Psychopathology factors in first-episode affective and non-affective psychotic disorders.

BACKGROUND: Since the onset, prevalence, and course of specific psychopathological features rarely have been analyzed simultaneously from the start of dissimilar psychotic illnesses, we compared symptom-clusters in first-episode DSM-IV affective and non-affective psychotic disorders. METHODS: Subjects (N=377) from the McLean-Harvard First Episode Project hospitalized for first-lifetime primary psychotic illnesses were followed prospectively for 2 years to verify stable DSM-IV diagnoses. We ascertained initial symptoms from baseline SCID and clinical assessments, applying AMDP and Bonn psychop

Characterization of affective domains within the nonaffective psychotic disorders.

OBJECTIVE: To characterize the affective dimensions of psychopathology in patients with a nonaffective psychosis and to examine their validity against a number of external variables. METHODS: Five-hundred and thirty-five patients with a lifetime DSM-IV diagnosis of nonaffective psychosis were assessed during the index episode for 22 affective symptoms. These symptoms were factor analyzed and the resulting factor scores were examined for associations with a number of risk factors, illness characteristics, index episode psychopathology and outcome variables. RESULTS: One-hundred and fifty-six

Alterations of Functional Connectivity Dynamics in Affective and Psychotic Disorders.

BACKGROUND: Patients with psychosis and patients with depression exhibit widespread neurobiological abnormalities. The analysis of dynamic functional connectivity (dFC) allows for the detection of changes in complex brain activity patterns, providing insights into common and unique processes underlying these disorders. METHODS: We report the analysis of dFC in a large sample including 127 patients at clinical high risk for psychosis, 142 patients with recent-onset psychosis, 134 patients with recent-onset depression, and 256 healthy control participants. A sliding window-based technique was u

[Interpersonal violence in the context of affective and psychotic disorders].

Some mental and neurobiological disorders are associated with an increased risk for violence against others. The stigmatization of people with mental illnesses essentially emerges from a distorted perception of this condition. This review article summarizes the available literature on the determinants, prevention, therapy and tools for prediction of serious interpersonal aggression in the context of people with mental disorders. The risks for violence against other people show substantial variation between the various diagnoses. Schizophrenia and mania carry a clearly increased risk particular

Childhood emotional and physical bullying in affective and nonaffective psychotic disorders.

OBJECTIVE: Bullying is common in childhood. Identifying developmental periods most sensitive to bullying may improve understanding of the link between bullying and psychotic disorders. METHOD: Forty-six individuals with schizophrenia spectrum disorders (schizophrenia or schizoaffective disorder; SZ), 53 with psychotic bipolar disorder (BP), and 51 healthy control (HC) participants completed the Maltreatment and Abuse Chronology of Exposures questionnaire. We tested for differences in the rates and severity of emotional and physical bullying across diagnostic groups, and also calculated odds r

Prevalence of hepatitis C in patients with non-affective psychotic disorders.

INTRODUCTION: the goal of this study was to determine the prevalence of hepatitis C virus (HCV) infection in patients with non-affective psychotic disorders and to compare it with population-based data. MATERIAL AND METHODS: an observational study was performed that measured anti-HCV antibodies (HCV-RNA in case of seropositivity) in 425 serum samples from patients with non-affective psychosis. Eight patients were positive for anti-HCV (1.9 %) and five had detectable HCV-RNA (1.2 %). The prevalence of viremia was significantly higher than in the general population (OR: 5.4; 95 % CI: 1.9-14.6).

Exploring and assessing demoralization in patients with non-psychotic affective disorders.

BACKGROUND: Demoralization, as assessed through the Diagnostic Criteria for Psychosomatic Research-Demoralization (DCPR/D) interview or the Demoralization Scale (DS), has been found to affect about 30% of patients with medical disorders, while few studies have been done in patients with psychiatric disorders. METHODS: A convenience sample of 377 patients with ICD-10 diagnoses of mood, anxiety, stress-related disorders or other non-psychotic disorders was recruited from two Italian university psychiatry centers. The DCPR/D interview and the Italian version of the DS (DS-IT) were used to assess

Analysis of TBX1 variation in patients with psychotic and affective disorders.

A significant portion of patients with 22q11 deletion syndrome (22q11DS) develop psychiatric disorders, including schizophrenia and other psychotic and affective symptoms, and the responsible gene/s are assumed to also play a significant role in the etiology of nonsyndromic psychiatric disease. The most common psychiatric diagnosis among patients with 22q11DS is schizophrenia, thought to result from neurotransmitter imbalances and also from disturbed brain development. Several genes in the 22q11 region with known or suspected roles in neurotransmitter metabolism have been analyzed in patients

Structural connectomic signatures of childhood maltreatment across affective and psychotic disorders.

Exposure to childhood maltreatment (CM) is a crucial risk factor for affective disorders and schizophrenia, which are linked to structural brain network alterations. We hypothesized a significant association between CM and brain structural connectivity in a transdiagnostic sample. Participants included patients with major depressive disorder (n = 827), bipolar disorder (n = 134), schizophrenia spectrum disorders (n = 118), and healthy controls (n = 932) aged 18-65 from the Marburg-Muenster Affective Disorders Cohort Study. Structural brain networks were reconstructed from structural and diffus

Correlates of the affective impact of auditory hallucinations in psychotic disorders.

While many who hear auditory hallucinations (AHs) experience them as unpleasant, some do not. Little is known about the correlates of AHs that are not unpleasant, or of the characteristics of those who hear them. To better understand this symptom, we used a comprehensive structured interview schedule to study 199 subjects who had experienced AHs. Subjects' responses to AHs were combined into two indexes: one assessing total affective impact and the other assessing the affective direction (positive or negative). Subjects who had grandiose delusions experienced their AHs more positively. AHs tha

Transdiagnostic modeling of clinician-rated symptoms in affective and nonaffective psychotic disorders.

Prevailing factor models of psychosis are centered on schizophrenia-related disorders defined by the Diagnostic and Statistical Manual of Mental Disorders and International Classification of Diseases, restricting generalizability to other clinical presentations featuring psychosis, even though affective psychoses are more common. This study aims to bridge this gap by conducting exploratory and confirmatory factor analyses, utilizing clinical ratings collected from patients with either affective or nonaffective psychoses (n = 1,042). Drawing from established clinical instruments, such as the Po

Reduced categorical congruence of cognitive and affective empathy in persons with psychotic disorders.

Empathy encompasses the ability to recognize the emotional states of others (cognitive empathy), adjust one’s own feelings accordingly (affective empathy), and develop the motivation to provide support (compassionate empathy). Emotional categorical congruence describes the similarity between the emotions we attribute to another person or experience ourselves in response to their situation and that person’s original emotional state. Some authors argue that categorical congruence is an essential feature of empathy, while others emphasize that similarity regarding the hedonic quality of the respe

Cannabis abuse and severity of psychotic and affective disorders in Israeli psychiatric inpatients.

The influence of cannabis abuse on the severity of existing psychotic and affective symptoms is still unclear. Among 470 consecutively admitted psychotic or affective patients, 54 active (in the previous month) cannabis abusers were detected via urine tests (Sure Step TM kits; Applied Biotech Inc, San Diego, Calif) and Structured Clinical Interview for DSM-IV (SCID- IV) questionnaire. In 24 cases, substances other than cannabis were abused; 392 patients were nonabusers. All patients were diagnosed according to the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition, criteria.

Lifetime Prevalence and Correlates of Schizophrenia-Spectrum, Affective, and Other Non-affective Psychotic Disorders in the Chinese Adult Population.

Lifetime prevalence of psychotic disorders varies widely across studies. Epidemiological surveys have rarely examined prevalences of specific psychotic disorders other than schizophrenia, and the majority used a single-phase design without employing clinical reappraisal interview for diagnostic verification. The current study investigated lifetime prevalence, correlates and service utilization of schizophrenia-spectrum, affective, and other non-affective psychotic disorders in a representative sample of community-dwelling Chinese adult population aged 16-75 years (N = 5719) based on a territor

Linking spontaneous speech, cognition, and psychopathology across affective and psychotic disorders: A network approach.

BACKGROUND: Language impairments are common in affective and psychotic disorders, yet their patterns and underlying pathomechanisms remain insufficiently understood. A transdiagnostic perspective provides a framework for identifying shared and disorder-specific language alterations across diagnostic boundaries. Combining natural language processing (NLP) with network analysis enables the investigation of complex associations between linguistic, cognitive, and psychopathological features. METHODS: Spontaneous speech from N = 372 participants (119 MDD, 27 BD, 48 SSD and 178 HC) was elicited usi

Revolving Door Phenomenon and Related Factors in Schizophrenia, Bipolar Affective Disorder and Other Psychotic Disorders.

BACKGROUND: Patients with chronic mental illness are frequently hospitalized and discharged from psychiatry wards. This situation is referred to as the "revolving door phenomenon" (RDP). In addition to factors related to the patient and the disease, limited number of beds leading to shortened hospital stay are among the reasons associated with frequent hospitalization. This study aims to compare patients with RDP and patients with single hospitalization in terms of clinical, sociodemographic, and treatment-oriented characteristics in order to evaluate the risk factors causing frequent hospital
