectasis is characterised by a widening of the airways, leading to excess mucus production and recurrent infection. It is more prevalent in women and those in middle age. Many patients with bronchiectasis do not adhere to treatments (medications, exercise and airway clearance) prescribed for their condition. The best methods to change these adherence behaviours have not been identified.
OBJECTIVES: To assess the effects of interventions to enhance adherence to any aspect of treatment in adults with bronchiectasis in terms of adherence and health outcomes, such as pulmonary e

Psoriasis is a chronic inflammatory skin disease for which long-term treatment adherence is essential to achieve and maintain disease control. However, adherence remains suboptimal in routine clinical practice. While a range of interventions have been developed to support adherence, variability in intervention design and outcome reporting has limited the ability to draw clear conclusions regarding their effectiveness. This systematic review aimed to evaluate interventions designed to improve treatment adherence among patients with psoriasis, to synthesise evidence on adherence outcomes as well

Antiretroviral therapy (ART) is a keystone of the public health response to HIV, making the support of adherence a point of focus in research and service delivery. Over the past decade, measurements of viral suppression have increasingly been used to evaluate interventions, with more robust study designs gaining traction. Effective approaches include adherence counselling beyond ART education (including mental health and wellbeing approaches), reducing burden of care, tackling structural and societal determinants of health, and using mHealth platforms to deliver interventions. Increasingly, si

BACKGROUND: Sub-optimal medication adherence in pregnant women with chronic disease and pregnancy-related indications has the potential to adversely affect maternal and perinatal outcomes. Adherence to appropriate medications is advocated during and when planning pregnancy to reduce risk of adverse perinatal outcomes relating to chronic disease and pregnancy-related indications. We aimed to systematically identify effective interventions to promote medication adherence in women who are pregnant or planning to conceive and impact on perinatal, maternal disease-related and adherence outcomes.
