[The characteristics of aerophagy and its treatment].

Clinical and roentgenological examination of esophagus and cervical spine were carried out in 27 patients with functional aerophagia. In 11 of them the air was found to pass through without any swallowing into the stomach and in remaining 16--into esophagus. All the patients revealed osteochondrosis, unstable and blocked spines. Manual therapy applied to cervical zone of spine promoted elimination of aerophagia.

[Early infantile autism and excessive aerophagy with symptomatic megacolon and ileus in a case of Ehlers-Danlos syndrome].

A case of Ehlers-Danlos syndrome is reported, which was associated with mental retardation and early infantile autism. The patient died at the age of 19 by ectasy of the stomach as the result of extreme aerophagy causing a megacolon and by compression so finally resulting in a mechanical ileus. Investigation of the cerebellum revealed a significant rarefication and diminuation of the Purkinjè cells as well as the cells of the stratum granulare in the lobuli VI and VII, obviously due to a genetically determined malformation.

[Complications of the digestive tract in patients after laryngectomy].

The authors describe the case of six patients who after having undergone laryngectomy suffered from complications affecting digestive tract. One of the patients contracted aerophagy and in the case of five other patients occurred bleeding from the upper digestive tract. Four of the letter five patients had perforation of the gastric ulcer; three of them were treated surgically (in two cases by partial resection of the stomach, in one by ligation of the vessels in ulceration area) and in one case the conservative treatment was applied. All four above patients were completely cured of neoplasmat

[Intestinal gases and digestive pathology: clinical syndromes and their treatment].

A number of annoying and frequent disturbances caused by an increase in intestinal meteorism have aroused the interest of students since the last century. Aerophagy, abdominal distension, flatulence represent the commonest aspects of this frequent gastroenterological pathology. The new diagnostic approaches have made it possible to classify the principal mechanisms involved in the production and elimination of intestinal gases. It has thus been possible to study with greater precision the composition of gaseous content and the origin and removal of intestinal gases. It is possible to demonstra

Functional gastroduodenal disorders.

While widely used in research, the 1991 Rome criteria for the gastroduodenal disorders, especially symptom subgroups in dyspepsia, remain contentious. After a comprehensive literature search, a consensus-based approach was applied, supplemented by input from international experts who reviewed the report. Three functional gastroduodenal disorders are defined. Functional dyspepsia is persistent or recurrent pain or discomfort centered in the upper abdomen; evidence of organic disease likely to explain the symptoms is absent, including at upper endoscopy. Discomfort refers to a subjective, negati

The association between non-ulcer dyspepsia and other gastrointestinal disorders.

Non-ulcer dyspepsia (NUD) is defined as dyspepsia in which investigation shows no evidence of focal gastroduodenal disease or oesophagitis. The aim of the present study was to determine the proportion of NUD patients with other identifiable diseases. We interviewed 327 consecutive patients who had at least 1 month of dyspepsia before a panendoscopy that showed no evidence of oesophagitis, malignancy, or peptic ulcer. Symptoms were assessed by a structured history questionnaire. The existence of gallstones was excluded radiologically. Of the subjects studied, 75 (23%) had irritable bowel syndro

[Belching (ructus)].

Accumulation of air in the stomach increases the gastric volume, which activates receptors in the gastric wall. This results in a reflex that relaxes the lower oesophageal sphincter, whereby the intragastric air can escape through the oesophagus. Ventilation of the stomach via the oesophagus is known as belching (ructus). Belching often occurs in combination with reflux symptoms and dyspepsia. In these cases, other symptoms are often more predominant, and it is advisable to treat these first. In patients with aerophagia, belching is the most common reason for medical consultation. These patien

Esophagoglottal closure reflex: a mechanism of airway protection.

Abrupt esophageal distention occurs commonly during gastroesophageal reflux, thereby generating a circumstance favorable to esophagopharyngeal regurgitation and laryngeal aspiration of gastric refluxate. The aims of the present study were to examine the glottal response to esophageal distention by air and regional esophageal distention by a balloon. Fifteen healthy volunteers (age, 25 +/- 5 years) were studied while they were in an upright position. Using concurrent videoendoscopy and manometry, glottal and upper esophageal sphincter (UES) responses to abrupt esophageal distention by air injec

[Therapy resistance of gastro-oesophageal reflux symptoms: acid reflux, non-acid reflux or no reflux].

The introduction of proton pump inhibitors (PPIs) was a huge step forward in the treatment of gastric acid-related disorders such as reflux disease and gastric ulcers. Despite the strong effectiveness of PPIs, in a small number of patients reflux symptoms are not adequately relieved by these drugs. The amount of acid inhibition that can be achieved using a PPI depends on a number of different factors, such as Helicobacter pylori infection, genetic variation in metabolizing enzymes, and lack of compliance. Nocturnal gastric acid breakthrough does not appear to be important in the pathogenesis o

Air swallowing caused recurrent ileus in Tourette's syndrome.

This report describes an adolescent boy who has Tourette's syndrome and developed a subtle but significant increase in vocal tics after an 8-month respite. The increase in vocal tics was associated with an acute increase in psychological stressors and resulted in recurrent air swallowing, which, in turn, caused abdominal cramping, eructation, and flatus, eventually leading to aeroenteria. Air swallowing was recognized only after a second hospital admission for recurrent ileus. Air swallowing and associated symptoms were mitigated by reinstitution of psychopharmacologic treatment and an increas

[Gastric vs. supragastric belching, singultus, aerophagia, and differential diagnoses : An interdisciplinary perspective for otorhinolaryngologists].

Belching is the act of expelling air from the stomach or esophagus into the pharynx. Although the process is regarded as physiological, excessive belching might be associated with a significant burden for affected patients in the sense of a belching disorder. Diagnosis of a belching disorder is often challenging, and its differentiation from other conditions such as rumination syndrome, singultus, or aerophagia can be difficult. Treatment of these disorders also represents a challenge for otorhinolaryngologists. Hence, the aim of this review is to provide an interdisciplinary overview of these

Roles of swallowing and belching in different phenotypes of gastroesophageal reflux disease.

BACKGROUND: The contributions of swallowing and belching to specific gastroesophageal reflux disease (GERD) phenotypes are unclear. METHODS: This study retrospectively analyzed esophageal pH/impedance studies, comparing reflux events preceded by gastric belching (GB), supragastric belching (SGB), air swallowing, and liquid/solid swallowing based on reflux position, lower esophageal sphincter (LES) pressure, and acid exposure time (AET). KEY RESULTS: 20 GERD patients and 10 controls were studied. Upright GERD patients and controls had a higher proportion of reflux events with a preceding swal

Aerophagia and subcutaneous emphysema in a patient with Rett syndrome.

A patient with Rett syndrome presented to our Emergency Department with extensive subcutaneous emphysema in the cervical region, chest wall, upper extremities, and back. Diagnostic evaluation revealed a mucosal tear in the posterior pharyngeal wall and an abscessed retropharyngeal lymph node, but she had no known history of trauma to account for these findings. This report discusses the occurrence of subcutaneous emphysema in the context of a rare neurodevelopmental disorder and proposes accentuated aerophagia, a sequela of Rett syndrome, as the most likely underlying mechanism.

[Pathologic aerophagia: a rare cause of chronic abdominal distension].

OBJECTIVE:: To describe an adolescent with pathologic aerophagia, a rare condition caused by excessive and inappropriate swallowing of air and to review its treatment and differential diagnoses. CASE DESCRIPTION:: An 11-year-old mentally impaired blind girl presenting serious behavior problems and severe developmental delay with abdominal distension from the last 8 months. Her past history included a Nissen fundoplication. Abdominal CT and abdominal radiographs showed diffuse gas distension of the small bowel and colon. Hirschsprung's disease was excluded. The distention was minimal at the mo

[Aerophagia due to noninvasive mechanical ventilation: a first manifestation of silent gastric carcinoma].

Noninvasive mechanical ventilation (NIV) techniques have proven useful in treating patients with respiratory insufficiency of various etiologies. The problems most frequently associated with this ventilatory technique are the appearance of nasal and oropharyngeal dryness, pressure sores where the nasal mask touches the skin, ocular irritation due to air leakage and epistaxis. Aerophagia appears in up to half the patients with NIV and may lead to discontinuing treatment. Drugs that accelerate gastrointestinal transit, changes in the respirator settings or changing the ventilatory modality may h
