[Pharmacovigilance in child and adolescent psychiatry].

Rational pharmacotherapy is a challenging task in child and adolescent psychiatry. Increasing prescription numbers contrast with the uncertainties of safety and efficacy issues. The lack of clinical (authorization) trials often implies a non- age-specific use of drugs. However, young patients show particular metabolic conditions and a higher vulnerability for adverse drug reactions. Thus it seems mandatory to create age-specific pharmacological data about efficacy and safety of psychotropic drug use in minors. Legislation authorities became aware of this situation and introduced European and n

Comorbidity in children and adolescent psychiatry.

Numerous epidemiological and clinical studies found a high incidence of comorbidity in children and adolescent psychiatry. In population of children and adolescents is most often described and researched comorbidity of depression, anxiety disorders, than in hyperkinetic and behavioral disorders, and finally the difficulty in reading (dyslexia) and antisocial behavior disorders. The common occurrence of two diagnoses may be the result of a disorder that has a complex pattern of symptoms, which apparently seems to be two disorders, but in fact is a single disorder. Comorbidity may be two disorde

The Microbiome in Child and Adolescent Psychiatry.

Recent research has increasingly emphasized the function of the microbiome in human health. The gut microbiome is essential for digesting food and seems to play a vital role in mental health as well. This review briefly overviews the gut microbiome and its interplay with the central nervous system. We then summarize some of the latest findings on the possible role of the microbiome in psychiatric disorders in children and adolescents. In particular, we focus on autism spectrum disorder, attention-deficit/hyperactivity disorder, anorexia nervosa, bipolar disorder, and major depressive disorder.

How animal models inform child and adolescent psychiatry.

OBJECTIVE: Every available approach should be used to advance the field of child and adolescent psychiatry. Biological systems are important for the behavioral problems of children. Close examination of nonhuman animals and the biology and behavior that they share with humans is an approach that must be used to advance the clinical work of child psychiatry. METHOD: We review here how model systems are used to contribute to significant insights into childhood psychiatric disorders. Model systems have not only demonstrated causality of risk factors for psychiatric pathophysiology, but have also

Clinical Stakes of Sexual Abuse in Adolescent Psychiatry.

BACKGROUND: The extent and nature of sexual abuse (SA) and its consequences in psychiatry are still poorly described in adolescence. OBJECTIVE: This article describes the frequency of SA reported in an adolescent population hospitalized in psychiatry, and assesses its links with the severity of mental disorders and the medical issues of these adolescents. METHODS: The study includes 100 patients for whom SA has been mentioned, among all patients aged 13 to 17 years old hospitalized for about 4 years. The characteristics of sexual abuse were correlated with the medical severity of the patient

Therapeutic drug monitoring in child and adolescent psychiatry.

Psychopharmacotherapy in children and adolescents is characterized by an increased susceptibility for adverse events and an increased risk of ineffective treatment due to specific age-dependent and developmental characteristics in comparison to adults. Dosing in paediatric psychiatric patients requires careful handling, since the dose recommendations for adults can not simply be extrapolated to minors because of pharmacokinetic and pharmacodynamic differences. In addition, psychopharmacotherapy in children and adolescents is hampered by lack of high quality evidence on efficacy and safety in m

[Treatment Gaps in the Transition of Adolescent Patients from Child and Adolescent Psychiatry to Adult Psychiatry in Berlin: An explorative predictor Analysis].

OBJECTIVE: This study investigates treatment gaps during the transition from child and adolescent psychiatry (CAP) to adult psychiatry (AP). METHODOLOGY: 100 patients from a Berlin AP clinic with prior CAP treatment were surveyed regarding their experiences and living conditions before and after transition. A stepwise regression analysis with backward elimination identified possible predictors. RESULTS: Significant negative associations were found with perceived social support, school-related overwhelm, previously helpful CAP treatments, and later diagnoses from the schizophrenia spectrum.

Treatment of eating disorders in child and adolescent psychiatry.

PURPOSE OF REVIEW: Recent research on the multimodal treatment of eating disorders in child and adolescent psychiatry has yielded a significant increase in randomized controlled trials and systematic reviews. This review aims to present relevant findings published during the last 2 years related to medical and psychological treatment of anorexia nervosa, bulimia nervosa and avoidant/restrictive food intake disorder (ARFID). RECENT FINDINGS: For anorexia nervosa, recent reports described the efficacy of different treatment settings, lengths of hospital stay and high vs. low-calorie refeeding p

[Catatonia is often overlooked in child and adolescent psychiatry].

Catatonia is a common but often overlooked motor syndrome in child and adolescent psychiatry. It is characterized by a variety of symptoms, most often excitement, immobility, stupor, catalepsy, grimacing, echolalia, echopraxia, stereotypies, mannerisms, logorrhoea, verbigeration, negativism, staring and withdrawal. This case report illustrates how a 17-year-old man was diagnosed with catatonia after one year of repeating psychiatric care. The catatonic symptoms decreased significantly after a short period of lorazepam administration.

[Symptomatology of pathologic inhibition in child and adolescent psychiatry].

We propose firstly to take a synchronic approach, looking at different classifications of the morbid features of inhibition, and at how aspects change according to the degree of inhibition. We will then move on to a diachronic approach, looking at how inhibition can be observed at the different stages of maturity of the child and adolescent, and at how the significance of the clinical picture alters according to the age of onset, and family, social and cultural background. The third part will consist of a pathogenetic approach to manifestations of inhibition. Finally, some case histories will

Prescribing antipsychotics in child and adolescent psychiatry: guideline adherence.

Antipsychotics are often prescribed to children and adolescents, mostly off-label. We aimed to assess adherence to recommendations of guidelines for antipsychotic prescription. We reviewed 436 medical records from 155 clinicians from 26 clinics within three Dutch child and adolescent psychiatry organizations (n = 398 outpatient, n = 38 inpatient care). We assessed target symptoms, diagnostic process, prior and concomitant treatment, and consideration of contra-indications. Multiple logistic regression assessed the role of age, sex, and psychiatric diagnosis on adherence to three main recommend

Child and adolescent psychiatry consultations during the COVID-19 pandemic.

This study aimed to identify the biopsychosocial stressors influencing child and adolescent mental health during periods when facetoface education was suspended and to evaluate the psychological sequelae of school closures and related restrictions. We conducted a retrospective observational study at Kartal Dr. Lütfi Kırdar City Hospital, a tertiary care center in İstanbul, reviewing all patients referred to the child and adolescent consultation-liaison psychiatry (CLP) unit between March 2020 and March 2022. We assessed associations between psychiatric diagnoses and health-related stressors, s

A community child and adolescent psychiatry fellowship for nurse practitioners.

Over the past three decades, there has been a well-documented shortage of child and adolescent psychiatric medical providers while demand continues to rise. Youth from minority racial and ethnic backgrounds, low-income families, and rural settings are disproportionately affected, increasing disparity in access and quality of services. While psychiatric mental health nurse practitioners (PMHNP) can deliver a full range of mental health care services across the lifespan, there are discrepancies across training programs in exposure to child and adolescent cases and high quality training.
