Military traumatic brain injury and blast.

The effects of blast on biological tissue are documented for some organ systems such as the lung. In the central nervous system (CNS) the mechanism of CNS injury following blast wave is unclear. For example is there a selective effect of blast on varying brain region or white matter bundles. The effect of blast on traumatic brain injury (TBI) has come into particular focus with the Global War on Terror and Operation Iraqi Freedom and Operation Enduring Freedom where TBI has become known as the signature injury of these conflicts. The reason for the prominence of TBI in these particular conflic

[Injured soldiers and their quality of life].

This is a commentary on the article entitled 'Dutch military casualties of the war in Afghanistan - Quality of life and level of participation after rehabilitation' by de Kruijff and colleagues, published in the Dutch Journal of Medicine (Nederlands Tijdschrift voor Geneeskunde, NTvG). The article addresses a very relevant issue: the impact of war injuries on a wounded soldier's recovery, quality of life and level of participation after rehabilitation. The choice was made to compare soldiers wounded during their mission in Uruzgan with the Dutch general trauma population by means of the 'Injur

"Einsatzchirurgie"--experiences of German military surgeons in Afghanistan.

INTRODUCTION: In 2010, the world witnessed 32 wars and other armed conflicts. Epidemiological analyses of mechanisms and patterns of injury of soldiers sent into these conflicts can be utilised to identify the surgical expertise that is required in a combat setting providing important parameters to adjust medical infrastructure and training requirements for future Military Surgeons. EXPERIENCES: Today in 2011, the German Bundeswehr runs a combat support hospital (role 3) in Mazar-e-Sharif in Northern Afghanistan providing a multidisciplinary capability profile. Furthermore, there are two role

Update: malaria, U.S. Armed Forces, 2013.

U.S. service members are at risk of acquiring malaria infection when they are present in endemic areas because of long term duty assignments, participation in shorter term contingency operations, or personal travel. The numbers of cases among service members in 2012 (n=40) and 2013 (n=30) were the lowest reported during the past ten years. In 2013 over one-third of cases were attributed to service in Afghanistan (n=11) and six cases were linked to Africa. Nine cases were caused by Plasmodium vivax; eight cases were caused by P. falciparum; and one-third were reported as "unspecified" malaria.

Evolution of Pararescue medicine during operation Enduring Freedom.

This article highlights recent advances made in U.S. Air Force Pararescue Medical Operations in relation to tactical evacuation procedures. Most of these changes have been adopted and adapted from civilian medicine practice, and some have come from shared experiences with partner nations. Patient assessment includes a more comprehensive evaluation for hemorrhage and indications for hemorrhagic control. Ketamine has replaced morphine and fentanyl as the primary sedative used during rapid sequence intubation and procedural sedation. There has been an increasing use of the bougie to clear an airw

Use of a gum elastic bougie in a penetrating neck trauma.

The case of a patient with a zone II penetrating neck injury who was intubated successfully utilizing the gum elastic bougie (GEB) is reported. He presented at a forward operational base in Afghanistan with a shrapnel wound in his neck as well as a cough and hoarseness. There were two wounds on each side of his laryngeal cartilages. The patient's breathing rate gradually increased and labored inhalation developed while the aeromedical evacuation was delayed for tactical reasons. Subcutaneous emphysema and edema concealed the anatomical landmarks, making a cricothyrotomy unsafe, and no fiber op

Anorectal injury in pelvic blast.

The signature injury of the Afghanistan campaign has, amongst other things, included an increased incidence of destructive anorectal injury. There is no significant body of evidence about this type of injury on which to base management strategies. This review examines the historical military data, later civilian reports, many of which have challenged the military dogmas of Vietnam, and the spartan contemporaneous military data which does not particularly address pelviperineal blast injury. There is no evidence to support a move away from the doctrine of the four D's (diversion, distal washout,

Primary Blast Lung Injury: The UK Military Experience.

INTRODUCTION: Primary blast lung injury occurs when an explosive shock wave passes through the thorax and transits through tissues of varying densities. It requires close proximity to an explosion and presents quick with respiratory distress in survivors. MATERIALS AND METHODS: The Joint Theatre Trauma Registry and the Defence Statistics (Health) Database were interrogated for casualties injured as a result of an explosion during the conflict in Afghanistan. The case notes and imaging of casualties meeting the criteria for diagnosis were reviewed. Demographic and clinical data on casualties w

Repeated Mild Concussive Events Heighten the Vulnerability of Brain to Blast Exposure.

Mild concussive events without loss of consciousness are typically left untreated and can result in neurological abnormalities at later stages of life. No systematic studies have been carried out to determine the effect of concussion or repeated mild concussive episodes on brain vulnerability towards blast exposure. We have evaluated the effect of repeated mild concussive events on the vulnerability of brain to blast exposure using neurobehavioral functional assessments. Rats were subjected to either repeated mild concussive impacts (two impacts 1 week apart using a modified Marmarou weight dr

Association of Premilitary Mental Health With Suicide Attempts During US Army Service.

IMPORTANCE: Approximately one-third of US soldiers who attempt suicide have not received a mental health diagnosis (MH-Dx) before their suicide attempt (SA), yet little is known about risk factors for SA in those with no MH-Dx. OBJECTIVE: To examine whether premilitary mental health is associated with medically documented SA among US Army soldiers who do not receive an MH-Dx before their SA. DESIGN, SETTING, AND PARTICIPANTS: This cohort study used data from a representative survey of soldiers in the US Army entering basic combat training from April 1, 2011, to November 30, 2012, who were fo

Norwegian male military veterans show low levels of mental health problems four years after deployment in Afghanistan.

BACKGROUND: Deployment of soldiers is associated with considerable rates of mental disorders after redeployment. AIMS: The aim of this study was to identity prevalence of probable mental disorders and associated factors among male Norwegian soldiers at a mean of 4 years following deployment to Afghanistan in 2001-2011. METHODS: The retrospective questionnaire-based study invited all Norwegian veterans deployed to Afghanistan from 2001-2011 to participate. The response rate was 59%, but only the 3403 men (47.5%) who completed all items of the four different screening instruments were used to

Missions, Training and Equipment: A Retrospective View from the 160th SOAR (A).

On 3 October 2001, the first chalk of Night Stalkers left Campbell Army Airfield enroute to Uzbekistan in support of what would become our nation?s longest war. The vast majority of Soldiers were untested in war and would quickly have the opportunity to meet with the enemies of our nation in close combat. The two Special Operations Task Forces (TF), TF Dagger to the north in Uzbekistan and TF Sword in the south (aboard the USS Kitty Hawk) were our nation?s first strike options against Al Qaeda and Taliban forces in Afghanistan. Due to the inherent nature of forced entry operations (restrictive

Psychological Stress-Induced Immune Response and Risk of Alzheimer's Disease in Veterans from Operation Enduring Freedom and Operation Iraqi Freedom.

PURPOSE: Psychological stress is a significant health problem in veterans and their family members. Traumatic brain injury (TBI) and stress lead to the onset, progression, and worsening of several inflammatory and neurodegenerative diseases in veterans and civilians. Alzheimer's disease (AD) is a progressive, irreversible neuroinflammatory disease that causes problems with memory, thinking, and behavior. TBIs and chronic psychological stress cause and accelerate the pathology of neuroinflammatory diseases such as AD. However, the precise molecular and cellular mechanisms governing neuroinflamm
