Pediatric airway management.

PURPOSE OF REVIEW: Children are at risk of severe hypoxemia in the perioperative period owing to their unique anatomy and physiology. Safe and effective airway management strategies are therefore key to the practice of pediatric anesthesia. The goal of this review is to highlight recent publications (2019-2021) aimed to advance pediatric airway safety and to highlight a proposed simple, pediatric-specific, universal framework to guide clinical practice. RECENT FINDINGS: Recent investigations demonstrate that infants with normal and difficult airways experience high incidences of multiple lary

Neonatal Airway Management.

Safe and effective airway management of neonates requires unique knowledge and clinical skills. Practitioners should have an understanding of neonatal airway anatomy and respiratory physiology and their clinical implications related to airway management. It is vital to recognize the potential sequelae of prematurity. Clinicians should be familiar with the skills and techniques available for managing normal neonatal airways. This review provides stepwise considerations for managing the neonatal airway: specific considerations for neonatal airway management, assessment and preparation, induction

Trauma airway management.

BACKGROUND: Airway management in a trauma patient can be particularly challenging when both a difficult airway and the need for rapid action collide. The provider must evaluate the trauma patient for airway difficulty, develop an airway management plan, and be willing to act quickly with incomplete information. DISCUSSION: Thorough knowledge of airway management algorithms will assist the emergency physician in providing optimal care and offer a rapid and effective treatment plan. CONCLUSIONS: Using a case-based approach, this article reviews initial trauma airway management strategies along

Difficult airway management.

Airway management is unequivocally the most important responsibility of the emergency physician. No matter how prepared for the task, no matter what technologies are utilized, there will be cases that are difficult. The most important part of success in the management of a difficult airway is preparation. When the patient is encountered, it is too late to check whether appropriate equipment is available, whether a rescue plan has been in place, and what alternative strategies are available for an immediate response. The following article will review the principles of airway management with an

Acute airway management.

Acute airway management has challenged clinicians for nearly four millennia. History underscores the discoveries of surgeons and anesthesiologists, whose advances in technology and surgical technique have transformed management algorithms from primarily surgical tracheostomy, to transoral endotracheal intubation under direct laryngoscopy. Despite this progress and a better understanding of airway anatomy, physiology and pathogenesis of disease, the acute airway, whether obstructed, traumatically disrupted, or externally compressed, remains a life-threatening challenge. The role of all clinicia

Cuffed oropharyngeal airway for difficult airway management.

Difficulties with airway management are often caused by anatomic abnormalities due to previous oral surgery. We performed general anesthesia for a patient who had undergone several operations such as hemisection of the mandible and reconstructive surgery with a deltopectoralis flap, resulting in severe maxillofacial deformation. This made it impossible to ventilate with a face mask and to intubate in the normal way. An attempt at oral awake intubation using fiberoptic bronchoscopy was unsuccessful because of severe anatomical abnormality of the neck. We therefore decided to perform retrograde

Emergency airway management with laryngeal mask airway.

When used correctly, laryngeal mask airway is a life saver. We report two cases wherein it was used for rescue airway management and as a ventilatory device during anesthesia. It is concluded that an anesthetist should be familiar with the use of this device.

The laryngeal mask airway: tool for airway management.

The laryngeal mask airway (LMA) is possibly the most significant recent advance in airway management. Designed to be passed into the hypopharynx without a laryngoscope, the LMA was conceived after bioengineering principles were applied to postmortem specimens of the adult larynx. The LMA has gained widespread, worldwide popularity and was approved for use in the United States in 1992. Sound knowledge of this device is important for all health care workers involved in airway management because it may become a standard item of resuscitation equipment when intubation is difficult. Nurses in the P

UPPER AIRWAY BLOCKS FOR AWAKE DIFFICULT AIRWAY MANAGEMENT.

Airway anesthesia is pivotal for successful awake intubation provided either topically or by blocks. Airway blocks are considered technically more difficult to perform and carry a higher risk of complications. However, in experienced hands, they can be useful as they provide excellent intubating conditions. For complete upper airway anesthesia, bilateral glossopharyngeal and superior laryngeal nerve blocks with translaryngeal injection are required. Superior laryngeal nerve block and translaryngeal injection can be performed easily, safely and with a high success rate in patients with normal a

Upper airway diseases and airway management: a synopsis.

This article summarizes some of the more important upper airway conditions likely to affect airway management. A number of upper airway conditions may present difficult challenges to the anesthesiologist. For instance, infected airway structures may lead to partial airway obstruction, stridor, or even complete airway obstruction. Partial airway obstruction may be mild, as in snoring or nasal congestion, or may be more severe, perhaps requiring the use of airway adjuncts, such as a nasopharyngeal airway. Complete airway obstruction is usually managed by prompt intubation, but surgical airways a

Ultrasonography for airway management.

Ultrasonography (USG) allows a new approach to the airway in anaesthesia and intensive care. USG visualises the airway from the mouth to the lungs. By exploring the entire airway, USG proposes new criteria (1) to assess the risk of difficult laryngoscopy, (2) to anticipate the management of a difficult airway, (3) to confirm the position of the endotracheal tube (ETT), and (4) to confirm that the lungs are effectively ventilated. Intraoperatively, USG may also help to resolve acute ventilatory problems such as pneumothorax, delayed selective bronchial intubation after patient positioning (Tren

Airway management in patients.

Stridor is caused by turbulent flow through a narrow airway. Common causes of stridor are deep neck infection, malignancy, angioneurotic oedema, trauma and congenital anomalies like laryngomalacia. The majority of these conditions can progress rapidly to complete airway obstruction. These conditions are traditionally managed with one of the following techniques: inhalational induction, awake tracheostomy, awake fibreoptic intubation or blind nasal intubation. The success and safety of each of these techniques continues to be debated.

AIRWAY MANAGEMENT IN NEUROANESTHESIA.

Neurosurgical patients have specific airway management. Various conditions and diagnoses make intubation difficult and may also cause neurological damage. Spinal pathology, neurotrauma, cervical spine surgery, and pituitary gland surgery are just some examples. The aim of this review article is to present a broad spectrum of neurosurgical operations and potential complications in maintaining airway patency related to these issues. Quality perioperative preparation is a prerequisite to avoid the potentially irreversible consequences of difficult airways with a poor neurological or even fatal ou

Complications of airway management.

Although endotracheal intubation is commonly performed in the hospital setting, it is not without risk. In this article, we review the impact of endotracheal intubation on airway injury by describing the acute and long-term sequelae of each of the most commonly injured anatomic sites along the respiratory tract, including the nasal cavity, oral cavity, oropharynx, larynx, and trachea. Injuries covered include nasoseptal injury, tongue injury, dental injury, mucosal lacerations, vocal cord immobility, and laryngotracheal stenosis, as well as tracheomalacia, tracheoinnominate, and tracheoesophag

Airway management in trauma.

Trauma has assumed epidemic proportion. 10% of global road accident deaths occur in India. Hypoxia and airway mismanagement are known to contribute up to 34% of pre-hospital deaths in these patients. A high degree of suspicion for actual or impending airway obstruction should be assumed in all trauma patients. Objective signs of airway compromise include agitation, obtundation, cyanosis, abnormal breath sound and deviated trachea. If time permits, one should carry out a brief airway assessment prior to undertaking definitive airway management in these patients. Simple techniques for establishi
