Effect of a monetary sanction on immunization rates of recipients of aid to families with dependent children.

CONTEXT: Immunization rates among low-income families have lagged behind those for the general community, with several possible barriers cit … Georgia. PARTICIPANTS: A total of 2500 families with children aged 6 years or younger who received Aid to Families with Dependent Children assistance. INTERVENTION: Families in the intervention group (n=1500) were informed that receipt of the welfare benefit for any preschool-aged children was contingent on provision of proof of up-to-date immunization status at the beginning of welfa …

The effect of parental monetary sanctions on the vaccination status of young children: an evaluation of welfare reform in Maryland.

OBJECTIVE: To determine whether financial sanctions to Aid to Families With Dependent Children (AFDC) recipients can be used to improve vaccination coverage of young children. DESIGN: Randomized controlled trial. SETTING: Six AFDC jurisdictions in Maryland. INTERVENTION: Recipients of AFDC were randomized to the experimental or control group of the Primary Prevention Initiative. Families in the experimental group were penalized financially for failing to verify that their children received preventive health care, including vaccinations; control families were not. PARTICIPANTS: Children age…

A longitudinal perspective on entitlement income among homeless adults.

OBJECTIVE: Sources of entitlement income were examined in a sample of homeless adults to determine whether certain subgroups more consistently obtain entitlement income and are more likely to continue receiving it over time. METHODS: From a baseline sample of 564 homeless residents of Alameda County, California, 397 were interviewed at both five- and 15-month follow-ups. Information was obtained on income received from public sources in the 30 days before each interview, including general assistance, Aid to Families With Dependent Children (AFDC), Supplemental Security Income, or Social Secur…

Pregnancy risk among the younger sisters of pregnant and childbearing adolescents.

There is increasing evidence that younger sisters of childbearing teenagers are at increased risk for adolescent childbearing. We critically review this research and discuss three plausible theoretical explanations (social modeling, shared parenting influences, and shared societal risk) why the younger sisters of childbearing adolescents would themselves be at risk for teenage pregnancy. Considerations for preventive interventions aimed at the younger sisters of pregnant teenagers and directions for future research are discussed.

A profile of children with disabilities receiving SSI: highlights from the National Survey of SSI Children and Families.

This article provides a nationally representative profile of noninstitutionalized children 0 to 17 years of age who were receiving support from the Supplemental Security Income (SSI) program because of a disability. To assess the role of the SSI program in providing assistance to low-income children with disabilities and their families, it is important to obtain detailed information on demographic characteristics, income and assets, health and disabilities, and health care utilization. Yet administrative records of the Social Security Administration do not contain many of the relevant data ite

Vaccination status of children in the Women, Infants, and Children (WIC) Program: are we doing enough to improve coverage?

BACKGROUND: Vaccination-promoting strategies in the Supplemental Nutrition Program for Women, Infants, and Children (WIC) have been shown to produce dramatic improvements in coverage and other health outcomes. OBJECTIVES: To determine national and state-specific population-based vaccine coverage rates among preschool children who participate in the WIC program, and to describe the strategies for promoting vaccination in WIC. DESIGN/METHODS: Demographic data, WIC participation, and vaccination histories for children aged 24 to 35 months in 1999 were collected from parents through the National

The links between types of maltreatment and demographic characteristics of children.

This paper examines the relative impact of demographic characteristics of the child, family structure, and economic variables on types of ch … dictable and distinguishable. It is most clearly related to economic factors such as low income and Aid to Families with Dependent Children (AFDC) status, regardless of race. Additionally, both sexual abuse and physical neglect occur at younger ages than previously shown. The policy implications for these findings are discussed.

Child Only Kinship Care Cases:The Unintended Consequences of TANF Policies for Families Who Have Health Problems and Disabilities.

The passage of the Personal Responsibility Work Opportunities Reconciliation Act (PRWORA) in 1996 changed the way America sought to help its most vulnerable citizens. The Temporary Assistance for Needy Families (TANF) legislation required families to work first. Many families with young children found the stringent requirements of TANF too restrictive for their needs. In this study a number of TANF customers were either physically ill or suffered mental health problems thus preventing them from finding suitable employment. Additionally they may have children who have health problems, which pre

Use of vitamin-mineral supplements by AFDC children.

Slightly more than 11 percent of the 1,616 children in Northern Mississippi households receiving Aid to Families with Dependent Children regularly used vitamins, according to the 540 personal interviews conducted in this study. Of the vitamins used, about 20 percent were obtained by prescription. Participation in Early and Periodic Screening, Diagnosis and Treatment (EPSDT) was found not to be related to vitamin use. The pharmacy was the main source of vitamins, which most frequently were those widely advertised on television. It is suggested that pharmacists, physicians, and EPSDT personnel m…

Trajectories of maltreatment re-reports from ages 4 to 12:: evidence for persistent risk after early exposure.

This study identified trajectories of maltreatment re-reports between ages 4 and 12 for children first referred to Child Protective Services … biological/stepparent; caregiver alcohol abuse, depression, and lack of social support; receipt of Aid to Families with Dependent Children (AFDC); and number of children in the home. For a subpopulation of high-risk children first reported in early childhood, risk for maltreatment re-reporting may persist longer than previously documented, continuing 8 to 12 years after the first repor …

The impact of public assistance factors on the immunization levels of children younger than 2 years.

OBJECTIVES: This study examined how children's immunization status varied with enrollment in the Women, Infants, and Children (WIC); Aid to Families with Dependent Children (AFDC); food stamp; and Medicaid programs. METHODS: A statewide survey was used to determine the percentage of children less than 2 years of age who were up to date for diphtheria, tetanus, and pertussis; polio; and measles, mumps, and rubella vaccines. RESULTS: WIC and uninsured children were more likely and AFDC and Medicaid children less likely to be up to date than others. CONCLUSIONS: The higher immunization status …

A comparison of ambulatory Medicaid claims to medical records: a reliability assessment.

This study compares the documentation of ambulatory care visits and diagnoses in Medicaid paid claims and in medical records. Data were obtained from Maryland Medicaid's 1988 paid claims files for 2407 individuals who were continuously enrolled for the fiscal year, had at least one billed visit for one of six indicator conditions, and had received the majority of their care from one provider. The patients sampled were also stratified on the basis of the case-mix adjusted cost of their usual source of care. The medical records for these individuals as maintained by their usual source of care we

Social and economic hardships of homeless and other poor women.

Homeless women generally face the same social and economic problems as do men but in an intensified form due to the social marginalization of women and the responsibilities of parenthood, often borne by women alone. Remedies that entail changes in public policy and the enhancement of social-support networks are discussed.

Infants and young children in orphanages: one view from pediatrics and child psychiatry.

A large body of medical knowledge exists that can inform the public policy debate as to whether the current needs and future life prospects of poor children could better be served in orphanages than by continuing safety net programs, such as Aid to Families with Dependent Children, Medicaid, and Supplemental Social Security Income, which maintain children in families. This special article explores a century of pediatric and child psychiatry research covering five areas of potential biologic and social risk to infants and young children in orphanage care: (1) infectious morbidity, (2) nutrition…

Welfare reform consequences for children: the Wisconsin experience.

BACKGROUND: The Temporary Assistance to Needy Families, enacted under the Personal Responsibility and Work Opportunity Reconciliation Act of 1996, is a reality for many working families. As public policies are enacted, unintended consequences for infants/children must be minimized. Child advocates in Wisconsin, leading this nation in reforming Aid to Families with Dependent Children (AFDC), are concerned about supporting eligible infants/children as safety-net programs are unlinked. OBJECTIVE: This study reviews the enrollment status of 4 linked programs over time in Wisconsin, from January 1…
