BACKGROUND: Endovascular thrombectomy (EVT) is limited to comprehensive stroke centers (CSCs), with patients who first present to primary stroke centers (PSCs) transferred for treatment by road ambulance or helicopter emergency medical services (HEMS). We aimed to determine if there were outcome differences in patients transferred for EVT via HEMS.
METHODS: A retrospective observational study was performed in the setting of a large regional network with 1 CSC receiving patients directly, or via road ambulance from 3 metropolitan PSCs or HEMS from 13 more distant PSCs. Consecutive patients wer

Air ambulances rapidly transport burn patients to regional centers, expediting treatment. However, limited guidelines on transport introduce the risk for inappropriate triage and overuse. Given the additional costs of air vs ground transport, evaluation of transportation use is prudent. A retrospective review of all burn patients transported by helicopter to a single burn center from May 2013 to January 2016 was performed. Data gathered included patient demographics, transfer origin, burn characteristics, and inpatient hospital stay. The primary outcome was appropriate triage based on literatu

Introduction: Each aspect of prehospital emergency care places a different demand on the physical and mental health of workers. Objective: We aimed to compare the stress and other risk factors of ground, air, water ambulance workers. Method: A cross-sectional study was carried out in August–October 2022. Nonrandom, convenience data sampling was conducted on a voluntary basis, using an anonymous, online questionnaire at the ambulance stations of the Hungarian National Ambulance Service (n = 70), the bases of the Hungarian Air Rescue Nonprofit Ltd. (n = 24) and the Hungarian Water Rescue Service

BACKGROUND: Outpatient or ambulatory treatment for prolonged air leak (PAL) has been reported previously in various studies. Evidence regarding efficiency and safety is nevertheless poor. This report describes the experience of 10 years ambulatory care with a digital chest drain system monitored by specialized nurses in our centre. The aim of the study is to give further insights in the effectiveness and safety of this treatment.
METHODS: Retrospective data of 10 years ambulatory care for PAL were examined. One hundred and forty patients with PAL after pneumothorax or pulmonary surgery were i

Advanced supportive therapy at the site of the accident, associated with direct transfer to a trauma centre increases survival and reduces morbidity rates. Patients with severe head injury, especially those with multiple injuries, often arrive in the emergency department with potentially causes of serious secondary systemic insults to the already injured brain, such as acute anemia (Hematocrit < or = 30%), hypotension (systolic arterial pressure (Pasys) < or = 95 mmHg, 12.7 kPa), hypercapnia (Paco2 > or = 45 mmHg, 6 kPa) and/or hypoxemia (Pao2 < or = 65 mmHg, 8.7 kPa). OBJECTIVE: Interfacility transports by helicopter air ambulance (IFT-HAAs) are requested for critically ill and injured patients. Requests for patient transport may originate from several settings within the referring facility: the emergency department (ED), procedural areas (such as a cardiac catheterization laboratory), or inpatient units such as an intensive care unit. Existing literature has not clarified the contributions of the ED compared with other facility settings.
METHODS: In this retrospective modeling study, we used data from the Federal Aviation Administration, National Hospital

OBJECTIVE: Powered 2-wheeler (PTW) riders are a highly vulnerable road-user group who frequently require helicopter emergency medical services (HEMS) for critical care and transport. This study aimed to describe the population of injured PTW riders attended by the Danish Air Ambulance and compare them with the wider Danish HEMS trauma population.
METHODS: This was a retrospective population-based study of Danish HEMS missions from January 1, 2018, to December 31, 2023, including all primary missions involving a PTW patient. Data were collected from the Danish HEMS database on patient demograp

A number of systematic reviews have investigated the association between air pollutants and health impacts, these mostly focus on morbidity and mortality from hospital data. Previously, no reviews focused solely on ambulance dispatch data. These data sets have excellent potential for environmental health research. For this review, publications up to April 2019 were identified using three main search categories covering: ambulance services including dispatches; air pollutants; and health outcomes. From 308 studies initially identified, 275 were excluded as they did not relate to ambulance servi

Many countries have adopted the use of helicopter emergency medical services (HEMS). One such system in Japan is called "Doctor Heli" (Doctor Helicopter), which is sometimes used as a means of maternal transport. Since 2008, there have been 18 cases of Doctor Heli maternal transport to Fukushima Medical University Hospital, the ultimate referral center of Fukushima Prefecture. Reasons for transport included umbilical cord prolapse (UCP), placental abruption, traffic accident, and fetal bradycardia. There were two cases of intrauterine fetal death upon arrival at the hospital. Although there is

Ambulances are enclosed environments that carry a high risk of airborne and surface microbial transmission, yet effective disinfection technologies remain limited. This study evaluated four photocatalytic oxidation (PCO) configurations-O₃+UVA + TiO₂, UVA + TiO₂, O₃+UVC + ZnO, and UVC + ZnO-against B. subtilis spores. The testing employed a prototype air purification system for ambulance applications, where the photocatalyst TiO2 or ZnO was integrated into a filter medium. This system operated in combination with its corresponding UV light source (UVA or UVC) and an optional ozone generator; al

INTRODUCTION: Disinfecting contaminated ambulances presents a challenge to emergency medical services. The process relies on a manual cleaning step or the use of foggers and gas generators. Equipment requires training, power supply, and access to reagent refills. Chlorine dioxide (ClO2) gas is an affordable, easy-to-use alternative.
METHODS: We field-tested ClO2 dry inside an ambulance against a surrogate for SARS-CoV-2. MS2 bacteriophage was aerosolized inside of an enclosed. Airborne MS2 concentrations were measured before and after ClO2 applications to compare reductions in viable virus. C

A circadian variation in the cardiovascular parameters has been detected. It is plausible that the influence of the environment varies during different periods of the day. We investigated the association between daily emergency ambulance calls (EC) for paroxysmal atrial fibrillation (AF) that occurred during the time intervals of 8:00-13:59, 14:00-21:59, and 22:00-7:59, and weather conditions and exposure to CO and PM10. We used Poisson regression to explore the association between the risk of EC for AF and environmental variables, adjusting for seasonal variation. Before noon, the risk was as

The article is devoted to an analysis of main causes of the development of acute gastroduodenal bleedings, methods of surgical treatment in small town and district hospitals with participation of consulting surgeons of aerial ambulance. Postoperative outcomes are also considered. The authors made a conclusion that active surgical management and performing primary radical operations on the spot allowed lethality to be reduced to 6.6%. However, the optimum policy is thought to be transportation of the patients to specialized regional medical institutions after temporary adequate endoscopic arres

The ventricular assist device (VAD) is a hemodynamic support device that augments cardiac output for patients with severe ventricular dysfunction. With improved reliability and technological advances, the use of VADs to support patients is increasing. Many VAD-dependent patients ultimately require heart transplants that are only available in specialized centers, necessitating an interhospital transfer. To date there are few reports of long-distance fixed wing aeromedical transport of patients dependent on a VAD. Here we describe the successful transfer of a patient supported by a biventricular

OBJECTIVE: In this article, we describe how we developed and validated key performance indicators (KPIs) for pre-hospital blood transfusion and offer suggestions for other organisations wishing to develop performance metrics.
BACKGROUND: KPIs are metrics that compare actual care against an ideal structure, process or outcome standard. An increasing number of UK-based pre-hospital critical care services now carry blood components to enable pre-hospital blood transfusion.
METHODS: A working group of pre-hospital physicians and paramedics was formed to create and validate performance indicators
